2/8/24 - For Commercial Plan added new coverage criteria #5: Failure of one preferred viscosupplement: Synvisc, Synvisc-One, or Euflexxa; preferred agents do not require prior authorization when administered preferred.
2/4/25 - For Commercial Plan added SynoJoynt as an eligible viscosupplementation treatment option when criteria are met.
