OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Regimen reserved for patients with prior exposure to ≥2 therapies including lenalidomide and a proteasome inhibitor

    • Bortezomib regimen: Elotuzumab used in combination with bortezomib and dexamethasone

    Renewal Therapy Criteria

Renewal coverage requires ALL of the following

Continuation of indication-specific criteria: Member continues to meet the indication-specific relevant criteria such as concomitant therapy requirements and performance status (does not include prerequisite therapy)

See initial criteria for regimen- and indication-specific requirements

Disease response: Documented disease response defined as stabilization of disease or decrease in size of tumor or tumor spread
Toxicity: Absence of unacceptable toxicity from elotuzumab (examples: severe infusion reactions, infections, second primary malignancies, hepatotoxicity)

Renewal may be denied for unacceptable toxicity

This policy is limited to use of elotuzumab (Empliciti) for previously treated, relapsed or progressive disease and does not provide coverage for primary prophylactic use or frontline-only indications. Coverage is specific to the combination regimens listed under the multiple myeloma and related plasma-cell disorder indications and requires that the member meet the regimen-specific criteria (see covered regimens and authorization requirements).

    Documentation Required

Provide documentation of diagnosis and prior treatment history

Document clinical evidence that supports a diagnosis of previously treated relapsed or progressive multiple myeloma (or specified plasma-cell disorders) and the member's prior treatment history, including therapies such as lenalidomide and a proteosome inhibitor; also document member age ≥18.

    • Clinical documentation must show relapsed or progressive disease and the specific combination regimen being used.
    • Include prior therapy details when applicable (e.g., number and type of prior regimens).
    • Include member demographic information confirming age ≥ 18.
    Denial Risk

Expect denials for unacceptable toxicity or lack of response at renewal

Renewal requests may be denied if the member has unacceptable toxicity or lacks disease response; examples of unacceptable toxicity include severe infusion reactions, infections, second primary malignancies, and hepatotoxicity.

    • Renewal requires documented disease response (stabilization or decrease in tumor size/spread).
    • Denial risk if there is unacceptable toxicity (examples listed by the policy).
