General indications and preconditions for lumbar fusion

Covered when ALL of the following are met (as supported by document text):

Indications and preconditions: Presence of underlying pathology as determined by radiological findings (e.g., instability, progressive deformity, infection, traumatic instability) or other clearly defined indications such as progressive neurological deficits; conservative management may be attempted and failed unless contraindicated

Imaging findings required per applicable procedure policy

    Approach selection

    Choice of surgical approach considerations

Approaches: Anterior (ALIF), posterior (PLIF, TLIF), lateral (XLIF/DLIF), AxiaLIF, and minimally invasive variations are appropriate alternatives depending on anatomy and pathology; each approach has specific risks and benefits

    Smoking and perioperative risk considerations

Coverage decisions should consider smoking status and its impact on outcomes

Smoking impact: Smoking is a risk factor for poor fusion healing and increased complications; non‑smoking is defined as serum cotinine ≤ 10 ng/mL; cessation is associated with reduced odds of complications compared with active smokingserum cotinine <= 10 ng/mL

Disc herniation / Degenerative Disc Disease (DDD)

    Fusion coverage for disc herniation/DDD

Disc herniation: Lumbar fusion is not supported as an adjunct to primary discectomy for disc herniation in the absence of instability; surgical discectomy alone improves outcomes for lumbar disc herniation with radiculopathy
DDD and chronic low back pain: Evidence is limited that lumbar fusion is superior to non‑operative care for chronic low back pain due to degenerative disease; meta‑analyses show no clinically significant improvement in ODI compared with conservative treatment

    Coverage-related evidence summaries by indication

Summary of evidence by indication (narrative)

DDD/CLBP evidence: Evidence supporting lumbar fusion as superior to nonoperative therapy is limited; meta‑analysis found no clinically significant reduction in ODI versus conservative treatment
Early surgery vs conservative for radicular pain: One large trial showed early surgery (6–12 weeks) yields faster short‑term pain relief vs prolonged conservative care but no differences at one and two years6–12 weeks of radicular pain
Degenerative spondylolisthesis with stenosis: SPORT trial analysis showed significant advantage for surgery (mostly decompression with fusion) up to four years for patients with neurogenic claudication or radicular pain persisting ≥12 weekssymptoms >=12 weeks

    Technique and device evidence statements

Device and approach evaluations (evidence statements):

Dynamic stabilization devices: Insufficient research to show improved health outcomes; not recommended by clinical guidelines
Personalized 3‑D cages: Insufficient research demonstrating equivalence or superiority to conventional cages; not supported
Minimally invasive LIF: Mid‑term outcomes comparable to open approaches after learning curve; intra‑operative outcomes (blood loss, LOS) often improved

Requires training; evidence from nonrandomized studies

DLIF/XLIF evidence: DLIF/XLIF show comparable outcomes; approach‑related transient neurologic symptoms and other complications reported in small series
Laparoscopic ALIF: Literature does not permit strong conclusions vs open fusion; potentially higher complication and conversion rates

    Evidence-based coverage considerations

Summary of coverage‑relevant evidence and guideline positions:

AxiaLIF evidence: Insufficient evidence whether AxiaLIF is as effective or safe as established techniques; case series exist but comparative evidence is limited
Interspinous fixation devices: Evidence lacking on efficacy of IFDs with interbody fusion; randomized trials with longer follow‑up needed; use without open decortication and fusion is not supported
Laparoscopic ALIF outcomes: Laparoscopic ALIF series show modestly shorter LOS for single‑level cases but higher operative time, conversion‑to‑open (~10%), and mixed complication/reoperation profiles vs open/mini‑open

When instability is identified or created during the operation, preoperative imaging documentation of instability is not required. This intra‑operative exception applies when iatrogenic spondylolisthesis or other instability is discovered or produced at the time of surgery, and allows fusion to be considered without meeting the usual pre‑op imaging thresholds.

A corpectomy counts toward anticipated iatrogenic instability only when a substantive portion of the vertebral body is removed. The policy specifies that removal must be at least one‑third of the vertebral body; corpectomy performed solely for osteophyte resection does not meet the extent requirement.

The excerpt provided does not enumerate additional specific exclusions in this section beyond the policy statements elsewhere. Reviewers should rely on the full policy text for any other named exclusions not present in the provided excerpt.

Documentation of nicotine‑free status is a required element. If an individual cannot document nicotine‑free status either by a record showing they are a never smoker or by objective cotinine testing demonstrating ≥ 6 weeks abstinence (serum, urine, or saliva with results within the laboratory’s normal range), the patient fails the nicotine requirement and fusion is not supported under these criteria.

Active nicotine use without documentation of abstinence is not acceptable. The policy requires that individuals either be documented as a never smoker or have objective cotinine testing validating abstinence for at least 6 weeks prior to the planned procedure; active nicotine use without such documented abstinence will preclude meeting the nicotine‑free requirement.

Patients with untreated underlying mental or behavioral health disorders are excluded until those conditions are addressed. Examples provided in the policy include depression, chronic pain syndrome, secondary gain concerns, and opioid or alcohol use disorders; documentation must show these issues are absent or adequately treated before fusion is supported.

Fusion is not supported when adequate conservative therapy has not been attempted. For repeat fusion for symptomatic pseudoarthrosis the policy requires at least 6 weeks of non‑surgical treatment (prescription‑strength analgesics/steroids/gabapentinoids/NSAIDs AND a provider‑directed exercise program). More generally, other indications require conservative management failures of specified durations (for example, ≥3 months for many indications and ≥12 months for discogenic DDD) before surgery is supported.

Provider Requirements, Prior Authorization, and Documentation

    Prior Authorization

Meyerding grade criteriaMeyerding Grade II or higher qualifies for degenerative spondylolisthesis; Grade I–II progression supports isthmic cases
    Grade definitionsGrade I 0–25%, Grade II 25–50%, Grade III 50–75%, Grade IV 75–100%, Grade V >100%
ApplicationUsed in imaging-based eligibility for fusion coverage
inv-173: Postoperative iatrogenic instability — Instability produced by facet joint excision thresholds
Postoperative iatrogenic instability definitionInstability produced by facet joint excision exceeding 50% bilaterally or 75% of a single facet
Clinical implicationMay warrant fusion when postoperative instability is created
DocumentationOperative report or imaging demonstrating extent of facet excision
inv-174: Anticipated iatrogenic instability definition — Expected instability after planned surgical disruption (facet excision, pars interarticularis removal, corpectomy extent)
Anticipated iatrogenic instability definitionExpected instability after planned surgical disruption (e.g., >50% bilateral or ≥75% unilateral facet excision; removal of pars interarticularis; corpectomy ≥1/3 vertebral body)
ExamplesFacet excision >50% bilaterally; single facet ≥75%; pars removal requiring stabilization; corpectomy removing ≥1/3 of vertebral body
DocumentationPre-op surgical plan describing intended resection and need for stabilization
inv-175: Adult degenerative spinal deformity definition — Coronal and/or sagittal deformity thresholds and examples
ASD definitionCoronal deformity (Cobb >30°) and/or sagittal imbalance (SVA >8 cm or PI-LL >15°) constitute adult degenerative spinal deformity
ExamplesAsymmetric disc collapse causing foraminal narrowing; global coronal imbalance; sagittal imbalance measures
PrerequisiteCandidate for decompression or corpectomy with concordant symptoms and failed conservative care per criteria

    Quick Reference: Codes and Thresholds

Quick reference: see the policy coding sections for the full lists of applicable CPT codes and the thresholds defined elsewhere in the policy (for example, translational instability > 3 mm, nicotine‑free abstinence ≥ 6 weeks validated by cotinine testing).
