Coverage Criteria for TPN and IDPN

Total Parenteral Nutrition (TPN) — Initial coverage criteria

Covered when ALL of the following are met

TPN initial criteria: 1) Documentation of nutritional insufficiency in the absence of TPN, as evidenced by: (a) adults: involuntary weight loss of 10% of usual body weight within 6 months or 5% within 1 month; (b) children: weight-for-length, weight-for-height, or sex < 10th percentile, or inadequate weight gain or a significant decrease in usual growth percentile; (c) neonates: extremely low birth weight <1000 g; AND 2) Evidence of structural or functional bowel disease or a non-functioning gastrointestinal tract that makes oral or tube feedings inappropriate, including but not limited to: Crohn's disease; short bowel syndrome; single or multiple fistulae (enterocolic, enterovesical, or enterocutaneous); obstructing stricture; motility disorder; newborn GI anomalies preventing oral feedings (e.g., tracheoesophageal fistula, gastroschisis, omphalocele, massive intestinal atresia); infants/young children failing to thrive from cardiac or respiratory disease, short bowel syndrome, malabsorption, or chronic idiopathic diarrhea; paralytic ileus or prolonged paralytic ileus post-major surgery or multiple injuries; radiation enteritis; liver failure in children approved for transplant who fail to grow on enteral support; liver failure in adults with hepatic encephalopathy intolerant of standard amino acids (for liver-specific amino acid mixtures); acute necrotizing pancreatitis in adults with inadequate oral intake >1 week where enteral feeds exacerbate symptoms; necrotizing enterocolitis; small bowel ischemia.

Intradialytic Parenteral Nutrition (IDPN) — Initial coverage criteria

Covered when ALL of the following are met

IDPN initial criteria: 1) Meets TPN criteria in section A; AND 2) Member/enrollee has stage 5 chronic kidney disease; AND 3) Member/enrollee is undergoing hemodialysis; AND 4) IDPN is offered as an alternative to regularly scheduled TPN.

The policy lists specific pediatric and other nuanced exclusions for TPN. TPN is considered not proven safe and effective for: (1) children who were previously well nourished or only mildly malnourished who are undergoing oncologic treatment associated with a low nutrition risk (for example, less advanced disease, less intense cancer treatments, or disease in remission during maintenance therapy); (2) members/enrollees with advanced cancer unresponsive to chemotherapy or radiation therapy; and (3) members/enrollees for whom liver transplantation is not feasible and prognosis will not change with TPN.

The policy explicitly states that IDPN is not covered when offered in addition to regularly scheduled TPN. It also specifies that IDPN provided for patients with acute kidney injury who do not have end‑stage renal disease (ESRD) is considered not proven safe and effective and is excluded from coverage.

State and federal payer rules take precedence over this clinical policy where they conflict. For Medicaid members/enrollees, applicable state Medicaid coverage provisions override the policy; providers should consult the state Medicaid manual for relevant guidance. For Medicare members/enrollees, applicable National Coverage Determinations (NCDs), Local Coverage Determinations (LCDs), and Medicare Coverage Articles must be reviewed and applied prior to using this policy's criteria.

The policy identifies specific use cases that are considered not proven safe and effective and therefore are excluded from coverage. These include: TPN for children previously well nourished or mildly malnourished undergoing low nutrition risk oncology therapy; TPN for members with advanced cancer unresponsive to therapy; TPN when liver transplantation is not feasible and prognosis will not change with TPN; and IDPN when provided as an addition to regularly scheduled TPN or for acute kidney injury without ESRD. See the exclusions section for details when submitting authorization requests.

Documentation, Prior Authorization, and Billing Guidance

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for Total Parenteral Nutrition (TPN) and Intradialytic Parenteral Nutrition (IDPN). Initial approval duration for both TPN and IDPN is three months. Continued approval duration is six months if there is no evidence of unacceptable complications and documentation supports a positive response to therapy.

    • Initial approval: 3 months for TPN and IDPN
    • Continued approval: 6 months if tolerated and clinically improved
    Documentation Required

Prior Authorization Guidance and Compliance Obligations

This clinical policy is intended as a guide to medical necessity and to assist in coverage decisions; it does not replace provider judgment. Providers must follow program-specific prior authorization processes and comply with all plan terms, exclusions, limitations, and state/federal requirements. State Medicaid provisions supersede this policy when conflicts exist; review applicable Medicare NCDs/LCDs for Medicare members.

    Background and Rationale

Parenteral nutrition provides macronutrients and micronutrients intravenously when enteral feeding is inadequate or inappropriate. Total Parenteral Nutrition (TPN) supplies complete nutritional needs via peripheral or central venous access depending on expected duration and clinical circumstances. Intradialytic Parenteral Nutrition (IDPN) is administered during hemodialysis sessions for patients with stage 5 chronic kidney disease on maintenance hemodialysis who continue to lose weight or have persistently low albumin despite oral supplementation; IDPN can deliver nutrients without placing additional venous access. Providers should follow applicable payer, state Medicaid, and Medicare NCD/LCD requirements when considering parenteral nutrition.

    • Medicaid state provisions take precedence over this policy when in conflict
    • Refer to Medicare NCDs/LCDs/CMS guidance for Medicare members
    Documentation Required

    Required Clinical Documentation

Document the clinical justification for TPN/IDPN thoroughly. For TPN, document nutritional insufficiency (e.g., adult involuntary weight loss: 10% in 6 months or 5% in 1 month; children: weight-for-age/length <10th percentile or inadequate gain; neonates: birth weight <1000 g) and evidence of structural or functional GI disease making enteral/oral feeding inappropriate. For IDPN, additionally document stage 5 chronic kidney disease, ongoing hemodialysis, and rationale for offering IDPN as an alternative to regularly scheduled TPN.

    • Adults: involuntary weight loss ≥10% in 6 months or ≥5% in 1 month
    • Children: weight-for-length/height or sex <10th percentile or inadequate weight gain or significant drop in growth percentile
    • Neonates: extremely low birth weight <1000 g
    • Clinical evidence of GI dysfunction (e.g., short bowel syndrome, fistulae, obstruction, motility disorder, necrotizing enterocolitis, small bowel ischemia, radiation enteritis)
    • IDPN-specific: stage 5 CKD + on hemodialysis; IDPN used as alternative to routine TPN
    Note

IDPN Positioning Relative to Other Nutrition Therapies

IDPN should be considered only after attempts at oral and enteral nutrition have failed or are not feasible. Societal guidance (e.g., ASPEN) recommends reserving IDPN for patients unable to meet needs orally who are not candidates for enteral nutrition or standard TPN because of GI intolerance, venous access issues, or other contraindications. IDPN provides partial nutritional delivery (approximately 70% of needs due to dialysate losses) and evidence of long-term benefit is limited; select patients carefully.

    • Consider after inadequate response to oral supplements and when enteral feeding is not tolerated
    • Use when venous access or GI issues preclude standard TPN
    • Recognize IDPN delivers ~70% of nutrients due to dialysate losses
    Denial Risk

Not Proven Safe and Effective — IDPN Misuse

IDPN is considered not proven safe and effective when used in addition to regularly scheduled TPN or when used for patients with acute kidney injury who do not have end-stage renal disease (ESRD). Such uses are subject to denial.

    • IDPN added to regularly scheduled TPN is not supported
    • IDPN for AKI patients without ESRD is not supported
    Billing Rule

Documentation and Compliance — Provider Obligations

Providers submitting requests for TPN/IDPN must supply documentation that supports medical necessity and meet the plan's prior authorization requirements. Failure to document required clinical criteria, use IDPN in unsupported scenarios, or ignore program rules may result in claim denial or recoupment.

    • Ensure documentation aligns with criteria to avoid denials
    • Unsupported indications (see Not Proven Safe and Effective) increase denial risk
    • Claims are adjudicated according to plan contracts and applicable laws
Provides approximately 70% of nutrients due to losses into the dialysate
Practical advantageAdministered through the dialysis access, avoiding placement of an additional venous catheter
