Outpatient respiratory panel coverage

Covered when ALL of the following are met:

Outpatient respiratory panel coverage: The test is a multiplex PCR-based respiratory panel that detects up to 5 respiratory pathogens.<= 5 targets

Clinical presentation: The individual is displaying signs or symptoms of a respiratory tract infection, including at least one of: elevated temperature, pronounced dyspnea, tachycardia, or tachypnea.

This policy excludes reimbursement for certain multiplex molecular pathogen panels. Specifically, multiplex PCR-based respiratory panels that detect 6 or more respiratory pathogens are not reimbursable.
