Coverage Criteria

Outpatient respiratory panel coverage

Covered when ALL of the following are met:

Outpatient respiratory panel coverage: The test is a multiplex PCR-based respiratory panel that detects up to 5 respiratory pathogens.<= 5 targets

Clinical presentation: The individual is displaying signs or symptoms of a respiratory tract infection, including at least one of: elevated temperature, pronounced dyspnea, tachycardia, or tachypnea.

Setting: Testing is performed in the outpatient setting.

This policy excludes reimbursement for certain multiplex molecular pathogen panels. Specifically, multiplex PCR-based respiratory panels that detect 6 or more respiratory pathogens are not reimbursable. The policy also lists several anatomic-site and purpose-based exclusions that are not covered for outpatient reimbursement under this policy, including panels for cerebral spinal fluid (CSF), molecular detection–based panels of pathogens in blood, molecular urine pathogen panels for diagnosis of urinary tract infections, molecular wound infection panels, and broad molecular-based microbial screening tests.
