Coverage Criteria for Implantable Intrathecal or Epidural Drug Delivery Systems

inv-01: Preliminary Trial — Medical Necessity

Preliminary epidural or intrathecal opioid trial is medically necessary when ALL of the following are met:

Preliminary trial — indications and prerequisites: Request is for either: (1) chronic intractable pain of malignant origin when: a) inadequate response to, or intolerable side effects from, noninvasive methods of pain control such as systemic opioids; b) life expectancy > three months; c) no evidence of epidural metastatic lesion(s) or tumor encroachment of the thecal sac by imaging; OR (2) chronic intractable pain of nonmalignant origin when: a) pathology for the pain has been identified; b) life expectancy > three months; c) failure or inability to tolerate conservative treatments (systemic pharmacotherapy, physical therapy, behavioral health, appropriate nonsurgical treatment); d) compliance with prior treatment attempts; e) a psychological evaluation confirms mental health is not a major contributor to chronic pain symptoms; f) further surgical intervention not indicated or likely effective; g) prior to the trial, systemic opioids have been weaned by at least 50%; h) opioid induced hyperalgesia has been ruled out as a possible cause of the chronic pain symptoms.
Preliminary trial — contraindications (none present): None of the following contraindications are present: known allergies to materials in the implant; active alcohol or drug abuse (including opioid addiction and intravenous drug abuse); diagnosis of dementia or psychosis; active systemic infection; active infection at the site of implantation.

inv-02: Permanent Implantation — Medical Necessity

Permanent implantation of a pump is medically necessary when ALL of the following are met (for malignant and nonmalignant indications as specified):

Permanent implantation — malignant origin: For chronic intractable malignant pain: the preliminary trial criteria are met and the trial provided ≥ 50% reduction in pain with minimal side effects; body size is sufficient to support the weight and bulk of the device; no other implanted programmable devices for which interaction may inadvertently change the prescription; no known allergy or hypersensitivity to the drug being used.
Permanent implantation — nonmalignant origin: For severe chronic nonmalignant pain: the preliminary trial criteria are met and the trial provided ≥ 50% reduction in pain and increase in function with minimal side effects; there is a plan in place to continue to wean systemic opioids; no active coagulopathy; body size is sufficient to support the weight and bulk of the device; no other implanted programmable devices for which interaction may inadvertently change the prescription; no known allergy or hypersensitivity to the drug being used; no evidence of increased intracranial pressure; no spinal anomalies that may complicate implantation and fixation of a catheter for drug delivery; continued active participation in behavioral health or psychological treatment modalities.
Permanent implantation — contraindications (none present): None of the following contraindications are present: known allergies to materials in the implant; active alcohol or drug abuse (including opioid addiction and intravenous drug abuse); diagnosis of dementia or psychosis; active systemic infection; active infection at the site of implantation.

Relative and absolute contraindications must be verified and documented prior to trial or implantation. Absolute contraindications include systemic infections, known allergies to materials in the implant, active intravenous drug abuse, diagnosis of dementia or psychosis, and active infection at the implantation site.

Relative contraindications that may preclude proceeding without additional evaluation include an underweight/atrophied patient (low BMI), ongoing anticoagulation that cannot be safely stopped, active bleeding, very high opioid tolerance, lack of adequate social or family support, and lack of reliable access to follow-up medical care.

Providers should document the absence of listed absolute contraindications and address any relative contraindications in the medical record before scheduling a trial infusion or permanent implantation to avoid denial of coverage.

Coverage decisions for preliminary trials and permanent implantation are guided by this clinical policy but remain subject to the member's benefit terms. Providers must confirm plan-level prior authorization and any applicable exclusions or limitations in the member's coverage documents prior to submitting requests.

Where applicable, state Medicaid provisions and Medicare NCDs/LCDs take precedence and may affect coverage determinations; discrepancies between this policy and governing law or coverage documents will be resolved according to those authorities.

Use of intrathecal drug delivery systems for nonmalignant (chronic) pain is considered controversial and is generally reserved as a last resort after exhaustive conservative and surgical options have failed or are not tolerated.

Because long-term comparative evidence of superiority over oral opioids is limited, permanent implantation for noncancer pain requires strict adherence to the policy criteria (including successful trial response, documentation of failed conservative therapies, psychological evaluation, and plans for opioid weaning) before coverage will be considered.

Provider Responsibilities, Prior Authorization, and Documentation

    Prior Authorization

Prior authorization required; use listed procedure and device codes

Prior authorization is required for services and device supply codes listed in this policy; those CPT and HCPCS codes must be used on authorization requests and claims submissions.

    • Affected CPT and HCPCS/HCPCS-like codes include (but are not limited to) 62320–62327, 62350–62371, A4300, A4301, E0782, E0783, E0785, E0786, C1772, C1755, J2274, S0093.
    Note

Policy is a medical necessity guide — confirm benefit coverage

This clinical policy is intended as a guide to medical necessity to assist coverage decisions and benefit administration; it does not replace plan-level benefit verification or constitute a guarantee of payment.

    • Providers should confirm plan-specific prior authorization requirements and member benefit coverage in the member's coverage documents.

    Required Conservative Therapies and Documentation

inv-23: Documentation of prior conservative treatments tried and failed — criteria group

Documentation must demonstrate prior conservative treatments attempted and reason(s) for failure or intolerance:

Required documentation of conservative treatments: For nonmalignant pain, documentation of failure or inability to tolerate conservative treatments including, but not limited to, systemic pharmacotherapy, physical therapy, behavioral health treatment for pain, and appropriate nonsurgical treatments. Records should document treatment type(s), duration, response, and reasons for discontinuation or intolerance.
Contextual documentation elements: Also document indication (malignant vs nonmalignant chronic intractable pain), prior attempts at medical and noninvasive care, results of preliminary intrathecal/epidural trial (including pain reduction ≥ 50% and increased function for nonmalignant pain where required), opioid wean of ≥50% prior to trial when indicated, psychological evaluation findings, and absence of listed contraindications.

    Frequency Limits and Trial Requirements

inv-24: Preliminary intrathecal or epidural trial — Trial infusion prior to permanent implantation; exceptions for advanced malignant disease may be reviewed case-by-case.
Trial infusion required prior to permanent implantationA preliminary intrathecal or epidural trial infusion is required before permanent pump implantation (with or without catheter)
Exception for advanced malignant diseaseExceptions to the trial requirement for limited advanced malignant cases may be reviewed case-by-case
Trial purposeTrial determines adequate response (predefined improvement, usually ≥50%) without intolerable adverse effects

    Imaging and Procedural Guidance

    Billing Rule

Use fluoroscopy or CT for catheter placement/implantation

Catheter placement for trial or implantation is typically performed under fluoroscopic guidance or CT; use imaging guidance as specified by procedure coding and clinical requirements.

    • Several CPT codes differentiate procedures performed with or without imaging guidance (eg, 62321, 62323, 62327).
    • Document imaging used for catheter positioning in operative/reporting notes.

    Background and Clinical Context

Implantable intrathecal and epidural drug delivery systems (also called intrathecal drug delivery or IDD systems) are devices that combine an implanted catheter with a constant-flow or programmable pump to deliver medications directly into the intrathecal or epidural space for targeted central nervous system analgesia.

These systems are used primarily for long-term management of select patients with recalcitrant pain—most commonly cancer-related pain—and are considered for severe nonmalignant pain only after conservative and surgical options have failed. A preliminary intrathecal or epidural trial infusion is performed to assess response before permanent implantation.

Successful trials are typically defined as providing a predefined improvement in pain, usually ≥ 50%, without intolerable adverse effects; if the trial is successful, the pump and catheter may be implanted under general anesthesia, with catheter positioning generally performed under fluoroscopic guidance.

    Definitions

inv-20: Intrathecal drug delivery system (IDD) — Definition of implanted pump and catheter delivering drug to CSF; programmable features.
DefinitionAn implanted pump and catheter that delivers drug directly into the spinal (cerebrospinal) fluid
Programmable featuresDevice can be constant-flow or programmable for continuous or variable infusion rates
Indication contextProvides targeted CNS delivery for long-term management of select patients with intractable pain
inv-21: Successful trial — Definition: preliminary infusion providing predefined improvement in pain (usually ≥50%) without intolerable adverse effects.
Successful trial definitionA preliminary intrathecal or epidural infusion that provides a predefined improvement in pain, usually ≥50%, without intolerable adverse effects
    Nonmalignant requirement

    Not Covered Indications

Intrathecal or epidural pump therapy is not covered for nonmalignant pain when all specified trial and permanent-implantation criteria have not been met. This includes failure to document prior conservative treatments, absence of a successful preliminary trial (see trial response requirements), or lack of required documentation such as psychological evaluation and opioid-wean plans.

Procedures are also not covered when any listed absolute contraindication is present (for example, active systemic infection, active infection at the implantation site, active substance abuse, or known allergy to implant materials). Providers should address contraindications prior to submission to avoid denials.

    Provider Coding and Billing Reminders

    Prior Authorization

Reminder — use the policy's listed CPT/HCPCS codes on requests and claims

Prior authorization and claims must use the CPT and HCPCS codes listed in the policy; reference the enumerated codes when submitting requests and claims.

    • Include codes such as 62320–62327, 62350–62370 and A4300, E0783, J2274, S0093 where applicable.
    • Verify code selection matches the service rendered (with/without imaging guidance, pump type, analysis/reprogramming).

    Step Therapy

Preliminary trial infusion required prior to permanent implantation

A preliminary epidural or intrathecal trial infusion (with or without catheter) is required before permanent pump implantation except in limited malignant cases reviewed on a case‑by‑case basis.

    • Trial must demonstrate a predefined improvement in pain (usually ≥50%) without intolerable adverse effects.
    • Exceptions for advanced malignant disease may be considered on individual review.
    Note

Provider-facing alert: follow clinical judgment and plan/regulatory rules

Providers should note the policy was developed by experienced/licensed clinicians and is consistent with accepted standards at the time of approval; follow applicable legal/regulatory and plan requirements when applying the policy.

    • This clinical policy does not dictate clinical practice — providers must exercise professional judgment and are responsible for medical advice and treatment.
    • Plan retains right to change or withdraw the policy; state Medicaid and Medicare NCDs/LCDs may take precedence where applicable.
    Documentation Required

Required documentation for authorization and coverage

Document the indication (malignant or nonmalignant chronic intractable pain), prior conservative therapies tried and failed or intolerable, life expectancy where applicable, and trial results demonstrating the required pain reduction and functional improvement.

    • Record opioid wean (≥50% reduction) prior to trial for nonmalignant pain and rule out opioid‑induced hyperalgesia.
    • Include psychological evaluation results and documentation that no listed contraindications are present.
    Note

Provider responsibility — exercise professional judgment; policy guides coverage

Providers are expected to exercise professional medical judgment in treating members; coverage determinations are guided by this policy but remain subject to contract terms, applicable laws, and plan administrative policies.

    • Providers remain solely responsible for medical advice and treatment decisions; the policy is not intended to recommend specific treatment.
    • Verify and follow plan‑level administrative requirements when submitting requests or claims.
    Denial Risk

Denial risk when trial absent/unsuccessful or contraindications present

Requests may be denied if a preliminary trial was not performed or the trial did not achieve the policy’s required response, or if any absolute contraindications are present.

    • Denial triggers include absence of a trial or failure to demonstrate ≥50% pain reduction (and increased function for nonmalignant pain).
    • Denial risk if contraindications such as active systemic infection, active infection at the implantation site, active substance or alcohol abuse, dementia/psychosis, or known allergy to implant materials are present.
    Note

Coverage subject to member contract terms, exclusions, and legal requirements

Coverage decisions are subject to all terms, conditions, exclusions and limitations of the member's coverage documents and to state and federal requirements; discrepancies are governed by law.

    • Plan-level benefits, exclusions, Medicaid provisions, and Medicare NCDs/LCDs may affect coverage and take precedence where applicable.
For nonmalignant pain, success includes ≥50% pain reduction and increased function with minimal side effects
Use in implantation decisionSuccessful trial result is required to proceed with permanent implantation per policy criteria
