OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Admission medical necessity criteria

Medically necessary when all of the following criteria are met.

    Acute psychiatric intensive care admission

    • Core clinical criteria

      • Age: Member is between 18 and 64 years of age.18-64 years
      • Serious mental illness: Member has a serious mental illness as defined in Iowa Administrative Code 441-subrule 77.47(1).
      • Imminent risk: Member has a current, severe, imminent risk of serious harm to self or others.
      • Additional complexity

        • Qualifying complexity factors

          • Complex comorbidities, including intellectual or developmental disability, autism spectrum disorder, substance use disorders, or traumatic brain injuries.
          • Violence or aggression: History of violence as a clinical risk or current aggression secondary to mental illness.
          • Rejected transfer: A requested transfer has been rejected by the inpatient level of care by one or more hospitals due to severity of symptoms.
          • Treatment resistance: Lack of responsiveness to typical interventions or a treatment-refractory condition.
          • Disorganized psychosis or mania:
    • Specialized care need

      • Qualifying specialized-care needs

        • Fall precautions: Fall precaution protocol is in place.
        • Restraint or seclusion capability: Restraints or a seclusion room are required.
        • Activities of daily living assistance: Assistance with activities of daily living is required.
        • Complex nursing care is required.
        • Acute cognitive impairment: Cognitive functioning is acutely impaired from baseline.

    Discharge criteria

Discharge when any one of the following criteria is sufficient.

Discharge from acute psychiatric intensive care

    • Lower or different level of care is appropriate: The member no longer meets continuing-stay criteria for acute psychiatric intensive care but meets admission criteria for general inpatient mental health services or another safe level of care.
    • Goals met and continuing care arranged: Treatment-plan goals and objectives have been substantially met and/or a safe continuing-care program can be arranged and implemented at a less-intensive level of care.
    • No expected progress: The member is not progressing toward treatment goals and there is no reasonable expectation of progress at this level of care.
    • Chronic condition requires long-term care:

Coverage is excluded when the member can be safely maintained and effectively treated at a less-intensive level of care rather than acute psychiatric intensive care.

Coverage is excluded when the member has serious and persistent mental illness but is not experiencing an acute exacerbation of that illness.

Coverage is excluded when the primary issue is social, legal, or medical without a concurrent major psychiatric episode meeting criteria for this level of care. Coverage is also excluded when admission is being used as an alternative to incarceration, justice-system involvement, respite, or housing.

Coverage is excluded when a medical condition requires treatment in a medical or surgical setting, regardless of the member’s psychiatric presentation.

Coverage is excluded when medical comorbidities cannot be safely managed in the specialty psychiatric setting.

Coverage is excluded for exclusive behavioral dyscontrol occurring in the context of traumatic brain injury, intellectual disability, pervasive developmental disorder, dementia, or another medical condition, when there is no acute crisis related to a current DSM diagnosis.

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for acute inpatient psychiatric intensive care services before services are provided.

    Documentation Required

    Clinical Necessity Documentation

Document that the member meets all applicable clinical-necessity requirements for acute intensive psychiatric care, including age, diagnosis, current risk, complexity, and the need for enhanced resources.

    • Member is between 18 and 64 years of age.
    • Member has a serious mental illness as defined in Iowa Administrative Code 441—subrule 77.47(1).
    • Member has a current, severe, and imminent risk of serious harm to self or others.
    • Document additional complexity of need related to at least one of the following: complex comorbidities (including intellectual or developmental disability, autism spectrum disorder, substance use disorder, or traumatic brain injury); history of clinical-risk violence or current aggression secondary to mental illness; rejected transfer request due to symptom severity; lack of responsiveness to typical interventions or treatment-refractory condition; disorganized psychotic state or manic thought process impairing function or safety; significant disruption to the unit milieu; high elopement risk; or another atypical reason the treating mental health provider determines additional resources are needed for safety.

    Background

Acute psychiatric intensive care is intended for rapidly developing, severe psychiatric conditions that require emergency treatment and intensive psychiatric services beyond the capabilities of a general inpatient psychiatric unit. It provides 24-hour skilled nursing, daily medical care, structured milieu treatment, multidisciplinary assessment, enhanced staffing, and increased capacity for observation or intervention. The clinical goal is acute stabilization followed by transition to a general inpatient psychiatric unit or another less-intensive level of care.
