    Clinical & Coding Specifications

    Clinical Context

A typical patient is an adult presenting for reconstruction of a soft-tissue defect requiring skin grafting or local flap coverage after tumor excision, trauma, or chronic wound debridement. The procedure described by 15106 is used when the surgeon harvests a full-thickness or split-thickness skin graft or prepares the recipient site and performs local tissue rearrangement for coverage of a defect that does not require complex myocutaneous flap reconstruction. The workflow includes preoperative evaluation, marking of the donor and recipient sites, administration of anesthesia (local, regional, or general depending on defect size and patient factors), surgical preparation of the wound bed with debridement or excision as needed, harvest and preparation of the graft or flap, inset and securing of the graft/flap, and postoperative dressing and instructions. Typical sites of service are the outpatient surgical suite, ambulatory surgery center, or hospital operating room. The patient typically requires a brief postoperative observation period and wound care follow-up visits to assess graft take and healing over 1–3 weeks.
