Tandem Transplant
Medical necessity criteria and coding guidance for planned sequential (tandem) high-dose therapy with hematopoietic cell transplant, primarily autologous transplants (and specified autologous→allogeneic sequences) for diagnoses including multiple myeloma, certain germ cell tumors, and high‑risk neuroblastoma; applies to members/enrollees of Centene-affiliated health plans (Ambetter Nevada).
Updated Criteria I.A.2. to include ovarian germ cell tumors.
Updated neuroblastoma criteria from INSS to INRGSS/COG high-risk characteristics and adjusted age criteria.
Replaced prior extensive contraindication list with a new, condensed contraindication list (I.B.1 through I.B.6).
Clarified that tandem autologous followed by reduced-intensity allogeneic transplant from an HLA-identical sibling is medically necessary for untreated, newly diagnosed multiple myeloma when contraindications are absent.
Removed criteria IV. stating, current evidence does not support tandem transplants for any other indication than what is listed above.
Added substance use contraindication I.B.15.
Updated I.A.3.b., c. and d. to reflect months of age instead of days.
Added a. through c. to I.B.10.; a. CD4 cell count > 200 cells/mm3, b. Absence of active AIDS-defining opportunistic infection, and c. Member/enrollee is currently on effective ART.
Updated Criteria I.A.3. from INSS to INRGSS and COG high-risk disease group characteristics for neuroblastoma.
Updated Criteria I.A.2. to include ovarian germ cell tumors.
