    Explicit adolescent-specific duration

Covered procedures (by transition direction and other interventions)

Procedure-specific coverage when pre-procedure criteria are met

Female-to-male/genitourinary procedures: When all pre-procedure criteria are met, the following breast and genital surgeries may be considered medically necessary: breast reconstruction (e.g., reduction mammaplasty), metoidioplasty, hysterectomy, penile prosthesis insertion, salpingo-oophorectomy, phalloplasty, scrotoplasty, urethroplasty, testicular prosthesis implantation, vaginectomy, and vulvectomy (simple, complete).

List in chunk 3 provides procedures

Male-to-female/genitourinary procedures: When all pre-procedure criteria are met, the following breast and genital surgeries may be considered medically necessary: breast augmentation, clitoroplasty, colovaginoplasty, labiaplasty, penectomy, orchiectomy, vulvoplasty, and vaginoplasty.

List in chunk 4 provides procedures

Other interventions: When all pre-procedure criteria are met, additional procedures may be considered medically necessary when expected to effectively treat the individual's gender incongruence or dysphoria, including (but not limited to) abdominoplasty, blepharoplasty/blepharoptosis repair, brow lift, cheek/malar implants, calf augmentation/implants, chin augmentation, collagen injections, dermabrasion/skin resurfacing, forehead lift, facial feminization/sculpturing, hair removal (electrolysis or laser) or hair transplantation, lip reduction/enhancement, laryngoplasty, liposuction, nose implants, mastopexy, removal of redundant skin, rhytidectomy, rhinoplasty, scrotoplasty, voice modification surgery, tracheal shave/reduction thyroid chondroplasty, and voice therapy/voice lessons.

Lists in chunks 5 and 6 provide examples

    Detransition

    Detransition surgical coverage

Detransition surgery: Detransition, including surgical intervention, may be considered medically necessary when the individual is being cared for by a comprehensive multidisciplinary assessment team; social transition has been discussed and considered; the assessing health care professional (a member of the multidisciplinary team) documents how detransition is in the individual's best interest; and the individual has lived in the social role for a prolonged period of time if recommended by the multidisciplinary team.

    Conditions enumerated in chunk 6

    Medical necessity and referral criteria

Covered when documentation demonstrates alignment with diagnostic criteria and referral requirements

Diagnostic criteria: Meets DSM-5-TR criteria for gender dysphoria: a marked incongruence between experienced/expressed gender and assigned gender of at least 6 months' duration with associated clinically significant distress or impairment.duration >= 6 months

See DSM-5-TR criteria excerpt in policy

Referral documentation: Referral letter from a Qualified Health Care Professional that includes: client's identifying characteristics; the duration and type of the health care professional's relationship with the client (including evaluation/therapy/counseling to date); an explanation that the criteria for surgery have been met with a brief clinical rationale; a statement that informed consent has been obtained; and a statement that the health care professional is available for coordination of care.none

No minimum duration of relationship required; a common period is about three months.

Benefit coverage for evaluation: Evaluation of candidacy for gender-affirming surgery by a qualified health care professional is covered under the member's medical benefit unless the services are necessary to evaluate and treat a mental health problem, in which case behavioral health coverage applies.N/A

See policy note regarding coverage distinction

Coding guidance for reduction mammaplasty: per AMA guidance cited in the policy, CPT 19303 (mastectomy) is intended for treatment or prevention of breast cancer and should not be used to report breast-size reduction. When breast tissue is removed for size reduction rather than cancer treatment, report CPT 19318 (reduction mammaplasty).

The policy includes historical coding changes and administrative notes: code 19303 was deleted from the policy (8/17/2021) and code 19350 was placed into the non-covered table; subsequent policy history entries document further coding table updates and administrative additions (e.g., later additions of CPT 11920 and 11952). Providers should consult the current coding table in this policy for the up-to-date covered vs non-covered status of procedure codes.

The policy affirms that gender-affirming interventions that are properly indicated and performed in accordance with recognized standards of care are not considered experimental or investigational. The background and policy updates emphasize that such interventions (including surgeries) are intended to establish congruence between gender identity and physical appearance and may be medically necessary when criteria are met.

Policy history clarifies that procedures previously characterized as cosmetic may now be considered medically necessary when the intervention is expected to effectively treat an individual's gender incongruence or gender dysphoria. This change is reflected in the policy revisions and coding updates documented in the revision log.

    Authorization, Documentation, and Submission Requirements

    Prior Authorization

Prior authorization required for listed procedure codes

Prior authorization is required for the procedure codes listed in the policy; inclusion of a code in this policy does not guarantee coverage and is subject to member benefit terms.

    • See Procedure Codes lists in chunks 16 and 17 for codes that require authorization.
    • Coverage remains subject to member benefits and medical necessity criteria.
    Prior Authorization

    Referral letter required for authorization

Prior authorization requires submission of a referral letter from a Qualified Health Care Professional that follows the policy's referral format and contains the required elements.

    • Referral letter must be from a Qualified Health Care Professional as defined by the policy.
    • Authorization will not be complete without the referral document described in the policy.
    Note

Obtain formal recommendation from a Qualified Health Care Professional

Obtain and document a formal recommendation for the proposed surgical intervention from a Qualified Health Care Professional prior to authorization and scheduling.

    • The recommendation must come from a professional who is usually a licensed behavioral health care professional with a postgraduate degree or another clinician who documents WPATH competencies.
    • Recommendation must be supported by the comprehensive evaluation elements listed in the policy.
    Note

Verify all pre-procedure criteria are met before submission

Confirm that the member meets all pre-procedure criteria (including documented gender incongruence/dysphoria, capacity to consent, exclusion of other causes, and assessed mental/physical health risks) before submitting authorization.

    • Pre-procedure criteria listed in the policy must be met for any listed procedure to be considered medically necessary.
    • Covered procedures are only those listed and only when pre-procedure criteria are fulfilled.
    Documentation Required

    Provide comprehensive pre-procedure recommendation letter

Include a recommendation letter from a Qualified Health Care Professional containing a comprehensive evaluation that documents marked and sustained gender incongruence/dysphoria, capacity to consent, exclusion of other causes, assessment of mental and physical health conditions with risks/benefits discussed, and documentation of required hormone therapy duration.

    • For adults: documentation of at least 6 months of continuous hormonal therapy as appropriate to the individual's goals (unless contraindicated or not desired).
    • For adolescents: documentation of 12 months of continuous hormonal therapy unless medically contraindicated or not desired; include a comprehensive biopsychosocial assessment.
    Documentation Required

Referral letter must include required identifying and clinical details

Ensure the referral letter contains the client's identifying characteristics; the duration and type of the health care professional's relationship with the client; an explanation that surgery criteria have been met with clinical rationale; a statement that informed consent has been obtained; and availability for coordination of care.

    • Include type of evaluation and therapy or counseling to date and the duration of the professional relationship (no minimum required).
    • Explicitly state that criteria for surgery have been met and provide a brief clinical rationale.
    Denial Risk

Absence of required pre-procedure documentation may lead to denial

Missing required pre-procedure documentation (for example, absence of the recommendation letter or lack of documentation of gender incongruence/dysphoria, capacity, or hormone therapy duration) risks denial or non-coverage of the requested procedure.

    • Authorization and coverage decisions depend on receipt of the complete documentation specified by the policy.
    • Failure to provide required hormone therapy documentation (6 months adults; 12 months adolescents when applicable) may lead to denial.
    Denial Risk

    Incomplete referral letter risks denial

An incomplete referral letter that does not explain that surgery criteria have been met or does not provide the clinical rationale and required elements may trigger denial of authorization.

    • Missing statements about informed consent, clinical rationale, or availability for coordination are examples of deficiencies that can result in denial.
    • There is no minimum relationship duration required, but omission of the relationship description or evaluation type can render the referral incomplete.

    Clinical Background

Gender incongruence describes a condition in which an individual's experienced gender identity does not align with the sex assigned at birth. Gender dysphoria refers to the psychological distress that can result from this incongruence and, per the policy, is defined consistent with DSM-5-TR criteria (marked incongruence of at least 6 months with associated clinically significant distress or impairment).

The goal of gender-affirming care is to holistically address the social, mental, and medical needs of transgender and gender diverse people. Interventions include puberty suppression, hormone therapy, and gender-affirming surgeries; surgery is described as intended to be a permanent change to establish congruence and is not easily reversible. The policy also documents updates to terminology and scope (e.g., use of “gender diverse”) and the procedural intent to align coding and coverage with clinical standards.
