What Providers Must Do
Obtain prior authorization for any genetic test on the Precertification List
Tests listed on Blue Cross Blue Shield - Alabama’s Genetic Testing Program Precertification List require prior authorization (precertification) before reimbursement will be considered. Providers must obtain precertification for services billed with any CPT or temporary U codes included on the precertification list prior to performing the service or submitting a claim.
- Affected codes include multiple CPT genetic testing codes (examples shown on the list: 81353, 81354, 81355, 81400–81416, 81419, 81420, 81422, 81432, etc.). [see list in document]
- Temporary U codes added (examples effective Jan 1, 2026: 0033U, 0131U, 0132U, 0135U, 0508U, 0543U, 0544U, 0549U, 0566U, 0567U, 0569U, 0571U, 0586U, 0592U, 0597U, 0605U, 0611U, 0612U, 0613U).
- Several codes have specified effective dates (codes added effective January 1, 2026 are listed in the document).
