    Coverage Criteria

    Medical Necessity for Definitive Testing

Definitive drug testing is medically necessary when it is individualized to substances specific to the member’s treatment plan, accurate and reliable results are necessary to integrate treatment decisions and clinical assessment, and all required documentation and clinical-use criteria are met.

    ALL of the following

    • Testing is individualized to test only for substances specific to the member’s plan of treatment.
    • Clinical documentation specifies how the test results will be used to guide clinical decision-making.
    • The medical record includes the member’s history, physical examination, and previous laboratory findings.
    • The medical record includes a signed and dated order, or an intent to order documented in the medical record.
    • The current treatment plan is documented.
    • The prescribed medication(s) are documented.
    • A risk assessment is documented.
    • Accurate and reliable results are necessary to integrate treatment decisions and clinical assessment.
    • The frequency and rationale for definitive urine drug testing are documented in the member’s medical record.

Frequency depends on the member’s specific substance use history. Providers may reference 019 Definitive Drug Testing Yearly/Same-Day Limit and 031 Presumptive Drug Testing Yearly Limits for further frequency guidance.

    Outpatient Definitive Testing Criteria

Outpatient definitive testing is covered when the requested testing is limited to the specific drugs or drug classes warranted by the member’s medical record and unpredicted initial-testing results.

    ALL of the following

    • The request includes only the specific drug(s) or number of drug classes for which initial testing resulted in unpredicted results.
    • The target drug or drug classes are pertinent to the member’s care based on the medical record.
    • The target drug classes are documented on the test order and in the medical record.
    • When definitive testing for substances of abuse is required based on the member-specific history, the applicable test is selected according to the number of pertinent drug classes: HCPCS G0480 for 1–7 drug classes or HCPCS G0481 for 8–14 drug classes.
    • Testing is limited to clinically relevant substances. The ASAM substance-of-abuse classes include amphetamines, opiates, phencyclidine, barbiturates, benzodiazepines, marijuana, cocaine, methadone, and propoxyphene.

Inpatient, Emergency Room, and Observation Criteria

Inpatient, emergency room, and observation settings may support broader definitive testing when the medical record documents medical necessity.

    ALL of the following

    • The member is receiving testing in an inpatient, emergency room, or observation setting.
    • Documentation supports the medical necessity of the requested definitive testing, including the member-specific clinical circumstances and how results will guide treatment or clinical decision-making.
    • The requested drug(s) or drug classes are relevant to the member’s history, treatment plan, risk assessment, or clinical presentation.
    • For testing of 15–21 drug classes, HCPCS G0482 may be used when documentation supports medical necessity.
    • For testing of 22 or more drug classes, HCPCS G0483 may be used when documentation supports medical necessity.

Outpatient definitive testing involving more than 14 drugs or drug classes is excluded from coverage under this policy. Definitive tests G0482 (15–21 drug classes) and G0483 (22 or more drug classes) are not indicated for outpatient testing.

For outpatient definitive testing, the request may include only the specific drug(s) or number of drug classes for which initial testing produced unpredicted results. Drug classes unrelated to those unpredicted initial-testing results are not covered.

Outpatient definitive testing of more than 14 drugs or drug classes is not medically necessary.

  4. Documentation Required

    Document Testing Rationale and Frequency

Individualize definitive testing to the member’s substance use history and treatment plan. Document the rationale and frequency in the medical record, including how accurate and reliable results will be used to guide clinical decision-making and assessment.

    • Test only substances specific to the member’s plan of treatment
    • Document the member-specific rationale and frequency
    • Explain how results will integrate treatment decisions and clinical assessment
    Documentation Required

    Identify Pertinent Target Drug Classes

When ordering G0480 or G0481 for substances of abuse, identify the pertinent target drug classes based on the member’s history and medical record. Document those target classes both on the test order and in the medical record.

    • G0480: 1–7 drug classes
    • G0481: 8–14 drug classes
    • Relevant classes may include amphetamines, opiates, phencyclidine, barbiturates, benzodiazepines, marijuana, cocaine, methadone, and propoxyphene
    Documentation Required

Support the Claim with Clinical Documentation

Ensure the ordering or referring clinician’s detailed clinical documentation supports and justifies every service submitted on the claim. The record should connect the clinical need to medical necessity and explain the definitive test results or services provided.

    • Justify all laboratory tests submitted for reimbursement
    • Connect clinical findings and treatment needs to the billed services
    • Explain the definitive test results or services
    Denial Risk

More Than 14 Drugs or Drug Classes

Outpatient definitive testing of more than 14 drugs or drug classes is not medically necessary. • Include only specific drugs or drug classes associated with unpredicted initial results
    • Base the requested scope on the member-specific history and the initial test findings
    • Broader testing is not indicated when it exceeds the supported unpredicted results
    Denial Risk

    Insufficient Clinical Justification

Claims may be denied when the ordering or referring clinician’s documentation does not justify the services submitted. Before billing, verify that the record contains sufficient clinical rationale and medical necessity support for the requested testing.

    • Confirm ordering/referring clinician documentation supports each submitted service
    • Ensure the medical record supports the requested test scope, frequency, and clinical use
Presumptive testing provides preliminary evidence rather than definitive identification
    ASAM substance-of-abuse classes
    Classes 1-3Amphetamines; opiates; phencyclidine
    Classes 4-6Barbiturates; benzodiazepines; marijuana
