    Coverage & Eligibility Criteria

    Initial coverage criteria

Covered when ALL of the following are met:

Primary coverage criteria: 1. Member has a stage 3 or stage 4 pressure ulcer; 2. Member is bedridden or chair bound due to severely limited mobility; 3. In the absence of an air-fluidized bed, the member would require institutionalization; 4. The air-fluidized bed is ordered in writing by the treating practitioner based upon a comprehensive assessment generally performed within one month prior to initiation; 5. Completion of at least one month of conservative treatment without progression toward wound healing (conservative treatment elements specified); 6. A trained adult caregiver is available to assist with required care; 7. The treating practitioner directs home treatment and reevaluates and recertifies the need monthly; 8. All alternative equipment has been considered and ruled out.

Conservative treatment elements include frequent repositioning, use of a Group 2 support surface, infection treatment, nutrition optimization, debridement (including wet-to-dry when used for debridement), maintenance of a moist wound bed with appropriate occlusive dressing when required, education, weekly assessment, and moisture/incontinence management.

    Continuation criteria

    Continued coverage requires:

Monthly recertification: Treating practitioner must document monthly that continued use is medically necessary for wound management until the ulcer is healed or document that other aspects of the care plan are being modified to promote healing.monthly

Supplier must retain the treating practitioner's monthly statement on file; KX modifier may be used only when criteria are met and monthly certification indicates continued need.

    Practitioner Statement Criteria

Covered when the treating practitioner documents relevant clinical conditions and medical necessity consistent with member contract and CMS/LCD/NCD guidance.

Practitioner-documented conditions: At least one of the following is indicated on the Statement of Treating Practitioner or equivalent documentation: completely immobile; limited mobility; any pressure ulcer on the trunk or pelvis; impaired nutritional status; fecal or urinary incontinence; altered sensory perception; compromised circulatory status.1 of 7

If none of the listed conditions apply, a separate statement documenting medical necessity must be attached. The practitioner must provide an estimated length of need and sign the form.

Payment for an air‑fluidized bed does not include compensation for a caregiver’s wages nor does it cover architectural modifications to the member’s home. Examples of excluded architectural adjustments include electrical or structural improvements required to accommodate the equipment (e.g., reinforcement of flooring or upgrades to the electrical system). These exclusions are separate from the clinical coverage criteria and remain the financial responsibility of the member or their contractor.

Equipment that is used primarily or customarily for a non‑medical purpose is not considered durable medical equipment (DME). Final determinations of whether a specific product is a covered DME item are made in accordance with the member’s contract and applicable plan terms; the policy’s medical necessity finding alone does not guarantee coverage if the contract excludes the item.

An air‑fluidized bed is considered not reasonable and necessary when contraindications are present. Specifically, coverage is denied when the member has coexisting pulmonary disease that impairs effective coughing or when the member requires unprotected wet soaks or moist dressings (i.e., dressings that are not covered with an occlusive barrier). Additional contraindications that preclude coverage include inability or unwillingness of the caregiver to provide required care, inadequate structural support for the bed, insufficient electrical capacity, or other known contraindications listed by the policy.

A determination that an air‑fluidized bed is medically reasonable and necessary does not by itself create coverage. Coverage remains subject to the member’s contract terms, limitations, and exclusions. If contract language excludes or limits a DME item, the contract provisions take precedence over the policy’s clinical criteria.

    Provider Requirements & Prior Authorization

    Prior Authorization

    Prior authorization required for E0194

Air-fluidized bed (HCPCS E0194) requires prior authorization and documentation that all policy coverage criteria are met, including: presence of a stage 3 or 4 pressure ulcer; bedridden or chair‑bound status due to severely limited mobility; comprehensive treating practitioner assessment generally within one month prior to initiation; completion of at least one month of conservative treatment without progression toward wound healing; availability of a trained adult caregiver; monthly practitioner recertification; and that alternatives have been considered and ruled out.

    • Code: E0194
    • All initial coverage criteria must be documented prior to authorization
    Note

Submit treating practitioner statement and clinical documentation

Submit the treating practitioner’s Statement of Treating Practitioner and supporting clinical documentation to demonstrate medical necessity; coverage determinations are made case‑by‑case and require the practitioner’s clinical responses and other documentation when requesting prior authorization.

    Medical Necessity Definitions & Criteria

Air-fluidized bed medical necessity — ALL of the following

Medical necessity for an air-fluidized bed is met when ALL of the following are satisfied:

DME-specific necessity: Stage 3 or Stage 4 pressure ulcer; member is bedridden or chair bound; without an air-fluidized bed the member would require institutionalization; the bed is ordered in writing following a comprehensive assessment generally within one month prior to initiation; completion of at least one month of conservative treatment without progression toward healing including specified elements; a trained adult caregiver is available; treating practitioner reevaluates and recertifies need monthly; all alternative equipment has been considered and ruled out.

DME medical necessity supported by practitioner-documented risk factors or pressure injury presence; staging informs severity

DME medical necessity is supported by practitioner-documented risk factors or presence of a pressure injury; staging definitions inform severity.

Clinical indicators: Presence of a Stage 4, Unstageable, or Deep Tissue Pressure Injury (DTPI) as defined by NPIAP/NPUAP OR at least one high-risk condition documented on the Statement of Treating Practitioner (completely immobile, limited mobility, any pressure ulcer on trunk/pelvis, impaired nutritional status, fecal or urinary incontinence, altered sensory perception, compromised circulatory status).

    Definitions & Clinical Terms

    Air-fluidized bed definition
DefinitionA support surface consisting of a bed filled with ceramic beads over which warm pressurized air is circulated to simulate fluid movement, distributing weight and creating a floating sensation
ComponentsUp to ~2,000 lbs of ceramic spherules covered by a porous polyester sheet allowing moisture and air passage
Therapeutic effectsReduces pressure and moisture at skin surface to help treat/prevent pressure ulcers and manage extensive burns
Stage 3 and Stage 4 pressure injuries (NPIAP definitions)
Stage 3 (NPIAP)Full-thickness skin loss in which adipose is visible; granulation tissue and epibole often present; slough/eschar may be visible; fascia/muscle/tendon/ligament/cartilage/bone are NOT exposed; undermining and tunneling may occur. If slough/eschar obscures extent, classify as Unstageable.
    Stage 4 (NPIAP)

    Background & Clinical Context

An air‑fluidized bed employs warm, pressurized air circulated through small ceramic beads to create a fluidized medium that supports and redistributes the patient’s weight. This mechanism produces a ‘floating’ effect and allows air and moisture to pass through a porous interface sheet, reducing pressure and moisture exposure to the skin. These characteristics make an air‑fluidized bed a Group 3 pressure‑reducing support surface intended for patients with severe pressure injuries (e.g., Stage 3 or 4) or other high‑risk conditions when conservative measures have failed.

    Exclusions — Not Covered

Items and services that are not covered in association with provision of an air‑fluidized bed include: payment for the caregiver’s services; costs for architectural adjustments such as structural reinforcement or electrical upgrades; and provision of the bed when policy contraindications exist (for example, significant pulmonary disease or need for unprotected moist dressings). In addition, devices primarily used for non‑medical purposes are not considered DME and final benefit determinations are governed by the member’s contract and plan exclusions.

    • Final benefit determinations follow the member’s contract language and applicable state/CMS requirements
    • Medical policy guidelines do not override contract exclusions or limits
    1 of listed conditions

Estimated length of need should be documented; if none of the listed conditions apply, additional justification is required.

On the provided Statement of Treating Practitioner form the treating practitioner must print their name, sign and date the form, provide UPIN, estimate length of need, and circle applicable clinical condition responses as indicated on the form; the form must not be completed by the DME provider.

    • Required fields on form: practitioner name (printed), practitioner signature, practitioner UPIN, date, estimated length of need, and circled clinical conditions.
    • If none of the listed conditions apply, attach separate documentation explaining medical necessity.
Full-thickness skin and tissue loss with exposed or directly palpable fascia, muscle, tendon, ligament, cartilage or bone; slough/eschar may be visible; epibole, undermining and/or tunneling often occur; depth varies by location.
SourceStaging per National Pressure Injury Advisory Panel (2016 Revision)
    Stage 4 Pressure Injury definition
Stage 4 Pressure InjuryFull-thickness skin and tissue loss with exposed or directly palpable fascia, muscle, tendon, ligament, cartilage or bone in the ulcer; slough and/or eschar may be visible; often includes epibole (rolled edges), undermining and/or tunneling. If slough or eschar obscures extent, classify as Unstageable.
Depth variabilityDepth varies by anatomical location; areas of significant adiposity can develop deep wounds
Note on obscured woundsIf slough or eschar obscures extent of tissue loss, the injury may be Unstageable until debrided
    Unstageable Pressure Injury definition
Unstageable Pressure InjuryFull-thickness skin and tissue loss in which extent of tissue damage cannot be confirmed because it is obscured by slough or eschar; removal would reveal a Stage 3 or Stage 4 injury
Heel/eschar cautionStable eschar on the heel or ischemic limb (dry, adherent, intact without erythema or fluctuance) should not be softened or removed
ImplicationCannot be staged until slough/eschar removal reveals true depth
Deep Tissue Pressure Injury (DTPI) definition
DTPI definitionIntact or non-intact skin with localized area of persistent non-blanchable deep red, maroon, or purple discoloration or epidermal separation revealing a dark wound bed or blood-filled blister; may precede pain and temperature changes and can evolve rapidly
Clinical cautionDo not use DTPI to describe vascular, traumatic, neuropathic, or dermatologic conditions
EvolutionMay progress to reveal extent of tissue injury or resolve without tissue loss; if necrotic tissue or deeper structures are visible, classify as Unstageable, Stage 3, or Stage 4
