If required, breast implants including tissue expanders and implant materials are covered when placed in the area where natural breast tissue has been removed for a medically necessary mastectomy or to achieve symmetry after medically necessary breast surgery; C. Nipple reconstruction and/or tattooing are covered when criteria in V.A are met; D. Periprosthetic capsulotomy/capsulectomy procedures are covered for contractions or adhesions following reconstruction when caused by medically necessary chemotherapy or radiation.

Referenced clinical criteria and assessment tools

Supporting assessment elements referenced for medical necessity determinations

Adolescent assessment: Use Tanner staging (Appendix A) to evaluate breast development in adolescents when considering breast surgery; Tanner stages 1–5 are defined in Appendix A.
Reduction mammoplasty threshold: Use the Schnur Sliding Scale (Appendix B) to map body surface area (BSA) to minimum grams of tissue to be removed per breast when assessing necessity for reduction mammoplasty; values provided in Appendix B (e.g., BSA 1.35 = 199 g through ≥2.30 = 1000 g).

If resected tissue weight falls below the 22nd percentile per BSA, medical director review is required.

Procedure definitions for coverage decisions: CPT definitions provided to clarify procedural intent for peri-implant capsulectomy (19371), revision of reconstructed breast (19380), and tattooing (11920).

The policy explicitly lists procedures that are excluded as cosmetic and therefore not covered. Examples include breast implants used solely for cosmetic enlargement, removal of implants for cosmetic purposes, augmentation mammaplasty for cosmetic purposes, correction of inverted nipples, preparation of moulage for custom implants, and periprosthetic capsulotomy/capsulectomy procedures following augmentation. Additionally, breast reduction and mastopexy are not covered unless the applicable medical necessity criteria are met.

Procedures performed solely for cosmetic purposes, as enumerated in the exclusions (for example, cosmetic breast augmentation or implant removal for cosmetic reasons), are considered not covered / not medically necessary. Requests for such procedures will be denied when documentation shows the sole intent is cosmetic rather than treatment of a medical condition or reconstruction meeting clinical criteria.

For Medicaid members, state Medicaid coverage provisions take precedence when they conflict with this clinical policy. Providers should consult the applicable state Medicaid manual for specific coverage rules and follow state-level provisions where discrepancies exist.

This clinical policy is intended as a guide to medical necessity to assist with coverage determinations and benefit administration; it does not guarantee payment or direct clinical care. Providers are expected to exercise professional medical judgment and to follow applicable plan-level administrative policies and procedures when submitting documentation to support medical necessity determinations.

    Background and Scope

This policy establishes medical necessity criteria and coverage guidance for a range of breast surgeries, including surgical management of gynecomastia, prophylactic mastectomy for high-risk patients, reduction mammoplasty for symptomatic macromastia or gigantomastia, and breast reconstruction following mastectomy or for severe disfigurement. It references required diagnostic confirmation (e.g., imaging and pathology for glandular tissue in gynecomastia), risk assessment or genetic testing for prophylactic indications, and objective thresholds for reduction mammoplasty such as the Schnur Sliding Scale (Appendix B) mapping body surface area to minimum grams of tissue to be removed.

    Clinical Guidelines and References

This policy references clinical guidance and literature to support assessment and coverage decisions. Key bibliographic sources include the State of North Carolina Medicaid Clinical Coverage Policy No:1A-12 — Breast Surgeries, the ASPS Evidence-Based Clinical Practice Guideline Revision: Reduction Mammaplasty (2022), ASPS recommended insurance coverage criteria for reduction mammoplasty and gynecomastia, ACOG Committee Opinion on breast and labial surgery in adolescents, UpToDate reviews on gynecomastia and breast reduction, and foundational research on the Schnur Sliding Scale and body surface area calculations. These references inform thresholds, conservative therapy requirements, and adolescent assessment (Tanner staging) used by the policy.

    Step Therapy

Document failure of at least two conservative therapies prior to reduction mammoplasty

For reduction mammoplasty requests, include documentation showing failure of conservative therapies. The physician must document therapeutic measures and evidence that at least two conservative treatments were tried before surgery.

    • Acceptable conservative measures include at least two of: NSAIDs/analgesics; physical therapy/exercise when skeletal pathology present; proper supportive devices (e.g., bras, wide straps); medically supervised weight loss program; chiropractic or osteopathic manipulative treatment; orthopedic or spine surgeon evaluation of spinal pain.
    Documentation Required

Include height/weight/age and unclothed pre-op photos and full supporting records

When requesting coverage for gynecomastia surgery or prophylactic mastectomy, submit the specific clinical documentation listed in the policy: height, weight, age, and unclothed pre-operative photographs (chin to waist frontal and side) plus supporting medical records and test results.

    • Required items include: height (inches), weight (pounds), age; unclothed pre-operative photos from chin to waist, standing frontal and side views with arms down; history and physical, diagnoses, objective signs/symptoms with duration; prior management and current medications; endocrine study results, imaging/pathology confirming glandular tissue; treatment plan including planned reconstruction where applicable.
    Documentation Required

Follow plan administrative policies and provide documentation supporting medical necessity

Follow Health Plan administrative policies and supply documentation supporting medical necessity per the coverage criteria; when plan documents or state Medicaid provisions apply, those terms and limitations govern coverage determinations.

    • Providers must adhere to plan-level administrative procedures and coverage documents (e.g., evidence of coverage, policy, contract).
    • For Medicaid members, state Medicaid coverage provisions take precedence when they conflict with this clinical policy.
    Denial Risk

Avoid denials for cosmetic/non–medically necessary procedures

Requests for procedures performed solely for cosmetic purposes or otherwise listed as not covered will be denied. Ensure the proposed procedure meets medical necessity criteria and is not among the policy exclusions before submitting for authorization.

    • Examples of procedures not covered: breast implants for cosmetic enlargement; removal of implants for cosmetic purposes; augmentation mammaplasty for cosmetic purposes; correction of inverted nipples; preparation of moulage; periprosthetic capsulotomy/capsulectomy following augmentation; breast reduction or mastopexy when medical necessity criteria are not met.
    Note

Coverage decisions subject to member contract terms, exclusions and limitations

Coverage and benefit administration are subject to the terms, conditions, exclusions and limitations of the member's coverage documents; failure to meet those terms (or the policy criteria) may result in denial of coverage.

    • This clinical policy is a guide to medical necessity and does not guarantee payment.
    • Coverage determinations are governed by the member's evidence of coverage, certificate of coverage, policy or contract and applicable laws.
    Documentation Required

Submit complete, criteria-specific documentation to support authorization requests

Include all requested medical records and evidence that criteria are met when submitting authorization requests; incomplete documentation (missing images, objective findings, or criteria-specific tests) may delay or result in denial.

    • Provide documentation demonstrating duration of symptoms, imaging/pathology confirming glandular tissue (gynecomastia), genetic or biopsy results for prophylactic mastectomy indications, and BSA/resection estimates per the Schnur Sliding Scale for reduction requests.
ConfirmationExcessive tissue must be confirmed as glandular by clinical exam and ultrasound or mammogram

Definition (CPT 19380)Revision of reconstructed breast involving significant removal of tissue, re-advancement and/or re-inset of flaps in autologous reconstruction, or significant capsular revision combined with soft tissue excision in implant-based reconstruction
ScopeIntended to clarify major revisions rather than minor contour adjustments
