    Repeat imaging within six months

Repeat MRI of the same anatomic site within six months is reviewed case by case and requires at least ONE qualifying circumstance.

Repeat MRI review: The prior testing was inconclusive or short-term follow-up imaging was recommended; clinical documentation shows progressive worsening of symptoms or new physical-examination findings prompting repeat imaging and imaging results will alter patient management; or interval surgery with suspected complication or significant new trauma occurred in the same anatomic region.same site within past 6 months

The most recent imaging reports must be submitted.

A condition or diagnosis not listed in the indications of coverage is considered experimental, investigational, and unproven to affect health outcomes.

Requests for MRI of the internal auditory canal are directed to the MRI Brain guideline rather than this guideline.

A listed diagnosis alone does not establish medical necessity. The applicable coverage criteria must also be met; otherwise, the MRI is considered not medically necessary.

For suspected infectious or inflammatory adenopathy, MRI is not medically necessary unless failure of a documented two-week treatment trial has been established.

    Covered Clinical Indication Groups

    • Applies to all neck, face, or orbit MRI procedures.
    • Submit a standard written order, medical-record information and medical-necessity documentation, and correct service coding.
    Documentation Required

    TMJ Trauma: Indeterminate Preliminary Imaging

MRI for TMJ trauma is medically necessary when assessment of meniscal position and integrity is needed and the preliminary plain X-ray or CT scan is indeterminate.

    • Submit the indeterminate plain X-ray or CT result as part of the preliminary-imaging pathway.

    Clinical Context

MRI of the neck, face, and orbit is used to evaluate infection, trauma, masses, and abnormalities involving the external eye and nose. It may also support therapy planning and follow-up for face and neck neoplasms, evaluation of vocal-cord lesions, and assessment of cervical lymphadenopathy. Cancer-related imaging is evaluated using NCCN as a primary reference together with current medical references and specialty-society guidance.
