OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

  1. Policy states these CPTs “WILL ONLY BE COVERED AS A PREVENTIVE SERVICE FOR ANY APPROPRIATE PREGNANCY DIAGNOSIS CODE OR INCREASED RISK DIAGNOSIS CODE.”
  2. Documentation Required

Required clinical documentation for preventive screening

Document the patient’s age, sexual activity status, and presence or absence of any listed increased‑risk factors (new sex partner; >1 partner; partner with concurrent partners or an STI; inconsistent condom use; history/current STI; exchanging sex for money/drugs) or an appropriate pregnancy/increased‑risk diagnosis code when billing for chlamydia screening as a preventive service.

    • Record patient age (policy covers sexually active females ≤24 years routinely; older females require documented increased risk)
    • Document sexual activity status and any enumerated risk factors or the pregnancy/increased‑risk diagnosis code used for billing
    Billing Rule

Coding linkage required for preventive coverage

Ensure billing for chlamydia screening uses the CPT codes listed in policy and is reported with an appropriate pregnancy or increased‑risk diagnosis code; failure to report the required diagnosis linkage may result in denial of preventive coverage.

    • Preventive coverage limited to CPTs 86631, 86632, 87110, 87270, 87320, 87490, 87491, 87810 (and 99000 specimen handling) when billed with an appropriate pregnancy or increased‑risk diagnosis code
    • Examples of required diagnosis codes include pregnancy and the listed Z‑codes in policy (e.g., Z11.3, Z20.2, Z72.51)
Individuals who exchange sex for money or drug
