Coverage and Medical Necessity Criteria

Initial therapy — tandem autologous transplant

Covered when ALL of the following are met for tandem autologous transplant:

Diagnosis

Multiple Myeloma: Newly diagnosed or responsive multiple myeloma.
Germ cell tumors refractory to cisplatin-based chemotherapy, including relapsed testicular germ cell tumors and relapsed ovarian germ cell tumors.
High-risk neuroblastoma in a member <18 years defined by INRGSS/COG characteristics (examples: Stage M aged ≥18 months; Stage M aged <18 months with MYCN amplification; Stage MS or L2 with MYCN amplification; Stage L2 with unfavorable histology aged ≥18 months; Stage M or MS aged 12–18 months with unfavorable features such as unfavorable histology, segmental chromosomal aberrations, or diploid tumor; Stage L1 incompletely resected with MYCN amplification).

Contraindications absent: Member does not have significant dysfunction of a major organ system or severe comorbidities, including but not limited to: bilirubin > 2 mg/dL; INR > 1.6 (unless on oral anticoagulants); EF < 45%; pulmonary function with FEV1 ≤ 50% predicted or DLCO ≤ 60% predicted; poor performance status (Karnofsky/Lansky < 70% or ECOG > 2); uncontrolled or disseminated infection.

Presence of any of these excludes medical necessity.

Autologous followed by allogeneic transplant

Specific covered allogeneic sequences when ALL conditions met:

Autologous→Allogeneic (sibling): Tandem autologous transplant followed by an allogeneic transplant from an HLA-identical sibling donor with reduced-intensity conditioning is medically necessary for untreated, newly diagnosed multiple myeloma when none of the contraindications in section I.B. are present.

Autologous→Allogeneic (unrelated) - case-by-case: Tandem autologous transplant followed by an allogeneic transplant from an HLA-compatible unrelated donor for untreated, newly diagnosed multiple myeloma will be considered on a case-by-case basis when none of the contraindications in section I.B. are present.
