    Telehealth Coverage Criteria

    General Telehealth Coverage

Telehealth may be considered medically necessary when all applicable requirements are met.

Medical necessity and professional relationship: The service is a medically necessary diagnostic or therapeutic service; a professional relationship exists or can reasonably be established between the distant-site healthcare professional and the patient in accordance with applicable state laws and regulations.
Coverage and standard of care: The service is a covered service, follows evidence-based standards of care, and is delivered consistently with the applicable Idaho community standard of care and the provider's scope of practice.
Eligible and qualified provider: The provider is recognized as eligible by Blue Cross of Idaho, remains within scope of practice, and the distant-site practitioner has appropriate credentials and licensing.
Safe telehealth delivery: The provider determines that delivery through telehealth is consistent with the standard of care and has access to medical and emergency resources near the patient's location for appropriate referrals when medically indicated.

    Treatment and Prescribing

When treatment or prescribing is provided through telehealth, all of the following requirements apply.

Evaluation before treatment: Before providing treatment, including a prescription drug order, the provider obtains and documents relevant clinical history and current symptoms sufficient to establish the diagnosis and identify underlying conditions and contraindications.
Provider-patient relationship: If no established provider-patient relationship exists, the provider takes appropriate steps to establish one using two-way audio and visual interaction when required by applicable law or the Idaho Virtual Care Access Act.
Prescribing authority: The provider prescribes within the scope of the provider's license and applicable laws, rules, and regulations. Controlled substances may be prescribed by telemedicine only in compliance with applicable federal requirements.

    Hosted Telehealth Visits

Hosted telehealth visits require all of the following conditions.

Originating-site control and licensure: The examination is under the control of the physician or practitioner at the originating site, and that physician or practitioner is licensed to furnish the services.
Confidential location: The hosted visit occurs in a space within the qualified patient location that permits confidential exchange.
Emergency availability: A medical practitioner is present in the office suite and available to respond to emergencies during the hosted telehealth service.

Telehealth is not medically necessary for emergency situations in which the patient’s life or health is in danger or imminent danger.

Partial Hospital Program services are not covered when furnished in a telehealth place of service.

Telehealth services included in a per diem rate cannot be billed separately.

Partial Hospital Program services are not covered when furnished in a telehealth place of service.

The following telehealth activities are not medically necessary: emergency situations where the patient’s life or health is in danger or imminent danger; generic information not specific to an individual; and reporting test results only. Administrative services, non-HIPAA-compliant email, patient-generated medical history, text messaging, and facsimile transmissions are also excluded from telehealth medical necessity.

    Provider Requirements and Billing Rules

    Prior Authorization

    Prior Authorization Applies to Telehealth

Blue Cross of Idaho’s published prior authorization protocols apply to services delivered both in person and through telehealth. Obtain authorization when required before providing the service.

    Documentation Required

    Document Clinical History and Symptoms

Before providing treatment, including a prescription drug order, obtain and document the member’s relevant clinical history and current symptoms to establish the diagnosis and identify underlying conditions and contraindications.

    Documentation Required

    Maintain Telehealth Medical Records

Generate and maintain medical records for every member receiving telehealth services in compliance with applicable state and federal laws, including HIPAA. Records must be accessible to other providers as permitted by law and meet PAP 107 Medical Records Standards.

    Telehealth Overview

Telehealth may deliver diagnostic and therapeutic care through synchronous real-time interaction or asynchronous store-and-forward communication. Synchronous interaction allows the provider and patient in separate locations to communicate simultaneously through interactive audio and video or audio technology. Asynchronous store-and-forward care involves medical information being transmitted for later review by a physician or practitioner at the distant site, so the patient and clinician do not need to be present at the same time and place. The appropriateness of either modality depends on the existence or establishment of a professional relationship, the applicable standard of care, and the provider’s ability to evaluate and treat the patient safely.

    Telehealth Definitions

Failure to meet the standard of care may result in adverse licensure action, even without an adverse outcome, and may create malpractice liability.
