OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • Testing is medically necessary for clinical management of a patient receiving controlled substances or being treated for a substance use disorder.
    • Testing is ordered by the treating clinician and supported by documented medical necessity.

    Exceptions to Definitive-Testing Noncoverage

Definitive confirmatory or quantitative testing is not covered unless at least one exception applies and the applicable requirements are met.

    At least one exception

    • Precise identification of a drug or metabolite is necessary to confirm adherence, misuse, diversion, or exposure to non-prescribed or illicit substances.
    • ALL of the following

      • The presumptive test result is positive.
      • Definitive testing is limited to substances identified as present or positive on the presumptive test.
      • The definitive test is ordered within 24 hours of the presumptive test.
    • ALL of the following

      • The presumptive test result is negative.

    Medicare Advantage Definitive-Testing Exclusions

PurposeVerifies or refutes initial screening results.
Specificity and sensitivityMore specific and sensitive than presumptive testing.
What it identifiesIdentifies specific drugs, drug concentrations, and associated metabolites.
Processing and methodsResults typically take longer because specimens are processed in a laboratory; methods may include GC/MS or LC/MS.

Use presumptive testing for initial screening

Use presumptive qualitative testing as the initial screening tool to detect prescribed, non-prescribed, or illicit substances.

    • CPT 80305, 80306, and 80307 are the listed presumptive testing codes.
    Denial Risk

    Apply definitive-testing exception criteria

Definitive confirmatory or quantitative drug testing is not covered unless a stated exception applies. Do not expect reimbursement without meeting the applicable exception criteria.

    • The listed definitive-testing codes are G0480, G0481, G0482, G0483, and G0659.
    Documentation Required

    Establish necessity for definitive testing

For definitive testing to qualify for reimbursement, precise identification of a drug or metabolite must be necessary to confirm adherence, misuse, diversion, or exposure to non-prescribed or illicit substances.

    Billing Rule

Meet definitive-testing timing and scope rules

Follow the presumptive-result, timing, and scope requirements when ordering definitive testing. After a positive presumptive result, test only substances identified as present or positive and order definitive testing within 24 hours; after a negative result, order it within 24 hours only when the finding is inconsistent with the individual’s medical history or behavior.

    Denial Risk

Avoid Medicare Advantage definitive CPT exclusions

For Medicare Advantage claims, do not bill the listed individual definitive drug-testing CPT codes or ranges because they are not covered and may result in inappropriate payment when individual tests are billed instead of a single comprehensive code.
