Coverage Criteria for TMJ/Temporomandibular Disorder Surgery

General medical necessity criteria for TMD surgery

Covered when ALL of the following are met:

General Criteria: 1) Documentation of persistent pain and/or functional impairment that directly corresponds to an intra-capsular condition; AND 2) The intracapsular condition is confirmed by magnetic resonance imaging (MRI) or computed tomography (CT); AND 3) The condition has not responded or improved following a six (6) month trial of provider-directed nonsurgical management including diet modification (e.g., soft chew diet), application of moist heat, nonsteroidal anti-inflammatory medications (unless contraindicated), intraoral appliance (splint), and physical therapy.6 months trial

Imaging required: MRI or CT; conservative therapies listed must be tried.

    Procedure-specific criteria

Surgical procedures may be considered medically necessary when the general criteria above are met AND the procedure-specific criteria (per referenced MCG guidance) are satisfied:

Procedures: Arthrocentesis with insufflation, lysis, and lavage for intra-articular pathology (e.g., anchored disc phenomenon or disc dislocation) when imaging and clinical examination support intra-articular disease with pain or dysfunction; Arthroscopy when general criteria are met plus the clinical criteria in MCG A-0492; Modified Condylotomy when general criteria are met plus the clinical criteria in MCG A-0521; Arthrotomy when general criteria are met plus the clinical criteria in MCG A-0522; Arthroplasty using an FDA-approved implant when general criteria are met plus the clinical criteria in MCG A-0523.MCG referenced criteria

Arthroplasty requires use of an FDA-approved implant as specified.

    Exceptions to conservative therapy requirement

    Conservative therapy waiver conditions:

Waiver: When indications for arthroplasty are met under MCG A-0523 and surgery is indicated for tumor removal requiring joint reconstruction, surgery may be considered medically necessary without failure of a six (6) month trial of provider-directed conservative therapy.tumor removal indication

Exception applies only when tumor removal necessitates joint reconstruction as documented and when MCG A-0523 indications are met.

Network Health considers orthognathic surgery (for example, LeFort procedures, reconstruction of mandibular rami, genioplasty) performed for the primary purpose of correcting temporomandibular joint disease or dysfunction to be experimental/investigational due to insufficient evidence supporting effectiveness for this indication in the medical literature.

The use of temporomandibular joint implants that are not FDA-approved, or the use of TMJ implants for surgical indications beyond arthroplasty or hemiarthroplasty, is regarded as experimental/investigational because peer‑reviewed evidence does not adequately demonstrate long‑term safety and efficacy.

TMD surgical procedures are considered not medically necessary when the policy’s general and procedure‑specific criteria are not met. This includes surgeries requested for individuals who are asymptomatic or for those who have demonstrated adequate improvement of symptoms with non‑surgical modalities.

Additional scenarios that will not be covered include any surgical intervention that does not meet the documented medical criteria in this policy, procedures that are contractually excluded by the member’s coverage document, and operations performed when symptoms have resolved or sufficiently improved with conservative therapy. Prior authorization and documentation requirements still apply where coverage is otherwise indicated.

    Provider Requirements and Prior Authorization

    Prior Authorization

Prior authorization required with imaging and conservative therapy documentation

Prior authorization is required for listed TMJ procedures. The request must include documentation that the general medical necessity criteria are met (persistent pain/functional impairment corresponding to an intra-capsular condition), confirmatory imaging (MRI or CT), and when applicable a six (6) month trial of provider-directed nonsurgical management as described in policy. Refer to MCG guidance for procedure-specific criteria when applicable.

    • Prior authorization applies to the policy’s listed CPT/HCPCS codes (e.g., 20605, 20606, 21010, 21240, 21242, 21243, 29800, 29804).
    • Procedure-specific MCG criteria (e.g., A-0492, A-0521, A-0522, A-0523) must be satisfied for the corresponding surgical intervention.
    Step Therapy

Six-month provider-directed conservative management trial required

A trial of conservative (non-surgical) management for six (6) months is required prior to most surgical interventions for TMD. The trial must be provider-directed and include the conservative therapies listed in the policy.

    Definitions and Conservative Management

    Conservative management — non-surgical treatments
DefinitionNon-surgical treatments including diet modification (e.g., soft chew diet), jaw rest, moist heat, NSAIDs (unless contraindicated), intraoral appliance (splint), and physical therapy.
Required trial durationSix (6) month trial of provider-directed nonsurgical management is required prior to most surgical interventions.
Examples of conservative measuresSoft chew diet, moist heat application, NSAID therapy, intraoral splint, and structured physical therapy program.
PurposeAttempt symptom resolution or functional improvement and avoid or defer surgical intervention when possible.
Intracapsular condition — pathology within TMJ capsule
DefinitionA pathology within the TMJ capsule (intracapsular) such as anchored disc phenomenon (ADP) or disc dislocation that corresponds to persistent pain and/or functional impairment.

    Background on Temporomandibular Disorders (TMD)

Temporomandibular disorders (TMD) encompass a range of conditions affecting the temporomandibular joint and surrounding structures and commonly present with jaw pain, limited mandibular motion, joint noises, locking, and functional impairment. Symptoms may resolve spontaneously or improve with non‑surgical measures such as a soft diet, jaw rest, moist heat, NSAIDs, intraoral splints, and physical therapy. Surgical intervention, including arthrocentesis, arthroscopy, arthrotomy, arthroplasty, or joint reconstruction, is reserved for cases with documented persistent pain or functional limitation that correspond to an intracapsular condition confirmed by MRI or CT and that have failed a provider‑directed trial of conservative management (generally 6 months), except in limited circumstances such as tumor removal requiring joint reconstruction.

    • Conservative management must include diet modification (e.g., soft chew diet), moist heat, NSAIDs (unless contraindicated), intraoral appliance (splint), and physical therapy.
    • Exception: waiver of the 6-month trial is allowed when arthroplasty is indicated for tumor removal requiring joint reconstruction (see policy).
    Documentation Required

Required documentation: symptoms, confirmatory MRI/CT, and conservative therapy details

Document persistent pain and/or functional impairment that directly corresponds to an intra-capsular condition, provide confirmatory MRI or CT imaging, and list the conservative therapies tried including duration (six months) and responses.

    • Correlate clinical findings (pain/functional impairment) with MRI or CT evidence of intra-capsular pathology.
    • Specify the conservative modalities used (diet modification, moist heat, NSAIDs unless contraindicated, intraoral appliance/splint, physical therapy) and dates/duration of each.
    • Include the CPT/HCPCS code(s) for the requested procedure and supporting documentation that procedure-specific criteria (per MCG) are met when applicable.
    Denial Risk

Denial risk if general or procedure-specific criteria not met

Procedures may be denied when the policy’s general and/or procedure-specific criteria are not met, including lack of required imaging confirmation or failure to document the six-month conservative therapy trial when applicable.

    • Denial may occur if the individual is asymptomatic or demonstrates adequate improvement with non-surgical modalities.
    • Denial may occur if MCG-referenced procedure-specific criteria are not satisfied or required FDA-approved implant criteria (for arthroplasty) are unmet.
Imaging confirmationIntracapsular condition must be confirmed by magnetic resonance imaging (MRI) or computed tomography (CT).
Clinical correlationDocumentation must show persistent pain and/or functional impairment that directly corresponds to the intracapsular pathology.
Procedure relevancePresence of intracapsular pathology (e.g., ADP or disc dislocation) supports procedures such as arthrocentesis, arthroscopy, arthrotomy, or arthroplasty when general criteria are met.
