    Clinical & Coding Specifications

    Clinical Context

A typical patient is a reproductive-age woman presenting with infertility, recurrent pregnancy loss, abnormal uterine bleeding, or cyclical pelvic pain after prior uterine instrumentation (e.g., dilation and curettage), cesarean delivery, or infection. The gynecologist evaluates with history, pelvic exam, transvaginal ultrasound, and saline-infusion sonohysterography or hysterosalpingography demonstrating intrauterine adhesions (Asherman syndrome) or synechiae. After counseling, the patient is scheduled for operative hysteroscopy with lysis of intrauterine adhesions (58559).

The clinical workflow: preoperative evaluation includes informed consent, review of imaging, and optimization of comorbidities. In the operating room or ambulatory surgery center (typical site of service), under monitored anesthesia care or general anesthesia, a hysteroscope is introduced transcervically. Adhesions are lysed using scissors, electrosurgical instruments, or a resectoscope as needed. Distention media is used for visualization. The surgeon documents indications, operative findings (location and extent of adhesions), lysis technique, estimated blood loss, any concomitant procedures, and postoperative plan (e.g., intrauterine balloon or estrogen therapy to prevent reformation). Typical recovery is same-day discharge with activity restrictions and follow-up imaging or hysteroscopic re-evaluation as indicated.
