Coverage Criteria for Injectable Medications
Initial Coverage Criteria
Covered when ALL of the following are met
See appendix for DSM-5 criteria (persistence typically >= 6 months).
Coverage for certain injectable classes is limited or excluded depending on plan design, regulations, and the clinical indication. Contraceptive injectables are covered only under plans that include a contraceptives rider or a contraceptives benefit and in non-grandfathered plans subject to DHHS preventive service requirements. Immunizations for travel may be excluded. Anabolic steroids used for performance enhancement are not covered because that use is not considered treatment of disease. Infertility injectable medications are covered only when required by regulation or when a specific benefit design includes them; otherwise they may be excluded.
Injectable drugs that are experimental or investigational, or that have not been proven safe and effective for the specific disease or mode of treatment and are not approved by the FDA and/or NIH, are not covered. This exclusion does not apply to established off‑label uses of otherwise FDA‑approved prescribed medications.

Provider Actions, Authorization & Documentation
Prior Authorization Required
Prior authorization is required per plan for injectable medications and related administration/drug HCPCS/CPT/J-series codes. Providers should obtain prior authorization before dispensing or billing for injectable therapies when the member's benefit plan requires it.
- Applies to physician-administered and self-administered injectable drugs when plan benefit requires prior auth
- Includes associated administration codes (e.g., relevant CPT/HCPCS/J-codes) where prior authorization is required
Step Therapy Reference
Refer to the Pharmacy Clinical Policy Bulletins (PCPBs) for step therapy requirements for oral and self‑administered injectable medications. Providers should follow PCPB step-therapy pathways where applicable prior to requesting coverage for an injectable alternative.
- See applicable PCPB for specific step therapy sequences and exceptions
- Step therapy may apply to oral agents and certain self-administered injectables
Needle Phobia Documentation
Documentation of needle phobia may support waiver of a required trial of an oral alternative. The medical record must show preexisting excessive fear of injections or blood draws, attempts at management (behavioral therapy, counseling, desensitization), and associated symptoms (e.g., vasovagal syncope, panic attacks). Include DSM‑5 criteria or equivalent behavioral health documentation in the request.
- Document preexisting excessive fear of injections/blood draws (not limited to the current request)
- Provide evidence of prior management efforts (psychological counseling, behavioral therapy, desensitization)
- Include associated symptoms (vasovagal syncope, panic attack) when present
- Reference DSM‑5 Specific Phobia criteria in the clinical record when available
Experimental/Investigational Exclusion
Coverage is denied for experimental/investigational drugs or uses not proven safe and effective or not approved by the FDA for the indicated mode of treatment. This does not exclude established off‑label uses of otherwise FDA‑approved medications when supported by evidence and consistent with benefit design.
- Drugs lacking sufficient evidence of safety/efficacy for the requested indication are not covered
- Established off‑label uses of FDA‑approved drugs may be considered if supported by evidence and benefit design
Non-covered Service
Drugs related to treatment of non‑covered services (for example, agents used for performance enhancement) are not covered. Providers should verify benefit plan design for service‑specific exclusions (e.g., infertility medications, contraceptive injectables, travel immunizations) prior to prescribing or billing.
- Anabolic steroids for performance enhancement are not covered
- Infertility injectable medications are covered only when required by regulation or when specified by the member's benefit design
- Contraceptive injectables and travel immunizations may be excluded depending on plan

Site of Care and Administration Settings
Coverage applies to home (self‑administered) and clinical administration
Coverage includes medications intended to be self‑administered as well as those administered in clinical settings; coverage remains subject to plan‑specific rules and benefit design.
- Verify member benefit design for site‑of‑care or plan exclusions (e.g., infertility injectables or contraceptive injectables).
Background
For this policy, injectable medications include agents administered via intramuscular, subcutaneous, intrathecal, or intravenous routes and encompass vaccines, immune globulins, therapeutic infusions, and other J‑code billed injectable drugs. Coverage applies to medications that are medically necessary and appropriate to the member’s condition, including those intended for self‑administration, subject to plan‑specific rules and benefit design.
