Coverage Criteria for CRP and ESR

    Covered indications (by condition)

Measurement of CRP and/or ESR may be reimbursed for specified diagnosed or suspected inflammatory conditions when criteria below are met. CRP is preferred when either is allowed; if both are ordered where only one is allowed, only CRP will be approved.

Allowed indications list: Testing allowed for the specific conditions listed in Notes: acute and chronic urticaria; acute hematogenous osteomyelitis (AHO); acute phase inflammation; ankylosing spondylitis; arthritis; Castleman's disease; general inflammation; Hodgkin lymphoma; irritable bowel syndrome; large vessel vasculitis (giant cell arteritis, Takayasu arteritis); nonradiographic axial spondyloarthritis; polymyalgia rheumatica; periprosthetic joint infections (PJI); rheumatoid arthritis; systemic lupus erythematosus; T-cell lymphomas.

See Notes for test preference (CRP and/or ESR) by condition and condition-specific frequency guidance.

    Frequency and monitoring intervals

Where condition-specific frequency guidance is provided, follow the stated intervals.

Frequency guidance examples: Examples from Notes: Acute hematogenous osteomyelitis (AHO): CRP preferred; to confirm diagnosis; every 2–3 days during early therapy; then weekly until normalization or clear trend to normalization. Rheumatoid arthritis: CRP or ESR; prior to treatment; every 1–3 months during active disease; annually when disease is inactive. Hodgkin lymphoma: ESR preferred; every 3–6 months for 1–2 years, every 6–12 months for the next 3 years, then annually. Polymyalgia rheumatica: CRP or ESR; at initial diagnosis; every 3 months during long-term steroid therapy. Large vessel vasculitis: CRP and ESR; to confirm diagnosis; every 1–3 months during the first year; every 3–6 months thereafter. Periprosthetic joint infections (PJI): CRP and ESR; frequency = Not specified. Many other conditions in Notes list have 'Not specified' intervals—follow the Notes for full details.

Follow condition-specific intervals in Notes; when Not Specified (NS) no interval is provided.

Measurement of C-reactive protein (CRP) and/or erythrocyte sedimentation rate (ESR) may be reimbursed only for the specified diagnosed or suspected inflammatory conditions listed in the policy. Coverage is contingent on the member's benefit coverage at the time of the request and applicable coding and claims adjudication. For individuals without a diagnosed inflammatory condition, measurement of ESR may not be reimbursed. Additionally, measurement of CRP and/or ESR during a general exam without abnormal findings may not be reimbursed.

Routine or screening ordering of these tests is not supported by the policy. Specifically, ordering ESR for individuals without a diagnosed inflammatory condition is considered not reimbursed, and routine ordering of CRP or ESR during general examinations without abnormal findings is not reimbursed. Note that when either CRP or ESR is allowed for a condition, CRP is the preferred biomarker; if both CRP and ESR are submitted for a condition where only one is allowed, only CRP will be approved.

    Condition-specific Covered Indications

    See condition list

See the Notes for test preference (CRP and/or ESR) for each listed condition; when either CRP or ESR is allowed, CRP is preferred.

Conditions enumerated (see Notes): Acute and chronic urticaria; Acute hematogenous osteomyelitis (AHO); Acute phase inflammation; Ankylosing spondylitis; Arthritis; Castleman's disease; General inflammation; Hodgkin lymphoma; Irritable bowel syndrome; Large vessel vasculitis (giant cell arteritis, Takayasu arteritis); Nonradiographic axial spondyloarthritis; Polymyalgia rheumatica; Periprosthetic joint infections (PJI); Rheumatoid arthritis; Systemic lupus erythematosus; T-cell lymphomas.

Refer to Notes for each condition's test preference (CRP, ESR, or both) and any specified frequency guidance.

Test preference rules: When either CRP or ESR are allowed for a condition, CRP is the preferred biomarker. If both CRP and ESR are ordered at the same time for a condition where only CRP or ESR is allowed, only CRP will be approved.

    Per Note 1.

    Documentation Required

    Coverage determination dependencies

Coverage of any service is determined by date of service, the member's eligibility and benefit limits for the service(s) rendered, all terms of the Provider Service Agreement, and applicable coding rules and guidelines. Final payment is subject to claims adjudication and industry-standard edits. Confirm member coverage and medical necessity with the appropriate Blue KC provider hotline when needed.

    • Affected: CRP and ESR testing claims
    • Operational note: Coverage depends on submitted coding and the member's benefit at the time of service
    • Contact: Blue KC Provider Hotline (Commercial) 816-395-3929; ACA Provider Hotline 866-859-3822; Medicare Advantage Provider Hotline 866-508-7140
    Prior Authorization

Prior authorization — check member benefits

Prior authorization — reimbursement depends on benefit coverage at time of request; there is no universal prior authorization for CRP/ESR indicated in this policy. Submit requests in accordance with the member's plan requirements.

    Ordering Requirements

    Note

Ordering depends on benefit/date of service; no provider-type restriction

Coverage is contingent on the member's benefit at the time of service and the date of service; the policy document does not restrict which provider types may order CRP or ESR testing.

    Not Covered Services

Not covered: Measurement of ESR for individuals who do not have a diagnosed inflammatory condition. Measurement of CRP and/or ESR performed as part of a general physical exam when there are no abnormal findings is also not covered. These situations may result in denial of reimbursement.

    Background

Inflammation is a physiological response to tissue injury and a variety of disorders such as arthritis, systemic lupus erythematosus, and infection. Laboratory measures used to detect and monitor inflammation include acute phase reactants such as C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). CRP testing may be performed using conventional or high-sensitivity methods. In clinical practice these tests are used in different contexts; when the policy allows either marker, CRP is the preferred biomarker.

    Definitions

    inv-11: CRP — C-reactive protein
DefinitionC-reactive protein (CRP) is an acute phase reactant released during inflammation; conventional CRP and high-sensitivity CRP (hsCRP) are allowed testing methods.
Preferred useWhen either CRP or ESR are allowed for a condition, CRP is the preferred biomarker; if both are ordered where only one is allowed, only CRP will be approved.
Clinical roleCRP can be used to detect and monitor inflammation across conditions (e.g., infection, arthritis) and for serial monitoring during therapy.
inv-12: ESR — Erythrocyte sedimentation rate
DefinitionErythrocyte sedimentation rate (ESR) is a laboratory measure using the sedimentation of red blood cells to assess inflammation.
Test roleESR is an alternative measure of inflammation for specified conditions (e.g., Hodgkin lymphoma, some rheumatologic diseases) when designated in the condition-specific notes.

Test preference — CRP preferred over ESR

When both CRP and ESR are ordered for a condition where only one is allowed, only CRP will be approved. Either conventional or high-sensitivity CRP methods are acceptable.

    • Allowed methods: conventional CRP or high-sensitivity CRP
    • Behavior: Concurrent CRP+ESR orders for a condition allowing only one will result in approval of CRP only
    Denial Risk

    Denial triggers — documentation required

Denial triggers — measurement of ESR for individuals without a diagnosed inflammatory condition and measurement of CRP and/or ESR performed during a general exam without abnormal findings may not be reimbursed. Ensure documentation supports a diagnosed or suspected inflammatory condition when submitting claims.

    • Denial risk: ESR ordered without a diagnosed inflammatory condition
    • Denial risk: CRP and/or ESR ordered during a general exam without documented abnormal findings or clinical indication
LimitationsWhen either CRP or ESR are allowed, CRP is preferred and if both are ordered where only one is allowed, only CRP will be approved; ESR alone may not be reimbursed for individuals without a diagnosed inflammatory condition.
