A hospital bed without a therapeutic benefit for the member's medical condition or illness is not medically necessary.

    • Variable-height bed criteria are met, including the fixed-height criteria and the need for a non-fixed bed height for transfers.
    • Semi-electric bed criteria are met, including the fixed-height criteria and the need for frequent or immediate position changes with the ability to operate the controls.

Hospital beds may provide positioning, elevation, transfer, and body-positioning functions that are not feasible with an ordinary bed. The policy addresses fixed-height, variable-height, semi-electric, total-electric, bariatric, pediatric, and air fluidized beds; air fluidized beds distribute body weight over a large surface area and are intended for selected patients with severe pressure sores or extensive burns.

    Denial Risk

Excluded, Non-Medically Necessary, or Inclusive Equipment

Do not bill equipment that is excluded, not medically necessary, or inclusive of the hospital bed rental or purchase. These services may be denied regardless of whether the equipment is ordered with a covered bed.

    • Benefit exclusions include personal, comfort, or convenience items; deluxe features when standard equipment meets functional needs; nonhospital adjustable beds; bed boards; bed elevators; bed wedges or pillows; bedrail pads; custom bedroom equipment; mattresses; over-bed tables, trays, and lap boards (E0274); power or manual lounge beds; safety enclosures or canopies; synthetic or lambswool sheepskin pads; and waterbeds.
    • HCPCS codes identified as benefit exclusions include E0187 (water pressure mattress), E0188 (synthetic sheepskin pad), E0189 (lambswool sheepskin pad), E0190 (positioning cushion, pillow, or wedge), E0198 (water pressure pad), E0199 (dry pressure pad), and E0271 (innerspring mattress).
    • HCPCS codes identified as benefit exclusions also include E0371 (nonpowered advanced pressure-reducing overlay), E0373 (nonpowered advanced pressure-reducing mattress), E0700 (safety equipment), E0710 (restraints), E0912 and E0940 (freestanding trapeze bars).

    • E0310, bedside rails, half-length, is denied as inclusive of hospital bed rental or purchase.
    Denial Risk

    Air Fluidized Bed Home-Setting Risks

Home use of an air fluidized bed is not medically necessary when the home setting cannot safely support the therapy or the required care cannot be delivered.

    • The patient requires wet soaks or has moist wound dressings that are not protected by an impervious covering.
    • The caregiver cannot provide the care required for a patient using an air fluidized bed.
    • The home’s structural support is inadequate for the air fluidized system, which weighs 1,600 pounds or more.
    • The home electrical system cannot support the anticipated increase in energy consumption.
    Note

    Incomplete Request or Contract Limitation

Incomplete authorization requests may delay a decision until missing information is submitted. Benefits vary by member contract and may differ for NEHP/ABNE, FEP, or ASO members; verify eligibility and coverage before providing the service, because contract language takes precedence when it conflicts with the policy.

    • Confirm that the request includes all information listed in the policy guidelines.
    • Check the member’s specific benefit plan, including applicable exclusions and contract limitations.
    • For FEP members, consult FEP customer service or the FEP Service Benefit Plan Brochure.
    • For ASO members, consult the employer’s benefit plan documents or customer service.
    Billing Rule

    Audit Recoupment

Blue Cross VT may audit any provider or facility for compliance with the medical policy. If an audit identifies noncompliant payments, the Plan may recoup all such payments.

    • Maintain documentation supporting medical necessity, prior approval, coding, and billing compliance.
    • Audit recoupment may apply to noncompliant payments identified after provider or facility review.
