    Coverage Criteria and Indications

    Covered Indications

Covered when ANY of the following are met:

Covered indications: 1) Drug screening/testing on admission to inpatient, residential, or intensive outpatient treatment (IP/Residential/IOP) for initial assessment or baseline; OR 2) Drug screening/testing prior to initiating chronic opioid therapy for initial assessment or baseline; OR 3) Additional drug screening/testing when the requesting practitioner will utilize results for treatment planning and any of: patient reports relapse; external reports of relapse; altered mental or physical status; other definitive information suggesting relapse; annual testing for chronic opioid therapy or more frequently with evidence of increased misuse risk; OR 4) Other medically necessary situations not outlined above.

From Procedure Detail II.A–C and II.F.2-6

    Confirmatory Testing Criteria

Confirmatory testing is covered when ALL of the following are met:

Confirmatory testing criteria: a) The result of the drug test is different than that suggested by the patient’s medical history, clinical presentation, or patient's own report; AND b) There are inconsistent findings if previous tests were performed; AND c) The ordered confirmatory tests are not part of a predesignated laboratory panel; AND d) A signed requesting practitioner order deeming the laboratory test(s) medically necessary includes the specific name of the lab test(s) to be performed.

    Procedure Detail II.D.1 a-d

    Full Panel Screening Criteria

Full panel screening should be considered only when ALL of the following are met:

Full panel criteria: 1) The patient's observed behavior suggests use of drugs not identified on initial screening; AND 2) Medical documentation supports the behavioral observation and medical justification for conducting a full panel screening; AND 3) A signed requesting practitioner order deeming the laboratory test(s) medically necessary includes the specific name of the lab test(s) to be performed.

    Procedure Detail II.E.1-3

Network Health does not cover routine or random drug screening or testing during inpatient, residential, or intensive outpatient (IP/Residential/IOP) treatment when there is no evidence suggestive of relapse. Testing ordered solely by third parties (for example, employers, schools, or courts) is not covered when there is no evidence suggestive of relapse. Additionally, when a urine drug screen (dipstick or simple screen) is consistent with the member’s prescribed medications and the member is not exhibiting unusual behaviors, further full-panel or confirmatory testing is not medically necessary.

Network Health considers drug testing via hair analysis NOT medically necessary and therefore does not cover hair-based drug testing under this policy.

    When Testing Is Covered

Initial/baseline testing on admission to treatment or prior to initiating chronic opioid therapy; additional testing when used for treatment planning as specified.

Initial or baseline testing covered when the following indication is present:

Initial/baseline testing: Initial/baseline drug testing is covered when performed on admission to treatment (inpatient, residential, or intensive outpatient) or prior to initiating chronic opioid therapy; additional testing is covered when the requesting practitioner will use results for treatment planning as specified (e.g., reported relapse, external reports of relapse, altered mental/physical status, other definitive information suggesting relapse, or yearly/more frequent testing for chronic opioid therapy based on risk).

    Procedure Detail II.A–C and II.F.2-6

    Testing Frequency

inv-17: periodic urine drug testing (including chromatography)
Frequency for low-risk chronic opioid therapyPeriodic urine drug testing (including chromatography): at least yearly for low-risk patients
Increased-risk patientsTesting more frequently than yearly when there is evidence of increased risk of misuse (e.g., PDMP findings, violations of opioid agreement, altered mental/physical status, external reports of relapse)
Triggers for additional testingAdditional testing is covered if external agencies report relapse, patient reports relapse, mental/physical status appears altered, or other definitive information suggests relapse
Ongoing assessment componentsOngoing urine drug screening assessment includes review of PDMP, periodic pill counts for high-risk patients, and monitoring for opioid agreement violations
inv-07: Periodic urine drug testing frequency

    Ordering, Documentation, and Denial Triggers

    Prior Authorization

Signed order required for confirmatory and full‑panel tests

Confirmatory and full-panel definitive tests require a signed requesting practitioner order that deems the test(s) medically necessary and specifies the exact name of the lab test(s) to be performed; confirmatory testing is appropriate only when the policy's confirmatory criteria are met (see confirmatory testing criteria).

    • Signed requesting practitioner order must include the specific name of the lab test(s) to be performed.
    • Confirmatory testing is only appropriate when a screening reveals non‑prescribed or illicit drug(s) and all confirmatory criteria (a–d) are met.
    Documentation Required

Order must specify test name and medical necessity

A signed requesting practitioner order deeming the laboratory test(s) medically necessary must include the specific name of the lab test(s) to be performed for confirmatory testing or a full panel screening; this order is required on pre‑determination.

    Non-Covered Services and Limitations

The following are not covered: hair analysis testing; routine or random drug screening/testing during IP/Residential/IOP without evidence suggestive of relapse; drug screening/testing ordered by third parties (employer, school, court) without evidence suggestive of relapse; testing to perform both blood and urine for the same drug simultaneously; and full-panel or confirmatory testing when an initial urine screen is consistent with prescribed medications and the member shows no unusual behaviors.

Hair testing is explicitly identified as not medically necessary and is excluded from coverage under this policy.

    Definitions and Test Types

    inv-13: Full Panel Screen
DefinitionFull Panel Screen: a drug test that screens for the most misused illicit and/or prescription drug classes.
Use caseConsidered only when observed behavior suggests drug use not identified on initial screening and medical documentation supports testing
Order requirementRequires a signed requesting practitioner order specifying the specific lab test(s) to be performed for medical necessity
    inv-14: Qualitative (presumptive) drug testing
DefinitionQualitative (presumptive) drug testing: detects the presence or absence of a substance belonging to a general class of drugs; results expressed non‑numerically (positive/negative).
PurposeUsed as an initial screen to identify classes of substances present before confirmatory testing is considered

    Background and Rationale

Drug testing (blood, urine, saliva, sweat, hair, breath) detects drugs and metabolites and supports treatment planning; detection windows vary and prolonged or frequent use can lengthen detectability. Clinical use of testing should be limited to the minimum frequency necessary to detect use and aligned with the member’s clinical history and presentation. Specifically, routine or random testing without evidence of relapse and hair analysis testing are not covered per policy; confirmatory and full-panel tests require a signed practitioner order and other clinical justification when used for treatment planning or to resolve inconsistent or unexpected results.

Coverage Criteria and IndicationsWhen Testing Is CoveredTesting FrequencyBilling and CodesOrdering, Documentation, and Denial TriggersNon-Covered Services and LimitationsDefinitions and Test TypesBackground and Rationale
    Baseline frequency statement
Periodic urine drug testing frequency: at least yearly in low‑risk cases
Escalation criteriaIncrease testing frequency with evidence of increased risk of misuse (e.g., PDMP abnormalities, relapse indicators, altered clinical status)
Context of usePart of ongoing urine drug screening assessment for patients on chronic opioid therapy or when clinically indicated
Related monitoring actionsMay include periodic pill counts and review of PDMP information as part of risk assessment
    • Order must state the specific name of the test(s) to be performed.
    • Order must explicitly deem the test(s) medically necessary (required for pre‑determination).
    Denial Risk

Denial triggers: routine/random or third‑party testing and hair analysis

The policy lists specific triggers that will lead to denial of coverage, including routine or random testing during inpatient/residential/IOP without evidence suggestive of relapse, third‑party‑ordered testing without evidence suggestive of relapse, and hair analysis testing.

    • Routine drug screening/testing during IP/Residential/IOP without evidence suggestive of relapse is not covered.
    • Random drug screening/testing during IP/Residential/IOP without evidence suggestive of relapse is not covered.
    • Testing ordered by third parties (employer, school, court) without evidence suggestive of relapse is not covered.
    • Hair analysis testing is considered not medically necessary and will be denied.
    Note

Provider must meet all confirmatory testing criteria before ordering

When a presumptive screen detects non‑prescribed or illicit drugs, confirmatory testing is indicated only if all described confirmatory criteria are met (result inconsistent with history/presentation, inconsistent with prior tests, not part of a predesignated panel, and supported by a signed order naming the confirmatory test).

    • Confirmation is indicated when: a) result differs from medical history/clinical presentation/patient report; b) inconsistent findings with previous tests; c) confirmatory test is not part of a predesignated panel; d) there is a signed order specifying the exact confirmatory test.
    Billing Rule

ORDERING REQUIREMENTS: Signed order must state medical necessity and exact test name(s)

Ordering requirements reiterate that a signed requesting practitioner order must deem the lab tests medically necessary and include the specific name of the tests to be performed for confirmatory and full panel testing; this is required for pre‑determination.

    • Signed order must both declare medical necessity and list the exact test name(s).
    • Full panel screening similarly requires documentation of medical necessity and the specific test name(s).
Relation to confirmatory testingA positive presumptive result may trigger confirmatory testing when results are inconsistent with clinical presentation or history
inv-15: Quantitative (definitive or confirmatory) drug testing
DefinitionQuantitative (definitive or confirmatory) drug testing: determines the specific quantity or concentration of a drug or drug metabolite; results are numerical.
Appropriate useIndicated when confirmatory testing criteria are met (e.g., unexpected or inconsistent presumptive results) and ordered tests are not part of a predesignated panel
Documentation requirementRequires a signed requesting practitioner order that specifies the exact lab test(s) to be performed and deems them medically necessary
