OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Beds, Mattresses, and Safety Enclosures

    Safety Enclosure Coverage Criteria

Safety enclosure with beds is covered when ALL of the following are met:

Safety enclosure coverage criteria: Use of equipment is required due to a diagnosis related to cognitive impairment (e.g., traumatic brain injury, cerebral palsy, seizure disorder) or a severe behavioral disorder
Medication optimization: Medications to address seizures and/or disruptive or harmful behaviors have been optimized or are not appropriate
Safety risk present: There is a safety risk (examples include claustrophobia; high risk of falls due to a clinical condition; uncontrolled movements; violent or self-destructive behaviors such as uncontrolled head banging)

    Examples listed in policy

Conservative measures attempted: Less intensive alternative methods (e.g., mattress placed on the floor, medical helmet, removal of all safety hazards, side rails, weighted blankets) have been tried as appropriate and have not been successful or are contraindicated

Enclosed bed systems and safety enclosures with beds are subject to an explicit exclusion when the equipment is used to restrain a member. Enclosed bed systems are not covered when the purpose is to restrain the beneficiary due to behavioral conditions, caregiver need, or convenience. This operational denial basis applies regardless of the specific safety enclosure product and should be considered when submitting claims or prior authorization requests; note that safety enclosure frames/canopies are reported with HCPCS code E0316 when billed.

Prior Authorization, Conservative Measures, and Denial Risks

    Prior Authorization

Follow InterQual CP criteria and submit documentation for prior authorization

Coverage decisions for durable medical equipment (support surfaces; hospital beds, cribs, and accessories) reference the InterQual® CP: Durable Medical Equipment criteria. Providers must follow InterQual criteria and submit applicable documentation when requesting prior authorization for DME items listed in the policy (for example HCPCS codes such as E0193, E0194, E0250, E0316, E1399, etc.).

    • Submit documentation that demonstrates the patient meets InterQual CP criteria for the requested DME item.
    • Include applicable HCPCS code(s) on the prior authorization request (policy lists E0193, E0194, E0250–E0316, E0328–E0329, E0910–E0911, E1399, and others).
    Step Therapy

Document trial of conservative measures before escalation

Before requesting coverage of a safety enclosure or more intensive support surface, document that less intensive alternatives were tried and were unsuccessful or are contraindicated. The policy lists specific alternatives that should be attempted as appropriate.

    Background and Clinical Context

Support surfaces — including overlays, specialty mattresses, powered air flotation, and air-fluidized systems — serve two principal roles: prevention of pressure injuries for individuals at risk, and treatment/management of advanced pressure injuries. Therapeutic support surfaces are indicated to assist with advanced wounds such as stage 3, stage 4, or unstageable pressure injuries and are referenced in coverage decisions using InterQual Durable Medical Equipment criteria for support surfaces.

inv-10: Support surfaces — definition and examples
Support surfaces — definitionMattress overlays, mattresses, and specialty air‑fluidized beds used preventively or therapeutically
Therapeutic indication (most actionable)Therapeutic support surfaces are indicated to assist individuals with advanced pressure injuries (stage 3, stage 4, or unstageable)
Preventive useFor individuals at risk for skin breakdown, preventive support surfaces are often incorporated into treatment
Specialty beds — definitionBeds designed to keep individuals with altered self‑awareness and/or neurological impairments from falling or having an injury; often have fewer gaps to reduce entrapment and may require caregiver assistance to exit

    DME Medical Necessity Criteria

    DME Medical Necessity Criteria

Support surfaces are indicated for individuals at risk for skin breakdown (preventive) or to assist individuals with advanced pressure injuries (stage 3, stage 4, or unstageable)

Refer to InterQual CP: Support Surfaces

Safety enclosures: Safety enclosure covered when criteria in Safety Enclosure Coverage Criteria are met

    See Safety Enclosure Coverage Criteria

    Documentation Requirements and Submission

    Documentation Required

Submit documentation supporting InterQual criteria and clinical necessity

When submitting a DME request, include documentation that demonstrates the InterQual criteria and overall clinical necessity are met; the medical record must fully support the request.

    • Provide relevant medical history, physical examination findings, and results of pertinent diagnostic tests or procedures.
    • Ensure documentation is legible and available upon request to support prior authorization and coverage decisions.

    Items and Uses Not Covered

Not covered: Enclosed bed systems used to restrain the beneficiary for behavioral control, for caregiver convenience, or whenever the primary purpose of the equipment is restraint. This exclusion is an operational denial basis and applies even when a safety enclosure product (for example, safety enclosure frames/canopies reported with HCPCS E0316) is furnished. Providers should not bill enclosed bed systems for restraint purposes and should ensure clinical documentation demonstrates that the requested equipment meets the policy’s medical necessity criteria rather than serving as a restraint.

    Documentation Required

Ensure complete, legible medical record documentation to support medical necessity

The patient's medical record must contain documentation that fully supports medical necessity for the requested DME; this must include relevant medical history, physical examination, and results of pertinent diagnostic tests or procedures and be legible and available upon request. Documentation may be requested during review and its presence does not guarantee coverage.

    • Include relevant medical history and physical exam findings that support the need for the device.
    • Include results of pertinent diagnostic tests or procedures that substantiate medical necessity.
    • Maintain legible documentation in the medical record and make it available upon request.
    Denial Risk

Avoid billing for enclosed beds used as restraints; use E0316 when appropriate

Enclosed bed systems are not covered when used to restrain a beneficiary for behavioral control, caregiver convenience, or similar purposes; safety enclosures should be coded with HCPCS E0316 when billed, and claims may be denied on this operational basis.

    • Do not request coverage or bill for an enclosed bed system when the intended purpose is restraint or caregiver convenience.
    • When billing an appropriate safety enclosure, use HCPCS code E0316 per the policy note.
