Site of service for elective surgical procedures is evaluated and determined medically necessary according to the following hierarchical logic:

Top-level site preference

When ASC not appropriate -> hospital outpatient may be considered: An elective surgical procedure performed in a hospital outpatient department may be considered medically necessary if any of the following are present: no qualifying ASC within 30 miles that can provide the necessary care; no geographically accessible ASC with required equipment; no geographically accessible ASC at which the individual's physician has privileges; the ASC's specific guideline prohibits use for the individual's health condition or weight; the individual is aged 18 years or younger; the service is being performed in conjunction with an additional service that requires a hospital outpatient department in the same operative session; or the individual has a clinical condition that places them at increased risk for complications.30 miles
Any one of these conditions suffices to justify hospital outpatient site.

Clinical risk factors justifying non-ASC site: Clinical conditions that place the individual at increased risk for complications may justify a non-ASC site. Categories include: - Anesthesia risk: ASA classification III or higher; personal history of anesthesia complication; documented alcohol dependence or history of cocaine use; prolonged surgery (>3 hours). - Cardiovascular risk: uncompensated chronic heart failure (NYHA III or IV); recent myocardial infarction (<3 months); poorly controlled/resistant hypertension; recent cerebrovascular accident (<3 months); increased ischemic risk after recent stent/angioplasty (drug-eluting stent <1 year or angioplasty <90 days); symptomatic arrhythmia despite medication; significant valvular heart disease. - Liver risk: advanced liver disease (MELD score > 8). - Pulmonary risk: COPD with reduced FEV1; moderate to severe obstructive sleep apnea. - Renal risk: end-stage renal disease on dialysis. - Other: morbid obesity (BMI ≥ 50); pregnancy; bleeding disorder requiring replacement products (DDAVP does not meet criterion); anticipated need for transfusion(s).

This list may not be exhaustive; presence of any listed condition supports selection of a hospital-based site.

ASC Coverage with Criteria

ASC is considered medically necessary for select surgical or diagnostic procedures when documented criteria are met and the procedure is included in the list of approved codes.

ASC medical necessity general condition: Procedure is one of the select procedures listed in the policy and meets the policy's clinical criteria for performance in an ASC.

Full, procedure-specific criteria are defined in the referenced policies (for example: 2.01.533, 2.02.510, 7.01.595); providers must request site-of-service review/authorization when performing listed CPT/HCPCS codes in an ASC.

The Site of Service Medical Necessity criteria in this guideline DOES NOT apply to Indian Health Services (IHS) facilities. For procedures performed at IHS facilities, follow the procedure-specific medical necessity criteria rather than the site-of-service rules in this policy.

Site-of-service review and the ASC-preference determinations described in this guideline do not apply to Indian Health Services (IHS) facilities. This header exemption was added in policy history; providers performing services at IHS sites should refer to the procedure-specific medical necessity criteria.

When the ASC-preference criteria are not met, an off‑campus or on‑campus hospital outpatient department or an inpatient hospital site may be considered appropriate. Conversely, these hospital outpatient or inpatient sites are considered not medically necessary for the listed elective surgical procedures when the ASC criteria and the justifications for hospital-based care are not present.

Services performed in an Ambulatory Surgical Center (ASC) that do not meet the policy's specified ASC clinical criteria for the listed procedures may be considered not medically necessary. Providers must ensure the procedure is one of the select procedures covered for ASC care and that documented ASC criteria are met when scheduling and requesting authorization for ASC site-of-service.
