inv-01: Medically necessary indications

Covered when ALL of the following are met

Breast ductal lavage (DL): DL is considered medically necessary as an appropriate alternative to aspiration to diagnose members with non-lactational sporadic nipple discharge when the yield of cellular material from a direct nipple aspirate is too low to permit adequate cytological analysis.

Examples of DL systems: Pro-Duct, ForeCYTE, MASCT, Halo (but screening indications are investigational).

Fiberoptic ductoscopy - diagnostic: Fiberoptic ductoscopy combined with cytology testing is considered medically necessary for diagnosing intraductal lesions in women with non-lactational sporadic nipple discharge accompanied by documented positive cytology.

Fiberoptic ductoscopy - therapeutic: Fiberoptic ductoscopy is considered medically necessary as a guide for resection of known breast intraductal cancer.

inv-02: Experimental / Investigational

The following interventions are considered experimental and investigational because effectiveness has not been established:

DL or devices for screening/ risk assessment: Breast ductal lavage and devices (e.g., Pro-Duct, ForeCYTE, Halo) are considered experimental and investigational for breast cancer screening and breast cancer risk assessment and for other indications (e.g., treatment of non-lactational mastitis).

Fiberoptic ductoscopy for screening: Fiberoptic ductoscopy (Acueity System) is considered experimental and investigational for breast cancer screening.

Combined DL and endoscopy for contralateral evaluation: Breast ductal endoscopy combined with ductal lavage for evaluation of contralateral breast in women with ipsilateral breast cancer is considered experimental/investigational.

Biomarker assays on lavage samples: Assessment of genetic methylation patterns and miRNA analysis in ductal lavage samples for prediction or detection of breast cancer are considered experimental/investigational.

General summary: DL and investigational nipple aspiration tests lack established clinical benefit as screening tools; DL has technical limitations (inconsistent NAF production, high inadequate sample rates, fair reproducibility) that limit its utility for screening or serial biomarker monitoring.

Supported by feasibility and reproducibility studies and FDA commentary.

Situations where FD/DL may have clinical value: FD can aid diagnosis and localization in patients with pathologic nipple discharge and may improve sensitivity when combined with cytology; DL/FD can be useful in research settings for biomarker discovery (metabolomic and miRNA profiling) and in select procedural contexts (e.g., guiding duct excision).

Evidence includes studies reporting high FD sensitivity and improved sensitivity with FD + cytology, and metabolomic/miRNA pilot studies.

Non-lactational mastitis: DL with intraductal infusion of corticosteroids/antimicrobials showed feasibility and a high complete response rate (93.8%) in a single-arm proof-of-concept trial; randomized controlled trial data are needed.

Use of ductal lavage (DL), nipple aspiration, or ductoscopy as a tool for population breast cancer screening is not supported by available evidence and is considered investigational/not covered. The policy notes that these interventions lack endorsement from federal public health agencies and professional organizations, and that data are insufficient to demonstrate clinical benefit as a screening test or as a routine risk‑assessment tool for asymptomatic women. Standard imaging and clinical evaluation remain the established approaches for breast cancer screening and risk assessment.

Nipple aspiration tests marketed as screening substitutes (for example, the ForeCYTE Breast Health test and related MASCT systems) are not supported by evidence and are considered investigational for breast cancer screening or risk assessment. The FDA has taken regulatory action relating to some commercial tests: a 2013 FDA warning letter cited misleading labeling for Atossa Genetics’ ForeCYTE test, and a voluntary recall was initiated in October 2013 to remove the ForeCYTE Breast Health Test from the market. These regulatory events may affect clinical availability and coverage.

The FDA has stated that the nipple aspirate test is no substitute for mammography. Unlike the cervical Pap smear, nipple aspirate testing lacks validated evidence demonstrating screening effectiveness, does not localize abnormal cells for targeted biopsy, and may produce a high proportion of samples with scant or no cellular material. Companies have reported that over 90% of nipple aspirate samples may contain very few or no cells, limiting diagnostic reliability and risking false reassurance or false positive/negative results.

Based on current evidence, DL, fiberoptic ductoscopy, and NAF-based tests used for breast cancer screening or routine risk assessment are considered not medically necessary / investigational. While DL and ductoscopy have limited roles in diagnostic contexts (see medically necessary indications), their use as screening modalities or for serial risk surveillance has not demonstrated sufficient sensitivity, reproducibility, or outcome benefit to support coverage outside of research or specified diagnostic indications.

Ductal lavage (DL) as a screening tool in asymptomatic women or for routine risk assessment is not supported due to technical and performance limitations documented in the literature: inconsistent nipple aspirate fluid (NAF) production, a high rate of inadequate cellular specimens, sub-optimal reproducibility of cytologic findings, and low sensitivity for detecting existing breast cancer. Studies report variable cannulation and cell-yield rates and fair to poor agreement for cytologic diagnosis over time, limiting the utility of DL for serial monitoring or population screening.

Background: Breast ductal lavage (DL) and nipple aspirate fluid (NAF) collection are techniques intended to obtain epithelial cells from the lactiferous ducts for cytologic examination or biomarker testing. DL typically involves catheterization of ducts with saline infusion to wash and collect ductal epithelial cells, while NAF collection uses noninvasive suction devices (examples include the Halo and ForeCYTE systems). Although some specimen‑collection devices have received FDA 510(k) clearance for collection, evidence supporting clinical effectiveness of DL/NAF testing as screening tools is insufficient, and major guideline bodies and federal agencies do not recommend DL for breast cancer screening outside of research settings.
