OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Therapeutic NMES medical necessity

Therapeutic NMES may be considered medically necessary when ALL core requirements and at least ONE eligible clinical indication are met.

    Core requirements

    • Medical necessity and documentation

      • The requested device is therapeutic NMES for the treatment of adequately documented disuse atrophy.
      • The nerve supply to the affected muscle is intact.
      • Clinical improvement may be expected.
      • Disuse atrophy is documented by physical examination and electromyography (EMG).
    • ONE eligible clinical indication

      • Disuse atrophy results from recent hip surgery, until the patient begins physical therapy.
      • Disuse atrophy results from prolonged casting or splinting of a joint.> 12 weeks
      • Contractures result from scarring of soft tissue from burns.
      • The patient has partial paralysis following stroke, and treatment is to improve wrist and finger function and prevent or correct shoulder subluxation.

    Functional NMES investigational uses

The following functional NMES uses are considered investigational.

    Investigational functional uses

    • Use to provide upper-extremity function in patients with nerve damage, including spinal cord injury or post-stroke nerve damage.
    • Use to improve ambulation in patients with foot drop caused by congenital disorders, such as cerebral palsy, or nerve damage, including post-stroke nerve damage or multiple sclerosis.
    • Use as a technique to provide ambulation in patients with spinal cord injury.
    • Use of exercycles with functional electrical stimulation technology for physical therapy or exercise in patients with spinal cord injury.

Functional neuromuscular stimulation intended to restore function following nerve damage or nerve injury is excluded from covered medical necessity because it is considered investigational. This includes use to provide upper-extremity function in patients with nerve damage, improve ambulation for foot drop caused by congenital disorders or nerve damage, provide ambulation in patients with spinal cord injury, and use of exercycles with functional electrical stimulation for physical therapy or exercise in patients with spinal cord injury.

    Therapeutic NMES Medical Necessity

    Therapeutic NMES equipment criteria

Therapeutic NMES equipment may be considered medically necessary when ALL of the following requirements are met.

    • Disuse atrophy is adequately documented by physical examination and electromyography (EMG).
    • The nerve supply to the affected muscle is intact.
    • Clinical improvement is expected.
    • ONE eligible underlying condition or indication

      • Recent hip surgery, until the patient begins physical therapy.

    Documentation Requirements

    Documentation Required

    Document Medical Necessity

Document medical necessity for therapeutic NMES used to treat adequately documented disuse atrophy. The record must establish the diagnosis by physical examination, include EMG findings, show that nerve supply to the muscle is intact, and identify an eligible underlying condition in which clinical improvement may be expected.

    • Eligible conditions include recent hip surgery until physical therapy begins; prolonged casting or splinting of a joint for more than 12 weeks; contractures caused by burn-related soft-tissue scarring; and treatment to improve wrist and finger function or prevent or correct shoulder subluxation in persons with partial paralysis following stroke.
    • Insufficient medical-necessity documentation may prevent approval or delay the coverage determination.
    • Functional NMES use is investigational and is not covered under the therapeutic NMES criteria.
    Prior Authorization

    Prior Approval and Benefit Verification

Obtain code-specific prior approval before providing NMES. Submit a complete authorization request containing all information required by the policy guidelines; incomplete requests may delay the decision while missing information is obtained.

    Background

Neuromuscular electrical stimulators are broadly categorized as therapeutic or functional. Therapeutic stimulation is intended to strengthen muscles weakened by disuse when the nerve supply to the muscle remains intact. Functional stimulation uses electrical stimulation, often with a microprocessor-controlled orthotic device, to assist functional movement by attempting to replace damaged or destroyed nerve pathways.

    • Verify the member's benefits before providing the service, including applicable exclusions, limitations, contract terms, group or individual benefit documents, and any differences for NEHP/ABNE, FEP, or ASO coverage.
    • Prior approval and benefit availability are subject to the member's subscriber contract; contract language takes precedence over the medical policy when the two conflict.

    Authorize listed NMES codes

Obtain prior approval for listed procedures, electrical stimulator supplies, conductive garments, neuromuscular stimulators, and implantable neurostimulator components when the coding table identifies the requirement.

    • Procedures: 64580, 95970, 95971, 95972
    • Supplies and garments: A4595, E0731, E0745
    • Implantable components: L8680, L8682, L8686, L8687, L8688, L8689
    Documentation Required

Document disuse atrophy and EMG findings

For NMES requested for disuse atrophy, submit medical-record documentation showing disuse atrophy based on physical examination and EMG. The documentation must also identify an eligible underlying condition or post-stroke indication.

    • Show that the nerve supply to the affected muscle is intact.
    • Identify the qualifying etiology, such as recent hip surgery, casting or splinting exceeding 12 weeks, burn-related contractures, or partial paralysis following stroke.
    Documentation Required

    Complete the authorization submission

Submit complete information required by the policy guidelines with the authorization request. Do not submit the request until the required clinical and supporting information is included.

    • Incomplete requests may delay the decision pending submission of missing information.
    Note

    Verify member benefits

Verify the member’s applicable contract or benefit plan before providing the service. Coverage, exclusions, limitations, and benefits may vary by group, individual contract, Federal Employee Program coverage, or Administrative Services Only plan.

    • Contact the member’s health plan or customer service department when required.
    • Member contract and employer benefit-plan language takes precedence over the medical policy when there is a conflict.
    Denial Risk

    Identify investigational functional NMES

Do not represent functional NMES used to restore function following nerve damage or nerve injury as covered medical necessity; these uses are considered investigational.

    • Providing upper-extremity function in patients with nerve damage
    • Improving ambulation for foot drop caused by congenital disorders or nerve damage
    • Providing ambulation in patients with spinal cord injury
    • Using exercycles with functional electrical stimulation for spinal cord injury physical therapy or exercise
    Denial Risk

    Avoid insufficient medical-necessity documentation

Therapeutic NMES requests may be denied when the record does not document disuse atrophy on physical examination, intact nerve supply, supporting EMG, and an eligible clinical condition.

    • Document disuse atrophy based on physical examination.
    • Include supporting electromyography (EMG).
    • Show intact nerve supply and an eligible etiology or post-stroke indication.
    Denial Risk

Avoid delays from an incomplete request

Incomplete prior authorization requests may delay the decision until the missing information is submitted. Ensure the request includes all information required by the policy guidelines.

    • A decision may remain pending while missing information is requested or submitted.
    Billing Rule

    Maintain audit compliance

Blue Cross VT may audit providers and facilities for compliance with the medical policy. Noncompliance identified during an audit may result in recoupment of all non-compliant payments.

    • Maintain documentation supporting compliance with the policy guidelines.
    • Non-compliant payments identified in an audit may be recouped.
