    Coverage and Medical Necessity Criteria

    inv-01: Medically necessary indications

Covered when ALL of the following are met:

ANA screening in symptomatic individuals: For individuals with signs or symptoms of an autoimmune disease, screening using antinuclear antibodies (ANA) may be reimbursable.Once during initial workup; up to two additional tests per lifetime if new or more severe signs or symptoms develop

    Statements 1, 1a, 1b

ENA panel after abnormal ANA: For individuals with an abnormal, raised ANA titer and clinical correlation with the appropriate autoimmune disorder, extractable nuclear antigens (ENA) panel testing of specific autoantibodies may be reimbursable.

    Statement 2

RF and anti-CCP for suspected RA: For individuals with painful and swollen joints suggestive of rheumatoid arthritis (RA), testing for rheumatoid factor (RF) and/or anti-cyclic citrullinated peptide (anti-CCP) antibodies may be reimbursable.Once during initial workup; if initial testing did not result in a diagnosis, up to two additional tests per lifetime if symptoms persist or additional symptoms develop

    Statement 3 and 3b

dsDNA monitoring in diagnosed systemic autoimmune rheumatic disease: For individuals with an initial positive ANA test and a diagnosis of systemic autoimmune rheumatic disease, testing for dsDNA may be reimbursable.Up to 4 times per year

    Statement 4

Condition-specific antibody testing when appropriate: For individuals with a negative or low positive ANA test, certain condition-specific antibody testing may be reimbursable (examples: anti-SSA in lupus or Sjogren's syndrome; anti-Jo-1 in selected myositis cases).

    Statement 5 and 5a/5b

inv-02: Not medically necessary / Not reimbursable

Not reimbursable in the following situations:

Asymptomatic or wellness testing: ANA and/or ENA testing for individuals without symptoms suggestive of an autoimmune disorder, including during wellness visits or general exams without abnormal findings, is not reimbursable.

    Statements 7 and 9

Specific antibody testing without positive ANA: Testing of specific antibodies in the absence of a positive ANA test is not reimbursable except as described in the condition-specific exceptions above.

    Statement 8

Proprietary multianalyte/panel tests: Serum biomarker panel testing that uses proprietary algorithms and/or index scores (examples: AVISE CTD, AVISE SLE Monitor/SLE prognostic, AVISE Lupus, aisle DX Disease Activity Index, Early Sjogren's Syndrome Profile), and proprietary RA panels or disease activity scores (examples: Vectra DA, PrismRA), are not reimbursable for diagnosis, prognosis, or monitoring.

    Statements 11-13

The policy distinguishes routine panel testing from targeted diagnostic evaluation. ANA/ENA testing for asymptomatic individuals or during wellness visits is not reimbursable; these tests are intended to support evaluation when there are clinical signs or symptoms suggestive of an autoimmune rheumatic disease. The policy also excludes a range of proprietary multianalyte and index-score panels (examples include AVISE CTD, AVISE SLE Monitor/prognostic, aisle DX Disease Activity Index, Early Sjogren's Syndrome Profile) and specific RA panel tests (such as Vectra DA and PrismRA) from reimbursement for diagnosis, prognosis, or monitoring.

For rheumatoid arthritis specifically, the policy operationally excludes testing of serum biomarker panels that are not discussed among the allowed RA-related tests. Testing for serum biomarkers not discussed above, alone or in a panel (e.g., Seronegative Rheumatoid Arthritis Profile), is not reimbursable for the diagnosis of RA.

The policy clarifies permitted and non-permitted uses of ANA and related antibody testing. Monitoring disease with ANA testing or ANA titers is not reimbursable; ANA is intended as a diagnostic/screening tool in symptomatic evaluation rather than for routine longitudinal monitoring of disease activity.

Additionally, the policy states that testing for specific antibodies in the absence of a positive ANA is generally not reimbursable except where condition-specific exceptions are described (for example, anti-SSA in lupus or Sjogren's, and anti-Jo-1 in select myositis cases). Providers should therefore ensure a positive ANA (or an allowed exception) and relevant clinical correlation before ordering isolated specific-antibody testing.

Provider Actions, Documentation, and Denial Triggers

    Prior Authorization

Verify coverage and potential prior authorization

Verify coverage and prior authorization requirements before ordering tests. Providers must check member benefits and plan documents to confirm if the service is a covered benefit and whether prior authorization or clinical payment/coding policy review is required. Use the specific procedure and test codes when checking eligibility and prior authorization systems to determine coverage and any PA requirements.

    • Check member plan documents (e.g., Certificates of Benefits, summary plan descriptions) for coverage specifics.
    • Verify prior authorization using procedure/test codes before performing tests.
    • When in doubt, submit clinical information and request PA prior to service delivery.
    Note

    Operational notice — provider responsibilities

Providers are responsible for accurate coding and submission of documentation for services rendered. Claims are subject to code edit protocols, provider contract language, medical policies, and CPCP — plan documents and provider contracts govern if conflicts arise. BCBSNM may request additional documentation and has discretionary authority in interpretation and application of this policy.

Extractable nuclear antigen (ENA) panelUsed to evaluate specific autoantibodies when ANA titer is abnormal and clinically correlated with an autoimmune disorder; reimbursable in that context.
Use limitationsANA/ENA testing is not reimbursable for asymptomatic individuals, during wellness visits, or without appropriate clinical signs.
    inv-13: Proprietary multianalyte/index-score panels
DefinitionProprietary multianalyte/index-score panels are commercial tests that combine multiple biomarkers into algorithm-derived scores for diagnosis, prognosis, or monitoring.
    Examples (not reimbursable)

    Background

Autoimmune rheumatic diseases are evaluated using serologic biomarkers such as antinuclear antibodies (ANA), extractable nuclear antigens (ENA), rheumatoid factor (RF), anti-cyclic citrullinated peptide (anti-CCP), and anti-dsDNA. The policy recognizes these tests as useful in diagnostic workup when clinical signs or symptoms suggest an autoimmune process: for example, ANA screening may be reimbursable once during the initial workup with up to two additional tests per lifetime if new or more severe symptoms develop, and RF/anti-CCP testing may be reimbursable in the workup of suspected RA with similar repeat allowances.

The policy also reflects contemporary test availability and utilization: commercially marketed proprietary multianalyte or algorithm-derived index-score panels exist to combine multiple biomarkers into disease activity or diagnostic scores, but several of these (examples called out include AVISE family tests, Vectra DA, PrismRA, aisle DX, and Early Sjogren's Syndrome Profile) are explicitly treated as not reimbursable for diagnosis, prognosis, or monitoring under this policy.

Unspecified RA biomarker panels for diagnosis: For the diagnosis of rheumatoid arthritis, testing for serum biomarkers not discussed above, whether performed alone or as part of a panel (e.g., Seronegative Rheumatoid Arthritis Profile), is not reimbursable.

    Statement 10

    • Submit claims using valid HIPAA-approved code sets (CPT, HCPCS, ICD-10, NDCs) and follow industry coding guidelines.
    • Respond promptly to requests for additional documentation to support billed services.
    Documentation Required

    Clinical documentation required

Clinical documentation must be submitted to support medical necessity when billing for ANA, ENA, RF, anti-CCP, dsDNA, and other autoimmune serology tests. Documentation should include signs, symptoms, physical findings, and clinical correlation demonstrating the need for testing.

    • Include clinical signs or symptoms of autoimmune disease when billing ANA/ENA screening or reflex testing.
    • For rheumatoid arthritis testing (RF, anti-CCP), document joint symptoms and clinical suspicion; note that repeat testing is limited per policy.
    • For positive ANA with suspected systemic autoimmune disease, document the diagnosis and clinical rationale for dsDNA testing and frequency.
    • Do not bill for tests ordered for screening in asymptomatic individuals without supporting clinical findings.
    Denial Risk

Denial triggers for noncovered tests and inappropriate indications

Claims are at high risk of denial when tests are ordered for inappropriate indications, as noncovered, or as proprietary/algorithmic panel tests. Examples likely to be denied include asymptomatic screening ANA/ENA, testing of specific antibodies without a positive ANA, monitoring disease with ANA titers, and proprietary serum biomarker panels (e.g., AVISE, Vectra DA, PrismRA, proprietary index scores). Also verify coverage for listed molecular/proprietary CPT and PLA codes prior to billing.

    • Not reimbursable: ANA/ENA testing for asymptomatic individuals or during routine wellness exams.
    • Not reimbursable: Specific antibody testing in absence of a positive ANA result.
    • Not reimbursable: Monitoring disease activity with ANA titers.
    • Not reimbursable: Proprietary serum biomarker panels and tests with algorithmic/index scores (e.g., AVISE CTD, AVISE SLE Monitor, Vectra DA, PrismRA).
