OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria for ENFD

    ENFD coverage criteria

Reimbursement for epidermal nerve fiber density (ENFD) measurement from a skin biopsy is provided only when ALL of the following are met; sweat gland nerve fiber density measurement is not reimbursable.

    ALL of the following

    • ALL of the following

      • Patient presents with symptoms of painful sensory neuropathy

        Symptom-based indication required

    • ALL of the following

      • No history of a disorder known to predispose to painful neuropathy (e.g., diabetic neuropathy, toxic neuropathy, HIV neuropathy, celiac neuropathy, inherited neuropathy)

        Exclude common predisposing conditions

    • ALL of the following

      • Physical exam shows no evidence of findings consistent with large-fiber neuropathy (e.g., reduced or absent muscle-stretch reflexes or reduced proprioception and vibration sensation)

Exam must not demonstrate large-fiber signs

    • ALL of the following

      • Electromyography and nerve-conduction studies are normal and show no evidence of large-fiber neuropathy

Normal EMG/NCS required to support small-fiber neuropathy evaluation

    ALL of the following

    • Epidermal nerve fiber density measurement is not reimbursable for situations not meeting the above criteria

Claims without all required criteria should not be reimbursed

    • Measurement of sweat gland nerve fiber density is not reimbursable

Sweat gland nerve fiber density explicitly excluded from reimbursement

    Provider Responsibilities and Billing

    Documentation Required

Submit accurate documentation and respond to documentation requests

Providers are responsible for submission of accurate documentation of services performed and must code claims using valid HIPAA-approved code sets and industry-standard guidelines; claims are subject to code edit protocols and review. Upon request, providers are urged to submit any additional documentation to support the claim.

    • Use valid code combinations from HIPAA-approved code sets (e.g., CPT, HCPCS, ICD-10-CM).
    • Follow industry standard coding guidelines (Uniform Billing Editor, AMA CPT Assistant, CMS NCCI edits, etc.).
    • Claims may be reviewed against benefit coverage, provider contract terms, medical and clinical payment policies, and coding software logic.

    Ordering and Authorization Requirements

    Note

Follow plan and contract requirements for ordering and submit required documentation

The policy does not specify who may order testing; providers must follow applicable plan documents and their provider contract when ordering and must submit the documentation required by the plan or contract.

    • Follow plan documents (Certificates of Benefits, benefit booklets, Summary Plan Descriptions) and your provider contract to determine ordering privileges.
    • Submit supporting documentation as requested by the plan per documentation and claims submission requirements.

    Services Not Covered

Epidermal nerve fiber density (ENFD) testing from a skin biopsy is reimbursable only when ALL of the following are met: the patient presents with symptoms of painful sensory neuropathy; there is no history of a disorder known to predispose to painful neuropathy (for example, diabetic, toxic, HIV-related, celiac, or inherited neuropathy); the physical examination shows no evidence of large‑fiber neuropathy (such as reduced/absent reflexes or reduced proprioception/vibration); and electromyography and nerve‑conduction studies are normal with no evidence of large‑fiber neuropathy. For any situations that do not meet these enumerated criteria, ENFD testing is not reimbursable.

Measurement of sweat gland nerve fiber density is explicitly not reimbursable. In addition, the policy states that ENFD testing is not reimbursable for all other situations not described in the specified coverage criteria.

    Frequency Limits

    Frequency limits
Stated frequency limitsNo specific frequency limits are provided in the policy (frequency limit fields are blank).
Policy update note relevant to frequencyPolicy update history entries show multiple literature-review updates but do not add frequency or periodicity limits.
Action for providersFollow plan-specific benefit documents for any applicable frequency limitations, as the policy itself does not specify a limit.
