Coverage Criteria for Multiplex Molecular Panels

Reimbursable outpatient respiratory multiplex PCR panels

Covered when ALL of the following are met

Outpatient respiratory panel (<=5 targets): Multiplex PCR-based panel testing (up to 5 respiratory pathogens) may be reimbursable for individuals who are displaying signs and symptoms of a respiratory tract infection, including at least one of the following: a temperature ≥102° F; pronounced dyspnea; tachypnea; or tachycardia.up to 5 targets

Policy applies only to outpatient testing

This policy addresses reimbursement for multiplex molecular pathogen panels used in the outpatient setting. Multiplex PCR-based respiratory panels that detect up to 5 respiratory pathogens may be reimbursable when performed on symptomatic individuals and when ALL coverage criteria are met, including at least one qualifying clinical sign: temperature ≥102°F, pronounced dyspnea, tachypnea, or tachycardia. Multiplex PCR-based panels that detect 6 or more respiratory pathogens are not reimbursable.

In addition to respiratory panels, molecular panel testing of other specimen types is specifically addressed. Panel testing of pathogens in cerebrospinal fluid (CSF) is not reimbursable, and molecular detection–based panel testing of pathogens in the blood is not reimbursable. The policy also identifies certain molecular panels for urine (for diagnosis of urinary tract infections) and wound infection testing as not reimbursable; examples cited in the document include the GENETWORx Molecular PCR UTI Test and GENETWORx PCR Wound Testing. Policy updates and code additions affecting outpatient reimbursement are summarized in the policy history and should be reviewed when billing.

The Plan considers molecular-based panel testing performed for general screening of microorganisms to be not reimbursable. The policy history and examples highlight tests that have been identified as general screening modalities and therefore subject to denial under the policy. Specific examples called out in the document include broad molecular screening platforms such as MicroGenDX qPCR+NGS and previously referenced specimen-specific screening panels (for example, certain vaginal swab or urine screening panels).

The policy revisions and historical notes (see Policy Update History) reflect changes in how general screening panels are enumerated and how reimbursable examples are handled; providers should consult the policy's not-covered list and update history when determining whether a given molecular panel performed for broad organism screening is eligible for reimbursement.

Use of molecular-based panel testing for the specimen types and broad screening indications listed in this policy is considered not reimbursable by the Plan. Specifically, the Plan does not reimburse: multiplex PCR-based panel testing of 6 or more respiratory pathogens; panel testing of pathogens in CSF; molecular detection–based panel testing of pathogens in the blood; molecular detection–based panel testing of urine pathogens for diagnosis of urinary tract infections (for example, GENETWORx Molecular PCR UTI Test); and molecular-based panel testing to screen for or diagnose wound infections (for example, GENETWORx PCR Wound Testing).

Providers should ensure testing and billing align with the policy’s outpatient-only scope and with the specific CPT/HCPCS/U-code assignments listed in the reimbursement section and update history to avoid claim denials.

Historically, the Plan has identified a set of molecular-based panel tests used for general screening of microorganisms as not reimbursable. The policy update history documents examples and revisions: earlier iterations explicitly listed specimen-specific screening panels (e.g., vaginal swab panels such as SmartJane™, urine screening panels such as UroSwab®) and later reinstated explicit non-reimbursable examples such as MicroGenDX qPCR+NGS. The policy history also records other example additions (e.g., GENETWORx assays) and clarifications to the outpatient-focused scope.

These historical entries are intended to guide providers about the types of broad screening molecular panels the Plan has previously determined are outside of reimbursable outpatient testing.

Indications Where Testing May Be Covered

Symptomatic outpatient respiratory tract infection with qualifying signs for outpatient multiplex respiratory panel (<=5 targets)

Symptomatic outpatient respiratory tract infection with qualifying signs: Patient is symptomatic with a respiratory tract infection and has at least ONE of: temperature ≥102° F; pronounced dyspnea; tachypnea; or tachycardia.

Diagnostic testing for suspected sepsis, meningitis/encephalitis, respiratory infections, and other acute infectious syndromes where rapid pathogen identification may affect management

Covered when rapid pathogen identification may affect immediate clinical management

Acute infectious syndromes where rapid ID may affect management

    • Sepsis: Suspected sepsis/septic shock or bloodstream infection where rapid molecular pathogen detection may alter antimicrobial management (see Surviving Sepsis Campaign guidelines).

    Not Covered Services and Panels

The following categories of molecular panel testing are identified as not covered under this outpatient-focused policy: multiplex PCR panels detecting ≥6 respiratory pathogens; cerebrospinal fluid (CSF) multiplex panels; molecular detection–based blood pathogen panels; urine molecular panels intended for UTI diagnosis; and molecular panels for wound infection diagnosis. The policy also clarifies that molecular-based panel testing performed for broad or general screening of microorganisms is not reimbursable, with specific examples provided in the policy history.

Providers submitting outpatient claims for panel tests should compare the test purpose, specimen type, and code(s) billed against this not-covered list to determine likely reimbursement outcomes.

Examples of molecular-based panel testing performed for general screening of microorganisms that the policy identifies as not reimbursable include broad screening platforms referenced in the policy history. The document specifically names MicroGenDX qPCR+NGS as an example and notes prior examples of specimen-specific screening panels (such as certain vaginal swab and urine screening tests) that have been discussed in earlier versions. Providers should treat similar broad molecular screening assays as non-reimbursable unless otherwise specified.

This policy history and its enumerated examples are intended to aid billing decisions; when in doubt, confirm whether a test is being billed as a diagnostic/outpatient clinically indicated assay versus a general screening service.

    Ordering and Setting Requirements

    Step Therapy

Confirm plan/product‑specific reimbursement applicability before ordering

Review Plan documents and product‑specific reimbursement criteria before ordering; this policy applies specifically to outpatient testing and may not apply to every product or benefit.

    • The Plan has implemented lab management reimbursement criteria; not all requirements apply to each product — verify member’s benefit.
    • Providers are urged to review Plan documents for eligible coverage for services rendered.
    Step Therapy

Ensure orders and claims are for outpatient setting to meet policy applicability

Ensure the order and billing indicate the outpatient setting since reimbursement statements and code applicability in the policy are specific to outpatient testing.

    • Policy statement: 'This policy is specific to testing in the outpatient setting.'
    • If the testing is not outpatient, the policy criteria do not apply.

    Background and Definitions

Multiplex molecular panels are assays that simultaneously detect multiple pathogen targets and can provide rapid identification of causative organisms for a range of syndromes. These platforms have been developed for respiratory infections, central nervous system infections (meningitis/encephalitis), bloodstream infections/sepsis, urinary tract infections, and wound infections.

When used appropriately in the outpatient diagnostic context, targeted multiplex respiratory panels (up to 5 targets) can support clinical management of symptomatic patients. However, clinical utility varies by syndrome and specimen type; the policy references systematic reviews and guideline literature (for example, evidence addressing meningitis/encephalitis and sepsis-related multiplex assays) to inform appropriate use. Because the Plan’s reimbursement policy is limited to the outpatient setting, providers should apply clinical judgment and relevant guidance to determine the appropriateness of multiplex testing for cases where rapid pathogen identification may affect patient management.
