These techniques are considered experimental/investigational because their reliability, validity and clinical value have not been established. Systematic reviews have methodological limitations and evidence is insufficient to support diagnostic accuracy or clinical utility.

This policy addresses the use of surface scanning electromyography (surface scanning EMG) and related techniques, including high-density surface EMG (HD-sEMG), paraspinal surface EMG, and macro-EMG, as diagnostic tests. It also covers related procedures such as the Neurophysiologic Pain Profile (NPP)/spine matrix scan and Spinoscopy (Spinoscope®). These modalities are considered experimental and investigational when used to evaluate low back pain or other thoracolumbar segmental abnormalities (for example, soft tissue injury, intervertebral disc disease, nerve root irritation, and scoliosis) and for all other indications because their reliability, validity and clinical value have not been established.

Published technical and pilot data show that HD-sEMG can provide richer spatial maps of muscle activation than single bipolar recordings and may improve identification of movement tasks in research or device-control applications. However, the policy concludes that the clinical effectiveness of HD-sEMG has not been established, and states that well-designed studies are needed to determine whether HD-sEMG improves diagnostic accuracy, clinical decision-making, or patient outcomes.

Surface scanning EMG (including paraspinal EMG and HD-sEMG), spine matrix scans (NPP), and Spinoscopy have been proposed for diagnosing back-related conditions based on recorded myoelectrical patterns. The policy notes that surface scanning EMG is distinct from conventional needle EMG and from surface electrodes used in medically necessary biofeedback. Systematic reviews and the existing literature reveal methodological limitations and inconsistent relationships between surface EMG measures and clinical low back pain, and therefore these surface scanning techniques have not been proven effective for routine diagnostic use in low back pain or thoracolumbar segmental disorders.

Although HD-sEMG demonstrates technical promise in mapping spatial activation and distinguishing task-related muscle activity in small studies, the policy considers the use of HD-sEMG for routine clinical diagnostic purposes to be unproven/experimental. The available evidence is insufficient to support its clinical utility for diagnosis or management decisions, and the policy recommends that additional robust clinical research is required before HD-sEMG is adopted for routine diagnostic use.
