OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria for TB Testing

    Medically Necessary / Reimbursable Tests

Covered when ANY of the following conditions are met

IGRA for latent TB: Interferon gamma release assay (IGRA) may be reimbursed to diagnose or screen for latent tuberculosis infection in individuals at risk for Mycobacterium tuberculosis infection or when screening is required by law.

Supported for LTBI screening and diagnosis

AFB smear, culture, NAAT: For all suspected TB infections, acid-fast bacilli (AFB) smear/stain, culture with culture-based drug susceptibility testing of Mycobacteria, and qualitative nucleic acid amplification testing (NAAT) for Mycobacteria spp, M. tuberculosis and M. avium complex may be reimbursed.

Includes qualitative NAATs and culture-based methods

LAM testing in advanced HIV: Urine-based detection of mycobacterial lipoarabinomannan (LAM) may be reimbursed for HIV-infected individuals with CD4 ≤ 100 cells/microL who have signs and symptoms of tuberculosis.CD4 ≤ 100 cells/microL

Supportive fluid testing for extrapulmonary TB evaluation

Molecular Drug Susceptibility Testing — Conditional Coverage

Molecular-based drug susceptibility testing may be reimbursed when ANY of the following are met

Past TB treatment: Individual has been treated for TB in the past.

    One qualifying condition

High-risk birthplace or residence: Individual was born in or has lived for at least 1 year in a foreign country with at least a moderate TB incidence (≥ 20 per 100,000) or a high primary MDR-TB prevalence (≥ 2%).see policy

Qualifies for molecular drug susceptibility testing

Contact with MDR-TB case: Individual is a contact of an individual with multidrug-resistant TB (MDR-TB).

    Contact exposure qualifies

    HIV infection:

    Repeat Drug Susceptibility Testing

Repeat drug susceptibility testing may be reimbursed when ANY of the following are met

Persistent positive cultures: Sputum cultures remain positive after 3 months of treatment.3 months

Indication for repeat drug susceptibility testing

Bacteriological reversion: There is bacteriological reversion from negative to positive.

Indication for repeat drug susceptibility testing

Not Medically Necessary / Excluded Tests

    Not reimbursed

Tests lacking supportive evidence: Quantitative nucleic acid testing for Mycobacterium spp, M. tuberculosis, and M. avium complex; whole genome sequencing of Mycobacterium spp for detection of drug resistance; genotyping of Mycobacterium spp; testing of adenosine deaminase (ADA) and interferon-gamma (IFN-γ) levels in cerebrospinal, pleural, peritoneal, pericardial, and other fluids for diagnosis of extrapulmonary TB; and testing of serum protein biomarkers or panels for TB detection are not reimbursed due to insufficient published evidence.

    Explicit exclusions listed by policy

The policy explicitly excludes several laboratory tests from reimbursement. Quantitative nucleic acid amplification testing (quantitative NAAT) for Mycobacterium spp., including M. tuberculosis and M. avium complex, is not reimbursed. In addition, whole genome sequencing (WGS) of Mycobacterium spp. for detection of drug resistance and genotyping of Mycobacterium spp. are not reimbursed. Testing of adenosine deaminase (ADA) and interferon-gamma (IFN-γ) levels in cerebrospinal, pleural, peritoneal, pericardial, and other body fluids for diagnosis of extrapulmonary TB is not reimbursed. Finally, testing of serum protein biomarkers or panels of biomarkers for TB detection and diagnosis is also not reimbursed.

These exclusions are based on an identified lack of sufficient published scientific evidence demonstrating that the tests are required and beneficial for diagnosis or treatment. Providers should be aware that claims for quantitative NAAT, whole genome sequencing, genotyping, measurement of ADA or IFN-γ in body fluids for extrapulmonary TB, and serum protein biomarker panels may be denied as not reimbursed when submitted.

    Provider Actions and Billing Guidance

    Prior Authorization

    Verify member benefits before ordering

Coverage requires verification of member benefits — Most listed TB diagnostic tests may be reimbursed when clinically indicated and benefit verified. Confirm the individual member's benefit coverage prior to ordering or billing to determine eligibility and any prior authorization or medical necessity documentation requirements.

    • Affected services: IGRA for latent TB screening, AFB smear/stain, Mycobacterial culture and drug susceptibility testing, Qualitative NAAT for Mycobacteria spp., urine LAM in HIV-infected individuals with CD4 ≤100, fluid analyses (cell counts, protein, glucose, LDH) for suspected extrapulmonary TB.
    Documentation Required

Coverage contingent on benefit and intended use

Coverage contingent on benefit and intended use — Application of the policy's coverage criteria depends on the individual's specific benefit plan and the clinical indication. Tests noted as reimbursable are for specified uses (e.g., IGRA for latent TB screening, qualitative NAAT and culture-based testing for suspected TB, molecular-based drug susceptibility testing only when criteria are met).

    Background

Infection with Mycobacterium tuberculosis may present as symptomatic active pulmonary disease or as asymptomatic latent tuberculosis infection (LTBI). Culture remains the diagnostic gold standard because it permits organism isolation and drug susceptibility testing. Nucleic acid amplification tests (NAATs) provide sensitive detection of mycobacterial DNA for suspected infections, while interferon-gamma release assays (IGRAs) detect a cellular immune response indicative of latent infection and are reimbursed for LTBI screening when clinically appropriate.

    Definitions

    IGRA — inv-19
DefinitionInterferon-gamma release assays (IGRAs) are blood tests measuring T-cell release of interferon-gamma after stimulation by M. tuberculosis antigens to detect latent tuberculosis infection (LTBI).
UseMay be reimbursed to diagnose or screen for latent TB infection in individuals at risk or when screening is required by law.
LimitationIGRA is not reimbursed for diagnosing active tuberculosis disease.
    NAAT — inv-20
DefinitionNucleic acid amplification tests (NAATs) use polymerase chain reaction (PCR) to enable sensitive detection and identification of low-density Mycobacteria infections.
CoverageQualitative NAATs for Mycobacteria spp, M. tuberculosis, and M. avium complex may be reimbursed for suspected TB infections.
