OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

  1. Determine the filing period by subtracting the date of service from the date the health plan receives the claim and comparing the resulting number of days with the applicable limit.
  2. For services rendered on consecutive days, count the filing limit from the last day of service.
  3. Count limits in calendar days unless otherwise specified.
  4. If the member has other health insurance that is primary, count timely filing from the date of the other carrier's Explanation of Payment (EOP).
  5. If the claim was filed outside the timely filing limit, reimbursement requires ONE of the following

    • The provider presents documentation proving that a clean claim was filed within the applicable filing limit.

Claims filed outside the timely filing limit are otherwise not subject to reimbursement.

    • The health plan has temporarily waived timely filing requirements following a documented natural disaster or under applicable state guidance.

    Provider Submission Responsibilities

    Billing Rule

Submit claims within the filing limit

Submit the initial claim so that it is received and accepted within the applicable timely filing limit. The standard is 12 months for both participating and nonparticipating providers and facilities, unless a controlling requirement provides otherwise.

    Documentation Required

    Document proof of timely filing

For a claim filed outside the timely filing limit, provide documentation proving that a clean claim was filed within the applicable filing limit. Without that documentation, the claim will not be subject to reimbursement.

    Billing Rule

    Calculate the filing period

Calculate timely filing by subtracting the date of service from the date the health plan receives the claim and comparing the result with the applicable limit. For services rendered on consecutive days, calculate the limit from the last day of service; limits are based on calendar days unless otherwise specified.

    Billing Rule

    Coordinate with other insurance

When the member has other health insurance that is primary, calculate timely filing from the date of the other carrier’s Explanation of Payment (EOP).

    Billing Rule

    Apply controlling requirements

Apply any applicable provider, state, federal, or Centers for Medicare & Medicaid Services (CMS) contract or requirement that supersedes the health plan standard. The policy also states that such requirements may determine the applicable filing limit.
