    Coverage Criteria

    Acute myocardial infarction evaluation

Covered when all of the following are met:

    Quantitative troponin criteria

    • Acute coronary syndrome presentation: The individual presents with signs and symptoms of acute coronary syndrome.

See Note 1 in the policy.

    • Appropriate evaluation setting: Testing is performed at an outpatient facility capable of performing an adequate clinical myocardial infarction evaluation.
    • Quantitative testing and frequency: Quantitative cardiac troponin T or I is used for diagnosis of myocardial infarction.Up to 4 measurements within the first 72 hours following initial presentation

Heart failure evaluation and severity assessment

BNP or NT-proBNP measurement meets criteria when any of the following indications applies:

    Covered indications

    • Heart failure diagnosis: The individual presents with dyspnea and testing is used to diagnose heart failure.
    • Chronic heart failure severity: Testing is used to establish disease severity in an individual with chronic heart failure in the outpatient setting.Up to 4 times per year

Biomarkers not meeting criteria for MI diagnosis or prognosis

The following testing does not meet criteria when performed for diagnosis or prognosis of myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome:

    Excluded biomarker testing

    • Excluded biomarker: C-reactive protein is requested for diagnosis or prognosis of myocardial infarction.
    • Excluded biomarker: Heart-type fatty acid binding protein (H-FABP) is requested for diagnosis or prognosis of myocardial infarction.
    • Excluded biomarker: Any other cardiac biomarker not listed in the policy is requested for diagnosis or prognosis of myocardial infarction.

Guideline recommendations for cardiac biomarker use

The following clinical guideline recommendations are informational and are not explicit payer coverage criteria:

    Guideline-supported biomarker uses

    • Suspected acute coronary syndrome: Measure cardiac troponin as soon as possible, preferably using a high-sensitivity assay.

If the initial result is nondiagnostic, perform serial measurement.

    • Suspected acute myocardial infarction: Interpret a rising and/or falling cardiac troponin pattern with at least one value above the 99th-percentile upper reference limit in the setting of clinical evidence of ischemia. At least one value above the 99th-percentile upper reference limit
    • Dyspnea with suspected heart failure: Measure BNP or NT-proBNP to support diagnosis or exclusion of heart failure when the cause of dyspnea is unclear and physical examination is equivocal.

    Myocardial infarction diagnostic criteria

MI diagnosis requires both the biomarker criterion and at least one additional clinical, ECG, imaging, or angiographic criterion:

    MI diagnostic requirements

    • Biomarker criterion: Detect a rise and/or fall of cardiac biomarker values, preferably cardiac troponin, with at least one value above the 99th percentile of the upper reference limit. At least one value above the 99th percentile of the upper reference limit
    • At least 1 additional criterion

      • Symptoms of ischemia.
      • New or presumed new significant ST-T changes or new left bundle branch block.
      • Development of pathological Q waves on ECG.

    Suspected acute coronary syndrome criteria

For suspected ACS, cardiac troponin is the preferred biomarker:

Suspected ACS biomarker assessment: Measure a biomarker of cardiomyocyte injury, preferably high-sensitivity cardiac troponin, in patients with suspected acute coronary syndrome.

Biomarkers other than cardiac troponin are not recommended for ACS diagnosis unless cardiac troponin is unavailable; CK-MB, myosin-binding protein C, or copeptin may have limited relevance in combination with standard cardiac troponin.

    Suspected heart failure criteria

BNP or NT-proBNP is recommended for assessment of suspected heart failure:

Suspected heart failure assessment: Measure BNP or NT-proBNP in patients with suspected signs and symptoms of heart failure.

Population-level screening for heart failure is not supported by current ESC guideline recommendations; targeted screening is described for selected high-risk populations.

AST/SGOT measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

CKMB measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Creatine kinase (CK) measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Creatine kinase isoenzyme measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Lactate dehydrogenase (LD/LDH) measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Myoglobin measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Cardiac biomarker testing for acute coronary syndrome does not meet criteria when performed in an outpatient setting that cannot perform an adequate clinical myocardial infarction evaluation, including an independent laboratory or physician’s office.

In the outpatient setting, qualitative cardiac troponin T or I measurement does not meet criteria.

Copeptin measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Troponin C measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

C-reactive protein measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Heart-type fatty acid binding protein (H-FABP) measurement does not meet criteria when used to diagnose or provide prognosis for myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

C-reactive protein measurement does not meet criteria for diagnosis or prognosis of myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

H-FABP measurement does not meet criteria for diagnosis or prognosis of myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Measurement of any other cardiac biomarker not listed in the policy does not meet criteria for diagnosis or prognosis of myocardial infarction in individuals presenting with signs and symptoms of acute coronary syndrome.

Serial BNP or NT-proBNP measurements should not be treated as established guidance for heart failure treatment because evidence remains insufficient to support treatment guidance based on serial values.

Newer biomarkers may provide prognostic information beyond natriuretic peptides, but they do not have established evidence of an incremental management benefit.

Copeptin is not encouraged for myocardial infarction assessment because troponins are generally superior.

For acute coronary syndrome diagnosis, biomarkers other than cardiac troponin are not recommended unless cardiac troponin is unavailable. CKMB, myosin-binding protein C, or copeptin may have limited relevance in combination with standard troponin in specific circumstances.

Current ESC guidance does not support population-level screening for heart failure. Targeted screening may be considered for selected high-risk populations.

The listed testing approaches and biomarkers do not meet criteria because published scientific literature is lacking to confirm that they are required and beneficial for diagnosis and treatment of the individual’s illness.

    Covered Clinical Indications

    Background

Cardiac biomarkers are biochemical markers released into the blood from injured myocardial tissue in acute and chronic conditions, including myocardial infarction and heart failure. Troponin and CK-MB are used in acute clinical settings, while natriuretic peptides support heart failure evaluation and prognosis. This policy identifies covered uses of quantitative troponin, BNP, and NT-proBNP and excludes specified biomarkers and testing approaches for the indications described.
