CPT 10005: Ultrasound-Guided Fine Needle Aspiration, First Lesion

Medicare pays $137 on average nationally for this procedure.

CPT code 10005 describes an ultrasound-guided fine needle aspiration (FNA) procedure in which a provider uses a specialized needle and syringe to collect a diagnostic specimen from a tissue, cyst, or mass; this is reported for the first lesion biopsied and is typically an image-guided diagnostic biopsy service performed in an outpatient clinic, radiology suite, or office-based procedure room.

For related coverage guidance, see recent payer policy updates: Destruction of Pre-Malignant Skin Lesions, Onychomycosis diagnostic testing and coverage, Treatment of Capillary Malformations (Port Wine Stains).

    Clinical & Coding Specifications

    Clinical Context

A 54-year-old female presents to an outpatient radiology clinic with a palpable, mobile 1.8 cm thyroid nodule noted on physical exam and prior ultrasound. The provider performs an ultrasound-guided fine needle aspiration to obtain cytologic material for diagnostic evaluation of the mass. The patient is positioned supine with neck extended, the skin is prepped, local anesthesia applied, and real-time ultrasound is used throughout needle placement. Multiple passes may be made to obtain adequate cellular material; specimens are placed into appropriate media and sent to cytology. The service is commonly billed when a single targeted lesion is sampled; additional lesions sampled would be reported with subsequent lesion codes. Typical site of service is an outpatient radiology or ambulatory surgery/procedural clinic and the service type is an image-guided diagnostic biopsy procedure. Relevant clinical workflow steps include informed consent, ultrasound localization, sterile preparation, local anesthesia, needle aspiration with syringe or vacuum device, specimen handling, and documentation of lesion targeted and number of passes.
