    POLICY UPDATE CHANGES

Policy criteria are not applicable to Exchange Plans for dates of service on or after January 1, 2026.

Effective September 15, 2026, coverage criterion 1 was revised to cover serum lipase for individuals presenting with signs and symptoms of acute pancreatitis, limited to no more than one test per day in the outpatient setting.

The amylase criterion was revised to address analysis of pancreatic cyst fluid rather than serum amylase.

Measurement of serum amylase for purposes other than analysis of pancreatic cyst fluid was designated as not meeting coverage criteria.

CPT code 86141 was added to the applicable state and federal regulations section.

The policy background, guidelines and recommendations, and evidence-based scientific references were updated following literature review.

The policy content was transferred to a ConnectiCare-branded template without changes to the policy title or policy number.

Serum Lipase Outside the Covered Indication

The testing does not meet coverage criteria when performed for situations or conditions other than the covered indication.

Measurement of serum lipase for any situation or condition not described as a covered indication above.

    Diagnostic Criteria for Acute Pancreatitis

The diagnosis of acute pancreatitis requires at least 2 of the following 3 criteria.

    At least 2 of 3

    • Abdominal pain not attributable to another cause or suggestive of acute pancreatitis.
    • Serum amylase or serum lipase at least 3 times the upper limit of normal.>= 3x the upper limit of normal
    • Imaging findings consistent with acute pancreatitis.

    Initial Pancreatic Enzyme Testing

For initial evaluation of suspected acute pancreatitis:

    ALL of the following

    • Use serum lipase in preference to total or pancreatic amylase for initial diagnosis.
    • Do not order both amylase and lipase routinely; simultaneous testing is generally discouraged because it increases costs while only marginally improving diagnostic efficiency compared with lipase alone.

Amylase may be considered when serum lipase is unavailable as a stat test and the patient has sudden-onset abdominal pain with nausea and vomiting, fever, hypotension, and abdominal distension.

    Repeat Testing and Monitoring

Repeat testing should be based on a new clinical diagnostic concern, not routine monitoring.

Repeat testing may be considered when ANY of the following applies

    • Signs or symptoms suggest persistent pancreatic or peripancreatic inflammation.
    • There is concern for blockage of the pancreatic duct.
    • There is concern for development of a pseudocyst.
    • There is clinical suspicion of acute-on-chronic pancreatitis.
Do not routinely repeat lipase to monitor a patient with an established diagnosis of acute or chronic pancreatitis.

In outpatient or community settings, laboratories may consider a 6-month hard-stop minimum retesting interval to reduce unnecessary retesting.

    Revised Covered Criterion: Serum Lipase

    The revised covered criterion is:

For individuals presenting with signs and symptoms of acute pancreatitis, measurement of serum lipase. No more than 1 test per day in the outpatient setting.

Revised Covered Criterion: Pancreatic Cyst Fluid Amylase

    The revised covered criterion is:

Measurement of amylase when ordered for analysis of pancreatic cyst fluid.

COVERAGE CRITERIA: When ordered for anything other than analysis of pancreatic cyst fluid, measurement of amylase does not meet coverage criteria. This includes serum and urine amylase testing.

COVERAGE CRITERIA: For the diagnosis, assessment, prognosis, and/or determination of severity of acute pancreatitis, measurement of serum or urine trypsin, trypsinogen, or trypsinogen activation peptide (TAP) does not meet coverage criteria.

COVERAGE CRITERIA: For the diagnosis, assessment, prognosis, and/or determination of severity of acute pancreatitis, measurement of C-reactive protein (CRP), interleukin-6 (IL-6), interleukin-8 (IL-8), or procalcitonin does not meet coverage criteria.

COVERAGE CRITERIA: Policy criteria are not applicable to Exchange Plans for dates of service on or after January 1, 2026.

COVERAGE CRITERIA: Routine repeat lipase testing is not recommended for monitoring patients with an established diagnosis of acute or chronic pancreatitis. Repeat testing should be considered only when there is a new diagnostic concern, including persistent pancreatic or peripancreatic inflammation, pancreatic duct obstruction, pseudocyst development, or suspected acute-on-chronic pancreatitis.

COVERAGE CRITERIA: Simultaneous amylase and lipase testing is generally discouraged because it increases costs while only marginally improving diagnostic efficiency compared with lipase alone. Serum lipase is preferred for the initial diagnosis of acute pancreatitis.

COVERAGE CRITERIA: Policy criteria are not applicable to Exchange Plans for dates of service on or after January 1, 2026.

COVERAGE CRITERIA: Serum amylase does not meet coverage criteria except when the amylase is measured from pancreatic cyst fluid. Other amylase testing, including urine amylase, is not covered.

COVERAGE CRITERIA: Policy criteria are not applicable to Exchange Plans for dates of service on or after January 1, 2026.

    Denial Risk

    Lipase Coverage Restriction

Serum lipase is covered only when ordered for individuals presenting with signs and symptoms of acute pancreatitis. Measurement of serum lipase for all other situations or conditions does not meet coverage criteria.

    • Do not bill serum lipase as covered when the patient is not being evaluated for suspected acute pancreatitis.
    Denial Risk

    Amylase Coverage Restriction

Amylase testing does not meet coverage criteria when ordered for anything other than analysis of pancreatic cyst fluid. This restriction applies to serum amylase and all other amylase testing, including urinary amylase.

    • Covered indication: analysis of pancreatic cyst fluid only.
    • Serum, urinary, and other amylase testing outside pancreatic cyst fluid analysis is not covered.

    Clinical Background

Acute pancreatitis is inflammation of pancreatic tissue that may range from mild disease to severe disease with persistent organ failure. Under the revised Atlanta classification, diagnosis is supported when at least 2 of 3 features are present: characteristic abdominal pain, serum amylase or lipase at least 3 times the upper limit of normal (ULN), or characteristic imaging findings. For symptomatic outpatient evaluation under this policy, serum lipase is the covered pancreatic enzyme test, while several other enzyme and inflammatory biomarker tests are not covered for acute pancreatitis assessment.

Diagnostic roleCounts as the biochemical criterion in the revised Atlanta framework

Testing may be used in the evaluation of suspected acute pancreatitis when characteristic pain, biochemical evidence, or imaging findings are being assessed.

Evaluation of suspected acute pancreatitis: Evaluate suspected acute pancreatitis using serum amylase or serum lipase when characteristic abdominal pain, biochemical evidence of pancreatitis, or compatible imaging findings are being assessed.Amylase or lipase >3× ULN when biochemical evidence is present

Evaluation of Suspected Exocrine Pancreatic Insufficiency

Use fecal elastase as the initial test when exocrine pancreatic insufficiency is suspected in a high-risk patient.

High-risk clinical condition: The individual has a high-risk condition, such as chronic pancreatitis, relapsing acute pancreatitis, pancreatic ductal adenocarcinoma, cystic fibrosis, or previous pancreatic surgery.
Initial test: Fecal elastase is used as the initial test for suspected exocrine pancreatic insufficiency.

A fecal elastase level below 100 μg/g of stool provides good evidence of exocrine pancreatic insufficiency; 100–200 μg/g is indeterminate.

Revised Atlanta Evaluation of Suspected Acute Pancreatitis

Evaluate suspected acute pancreatitis under the revised Atlanta diagnostic framework.

At least 2 of 3 criteria

    • Characteristic abdominal pain: Abdominal pain is not attributable to another cause or is suggestive of acute pancreatitis.
    • Enzyme elevation: Serum lipase or, when appropriate, amylase is at least three times the upper limit of normal.≥3× ULN
    • Compatible imaging: Imaging is consistent with pancreatitis.

Repeat Testing for Persistent or Recurrent Disease

Repeat testing may be considered for specific clinical concerns, but not for routine monitoring.

Indications for repeat testing: There are signs or symptoms of persistent pancreatic or peripancreatic inflammation, pancreatic duct obstruction, pseudocyst development, or suspected acute-on-chronic pancreatitis.

Serum Lipase Testing for Acute Pancreatitis Symptoms

Covered for individuals presenting with signs and symptoms of acute pancreatitis, limited to one outpatient test per day.

Covered serum lipase evaluation: Evaluate an individual presenting with signs and symptoms of acute pancreatitis using serum lipase testing. No more than one test per day in the outpatient setting

    Pancreatic Cyst Fluid Amylase Analysis

Covered for analysis of pancreatic cyst fluid.

Amylase measurement of pancreatic cyst fluid: Analyze pancreatic cyst fluid using amylase measurement.

The revised wording removes the serum limitation because the criterion concerns pancreatic cyst fluid.

    Serum lipase
Maximum frequencyNo more than one test per day
Test and settingSerum lipase in the outpatient setting
Coverage contextApplies to individuals presenting with signs and symptoms of acute pancreatitis
    11/2024Policy clarification and criteria update

Updated the policy effective March 15, 2025 to clarify that serum lipase or amylase testing is not allowed for individuals already diagnosed with acute or chronic pancreatitis, added Coverage Criterion 6, and revised the signs and symptoms in Note 1.
