    Clinical & Coding Specifications

    Clinical Context

A patient with a medium-to-large partial-thickness or full-thickness wound presents for ongoing wound care. Typical examples include a large stage II–III pressure ulcer on the sacrum, a non-infected venous stasis ulcer of the lower extremity with significant exudate, or a postoperative dehisced incision with clean granulating tissue requiring moisture management. The dressing described (A6244) is a sterile hydrogel pad larger than 48 square inches without an adhesive border, intended to maintain a moist wound environment and facilitate autolytic debridement.

The clinical workflow: initial wound assessment and measurement, wound cleansing and irrigation, selective sharp debridement if indicated, application of topical agents as ordered, placement of the hydrogel dressing to cover the wound bed ensuring contact with all wound surfaces, secondary absorbent dressing or securement as required, and patient/caregiver education on dressing change frequency and signs of complication. Documentation includes wound size, depth, appearance, exudate amount and character, presence of infection, pain level, dressing type and quantity (A6244 – units applied), and next scheduled dressing change.
