    Policy Background

Sacroiliac joint pain may be evaluated with image-guided anesthetic injections and treated with corticosteroid injection or, in selected patients, sacroiliac joint fixation or fusion. Before fusion or fixation is considered, the policy requires prolonged conservative treatment together with objective clinical, imaging, and diagnostic-injection findings. Diagnostic blocks should be performed under fluoroscopic guidance; a successful controlled diagnostic block involves 2 separate positive blocks on different days using local anesthetic only, or a placebo-controlled series, with pain relief lasting for the expected duration of the anesthetic.

    Key Definitions

    Successful Controlled Diagnostic Block
Positive blocksTwo separate positive blocks performed on different days
Medication usedLocal anesthetic only; no steroids or other drugs
Alternative methodA placebo-controlled series of blocks may be used
Guidance and responseBlocks are performed under fluoroscopic guidance and must reduce pain for the duration of the local anesthetic used

  4. Document the individual’s compliance with the conservative-treatment program.
  5. For SI joint fixation or fusion, submit clinical documentation supporting the required imaging evaluation of the SI joint, pelvis, and lumbar spine. The imaging must establish that competing or causative pathology has been excluded and that SI joint injury or degeneration is present.

    • SI joint plain radiographs plus CT or MRI exclude destructive lesions, such as tumor or infection, and inflammatory arthropathy.
    • An anteroposterior pelvic radiograph rules out concomitant hip pathology.
    • Lumbar spine CT or MRI rules out neural compression or other degenerative conditions that may cause low back or buttock pain.
    • SI joint imaging indicates evidence of injury and/or degeneration.
    • Documentation must also support at least 6 months of intensive nonoperative treatment, including medication optimization, activity modification, bracing, active therapeutic exercise targeting the lumbar spine, pelvis, SI joint, and hip, and a home exercise program.

    • Radiofrequency denervation: CPT 64625
    • SI joint arthrography: HCPCS G0259
    • Fusion or stabilization outside the listed criteria or using unlisted devices
    Note

    Confirm benefit-plan limitations

Coverage and authorization depend on the member’s contract and benefit plan. Benefits may be unavailable or different under an employer plan, ASO arrangement, NEHP/ABNE benefits, or the Federal Employee Program; contract and employer benefit-plan language takes precedence over the medical policy when there is a conflict.

    • Verify benefits before providing the service
    • Check for plan-specific exclusions or limitations
    • Contact the applicable health-plan or FEP customer service when needed
    Localized tenderness with palpation
Clinical significanceIdentifies localized sacroiliac joint tenderness; tenderness should not be of similarly severe intensity elsewhere
