Thyroid function tests may be reimbursable in specific clinical scenarios and per specified monitoring intervals.
Hypothyroidism evaluation and monitoring: Covered when TSH is used to confirm or rule out primary hypothyroidism; free T4 (fT4) as follow-up to abnormal TSH; TSH and fT4 for suspected secondary (central) hypothyroidism; for individuals treated for primary hypothyroidism, monitor with TSH and fT4 every 6 weeks after a dosage change and annually when stable; for individuals treated for secondary hypothyroidism, monitor with fT4 every 6 weeks after a dosage change and annually when stable.
See Note 1 for symptoms list.
Hyperthyroidism evaluation and monitoring: Covered when TSH is used to confirm or rule out overt hyperthyroidism; free T4 (fT4) as follow-up to abnormal TSH; total T3 (TT3) or free T3 (fT3) to confirm hyperthyroidism; fT4 to distinguish overt from subclinical hyperthyroidism; monitoring after treatment with repeat TSH and fT4 every 8 weeks during the first year and annual monitoring thereafter for risk of relapse or late-onset hypothyroidism.
See Note 2 for symptoms list; annual monitoring after first year even if asymptomatic.
Asymptomatic at-risk one-time screening: One-time TSH screening may be reimbursable for asymptomatic individuals at high risk for thyroid disease due to personal or family history of thyroid dysfunction; personal or family history of type 1 diabetes or other autoimmune disease; disease or neoplasm of the thyroid or other endocrine glands; chronic or acute urticaria; pediatric short stature; or pediatric failure-to-thrive.one-time
Consolidated one-time screening criteria.
Pregnancy and postpartum: For pregnant or postpartum individuals with symptoms of thyroid dysfunction, TSH and fT4 testing once every 4 weeks may be reimbursable. For pregnant/postpartum individuals diagnosed with hyperthyroidism, total T4 (TT4), antithyroglobulin antibody (TgAb), thyrotropin receptor antibodies (TRAb), and antithyroid peroxidase antibody (TPOAb) may be reimbursable.frequency: every 4 weeks
Assays must have trimester-specific normal ranges; fT4 preferred; TT4 useful if discordant or trimester-specific fT4 ranges unavailable.
Immune reconstitution therapy monitoring: TSH testing once every 3 months with reflex fT4 and fT3 when TSH is abnormal is reimbursable for individuals undergoing immune reconstitution therapy (e.g., alemtuzumab for MS, HIV on HAART, post-allogeneic BMT/HSCT).every 3 months
Reflex fT4/fT3 when TSH abnormal.
Hypothalamic-pituitary disease monitoring: For individuals with hypothalamic-pituitary disease, monitor TSH and fT4 biannually for individuals less than 18 years of age and annually for individuals 18 years of age or older.biannually (<18), annually (>=18)
Primary mitochondrial disease screening: Annual screening of TSH and fT4 for individuals diagnosed with primary mitochondrial disease may be reimbursable.annually
Thyroid cancer surveillance: Serum thyroglobulin and/or Tg-Ab testing may be reimbursable for detection of tumor recurrence, post-surgical evaluation, surveillance, and maintenance for differentiated thyroid carcinomas.
Periodic antibody testing: For individuals with hypothyroidism or hyperthyroidism, testing once every three years for thyroid antibodies (Tg-Ab, TPOAb, TRAb, thyroid-stimulating immunoglobulins/TSI) may be reimbursable.every 3 years
Specific antibodies enumerated.
