    Coverage Criteria

    Covered HPV Tumor-Status Testing

Coverage is subject to the individual's benefit coverage at the time of the request. Covered when ALL of the following are met:

    ALL of the following

    • The individual has oropharyngeal squamous cell carcinoma or metastatic squamous cell carcinoma of unknown primary origin in a cervical lymph node.
    • Testing is performed to establish HPV tumor status.
    • Testing is for high-risk HPV using mRNA expression testing for HPV E6/E7.

    Noncovered Screening and Diagnostic Tests

The following screening and diagnostic testing does not meet criteria for coverage:

    ALL of the following

    • The testing is intended to screen for, detect, or diagnose oral cancer.
    • The testing is not high-risk HPV mRNA expression testing for HPV E6/E7 used to establish tumor status in an applicable individual.

Noninvasive tests designed to screen for, detect, or diagnose oral cancer, including the OmniPathology Oropharyngeal HPV PCR Test, DO NOT MEET CRITERIA.

To screen for, detect, or diagnose oral cancer, salivary biomarker testing, including testing for peptides, proteins, nucleic acids, or metabolites; genotyping of HPV, such as OraRisk® HPV; gene-expression profiling; panels that incorporate genetic risk factors with nongenetic biomarkers; and detection of HPV from an oropharyngeal swab DO NOT MEET CRITERIA.

The noncovered noninvasive and molecular oral cancer screening and diagnostic tests listed above do not meet criteria because there is a lack of available published scientific literature confirming that the tests are required and beneficial for the diagnosis and treatment of an individual's illness.

    Documentation Required

    Clinical Background

Oral cancer is malignancy arising in the oral cavity, including the area between the vermilion border of the lips, the junction of the hard and soft palates, and the posterior one-third of the tongue. Oral squamous cell carcinoma is the predominant type of oral cancer. HPV is associated with oropharyngeal and head and neck squamous cell carcinoma; however, the CDC states that no approved HPV test is available to find HPV in the mouth or throat.

    Covered Clinical Indications

    Establish HPV Tumor Status

High-risk HPV mRNA expression testing meets criteria to establish tumor status when the applicable disease indication is present.

    Required indication and testing method

    • Applicable indication

      • The individual has oropharyngeal squamous cell carcinoma.
      • The individual has metastatic squamous cell carcinoma of unknown primary origin in a cervical lymph node.
    • Testing uses high-risk HPV mRNA expression testing for HPV E6/E7 to establish HPV tumor status.High-risk HPV mRNA expression testing for HPV E6/E7

Newly Diagnosed Oropharyngeal Squamous Cell Carcinoma

    Eligibility Requirements

For equivocal p16 immunostaining, defined as 50%-70% staining or extensive but weak staining, CAP recommends HPV-specific testing. HPV-specific testing is also recommended when p16 staining is discordant with morphology, for large multisite tumors overlapping the oropharynx, for non-tonsillar and non-base-of-tongue oropharyngeal sites, or when required by a clinical trial.

For squamous cell carcinoma of unknown primary, p16 and EBER testing are recommended; when p16 is positive, another specific HPV test should be performed to confirm HPV status.

    Noncovered Services

The OmniPathology Oropharyngeal HPV PCR Test is not covered when used as a noninvasive screen for oral or oropharyngeal cancer. The following approaches also do not meet criteria for oral cancer screening, detection, or diagnosis: salivary biomarker testing, including peptides, proteins, nucleic acids, or metabolites; HPV genotyping, including OraRisk HPV; gene-expression profiling; panels incorporating genetic risk factors with nongenetic biomarkers; and detection of HPV from an oropharyngeal swab.
