Medical Necessity and Coverage Criteria

Initial transplant medical necessity

Covered when ALL of the following are met:

Overall coverage

A. Indication: Member/enrollee has one of: (1) diagnosis of diabetes mellitus requiring insulin (members with insulin requirements >1 unit/kg should be closely evaluated), (2) diagnosis of exocrine pancreatic insufficiency, (3) requirement for procurement or transplantation of a pancreas as part of a multiorgan transplant for technical reasons.
B. Absence of contraindications: Does not have ANY of the listed contraindications (items 1–14) including malignancy with high risk of recurrence or death related to cancer; GFR <40 mL/min/1.73m2 unless being considered for multiorgan transplant; recent stroke/acute coronary syndrome or myocardial infarction within specified timeframes; acute liver failure or cirrhosis with portal hypertension unless multiorgan transplant; septic shock; active uncontrolled infection with highly virulent/resistant microbes; active tuberculosis; HIV with detectable viral load unless specified conditions are met; progressive cognitive impairment; inability to adhere to regimen despite caregiver support; active substance use or dependence (including current tobacco/vaping/marijuana use unless prescribed) without evidence of risk reduction; chronic non-healing wounds; significant comorbidities limiting survival; other severe uncontrolled medical conditions expected to limit survival.
C. Procedure-specific criteria
- PTA: All: recurrent, severe, and potentially life‑threatening metabolic complications requiring medical attention (including severe hypoglycemia unawareness; recurring severe hypoglycemic attacks; marked hyperglycemia; recurring severe ketoacidosis); clinical or clinical and emotional problems with exogenous insulin therapy that are incapacitating or failure of insulin-based management to prevent acute complications; and medical management by an endocrinologist for at least 12 months.
- SPK: All: meets PTA criteria; end‑stage renal disease (presence of uremia and requires dialysis or is expected to require dialysis within 12 months); and GFR ≤ 20 mL/min or CrCl < 20 mL/min.
- PAK: All: meets PTA criteria; prior successful kidney transplant without significant chronic rejection; and stable kidney transplant function defined by stable CrCl ≥ 30 mL/min and absence of significant proteinuria.

Autologous islet transplantation

Autologous islet cell transplantation is considered medically necessary when performed as an adjunct to total or near‑total pancreatectomy for severe, refractory chronic pancreatitis.

Pancreas retransplantation

Covered when ALL of the following are met:

Pancreas retransplantation is medically necessary after one failed primary pancreas transplant.

Not medically supported

Not supported for the following:

Not supported indications: Re‑transplantation after two or more failed primary pancreas transplants; allogeneic islet cell transplantation or xenotransplantation; SPK for patients whose indication is amputation due to peripheral obstructive vascular disease; and other conditions not specified as covered in this policy.

Requests for pancreas re-transplantation after two or more failed primary pancreas transplants are also listed as not supported; by contrast, retransplantation after one failed primary pancreas transplant is considered medically necessary when other criteria are met. Autologous islet cell transplantation is addressed separately in the policy and is considered medically necessary only as an adjunct to total or near-total pancreatectomy for severe, refractory chronic pancreatitis (see policy criteria).

The policy’s cardiac-related exclusion wording was revised: a history of myocardial infarction within 6 months prior to evaluation is exclusionary (previously specified as 30 days), and the policy language concerning recent stroke or acute coronary syndrome was also reworded. These timing-based exclusions are operational criteria and should be verified and documented in the clinical record when submitting requests for pancreas transplant authorization.

CPT codes 0584T–0586T (islet cell transplant codes) are specifically listed under ‘CPT/HCPCS Codes that do not support coverage criteria’ and therefore do not support coverage under this policy. Providers billing or requesting authorization for islet cell transplant procedures using these CPT codes should expect them to be considered not medically necessary per this policy.

The policy separately addresses autologous islet transplantation (covered as an adjunct to pancreatectomy for chronic pancreatitis) using other procedure codes; the listed 0584T–0586T codes remain categorized as not supporting coverage.

Contraindications to Transplant

Contraindications to pancreas transplantation include malignancy that confers a high risk of cancer recurrence or death and other conditions expected to limit post-transplant survival.
