    Coverage Criteria

    Covered clinical indications and documentation

Considered appropriate when the clinical indication and documentation requirements are met.

    Clinical appropriateness

    • Acceptable clinical indication

      • Gingivectomy or gingivoplasty is used for mild to moderate periodontal disease or a documented restorative-access indication.Mild to moderate periodontal disease when treating periodontal disease

The procedure may eliminate suprabony pockets, restore normal gingival architecture when gingival enlargement, asymmetry, or unaesthetic topography is present with normal bony configuration, reduce pocket size, or recontour gingival enlargement, including drug-induced gingival hyperplasia.

      • Gingival disease remains after nonsurgical methods, such as root planing and scaling, have been unsuccessful in removing subgingival plaque and calculus or resolving the relevant periodontal condition.

Nonsurgical therapy should be completed before periodontal surgery.

    • Required periodontal documentation

      • Current, dated six-point periodontal charting obtained within the prior 12 months documents pocket depths of at least 5 mm and was performed after completion of nonsurgical periodontal therapy and/or periodontal maintenance.Minimum pocket depth 5 mm; charting within 12 months
      • Current, diagnostic-quality pretreatment radiographs obtained within the prior 12 months show the periapical area and alveolar crest.Radiographs within 12 months
      • The periodontal diagnosis, including staging and grading, and the clinical indication for the procedure are documented consistent with current American Academy of Periodontology and American Dental Association guidance.
    • Prior therapy and healing

      • Initial periodontal therapy, such as scaling and root planing, has been completed, with at least four weeks allowed before surgical treatment for tissue healing and assessment of nonsurgical treatment response.Minimum 4 weeks

Benefit limitations and related procedure rules

Benefit limitations and procedure-specific rules apply even when the procedure is clinically indicated.

    Benefit and coding limitations

    • Coverage is subject to the member's group contract and the documented site, indication, and associated procedures.
    • Benefits are limited to no more than two quadrants per date of service, although case-by-case exceptions may be allowed.Generally no more than two quadrants per date of service
    • Benefits are generally limited to one periodontal surgical procedure in a 36/60 month period per single tooth or multiple teeth in the same quadrant, when the pocket depth is at least 5 mm.Generally one periodontal surgical procedure per 36/60 months; pocket depth at least 5 mm
    • Restorative access handling

Gingivectomy or gingivoplasty is not appropriate for infrabony pockets, pockets extending below the mucogingival junction, or minimal attached keratinized tissue. It is also contraindicated when the procedure requires access to the alveolar bone.

Gingivectomy performed to correct altered passive eruption is not benefited.

When the procedure removes inflamed or hypertrophied tissue around a partially erupted or impacted tooth, report D7971, excision of pericoronal tissue, rather than gingivectomy.

For non-restorative access, such as anatomical crown exposure, when both gingival tissue and supporting bone are removed, report D4230 or D4231 rather than gingivectomy or gingivoplasty.

Gingivectomy performed within six months of orthodontic treatment is considered cosmetic and is not benefited.

The procedure may be considered unnecessary when diagnostic findings show that existing periodontal support—including the bone level, gingival level, or recession—already provides adequate access.

Gingivectomy associated with crown or restoration placement may be considered incidental when submitted without information supporting its medical or dental necessity. When performed to obtain restorative access, it is generally inclusive of the primary restorative procedure and remains subject to the group contract.

    Step Therapy

    Complete Nonsurgical Therapy Before Surgery

Complete and document initial nonsurgical periodontal therapy before surgical treatment. Gingivectomy or gingivoplasty is generally considered only after nonsurgical methods, such as scaling and root planing, have been unsuccessful in removing subgingival plaque and calculus.

    • Document the nonsurgical treatment performed and its outcome before proceeding to periodontal surgery.
    Documentation Required

    Submit Six-Point Periodontal Charting

Submit current, dated six-point periodontal charting to establish periodontal necessity and document pocket depths of at least 5 mm.

    • Charting must be within 12 months of the date of service.
    • Charting is required after completion of nonsurgical periodontal therapy and/or periodontal maintenance.
    • Record pocket depths at six sites per tooth or the applicable periodontal charting format.

    Clinical Background

Gingivectomy is the surgical removal of diseased gingival tissue, typically in the presence of periodontal disease. Gingivoplasty reshapes otherwise healthy gingiva in the absence of periodontal pockets. These procedures may eliminate suprabony pockets, restore gingival architecture, reduce pocket size, or recontour gingival enlargement, including drug-induced gingival hyperplasia.

Gingivectomy to allow access for crowns or restorations is generally inclusive of the primary restorative procedure.Group-contract dependent

If diagnostic findings show that periodontal support levels, including bone level, gingival level, or recession, already allow adequate access, the procedure may not be necessary or may be considered incidental to crown or restoration placement.

  1. When submitted without an associated restorative procedure, the claim is evaluated under D4211 guidelines.
  2. Specialized technique handling

    • Laser or electrosurgical techniques are treated as adjuncts to treatment and are not eligible for an additional or separate benefit. No separate benefit

    Documentation Required

    Submit Pretreatment Radiographs

Submit current, diagnostic-quality pretreatment radiographs that support the proposed gingivectomy or gingivoplasty and show the required anatomic areas.

    • Radiographs must be dated within 12 months.
    • Include views showing the periapical area and alveolar crest.
    Documentation Required

    Document Diagnosis and Clinical Rationale

Document the periodontal diagnosis, staging and grading, and the clinical rationale for treatment. The record should explain why gingivectomy or gingivoplasty is appropriate for the documented periodontal condition and anatomy.

    • Describe the indication, such as elimination of suprabony pockets or restoration of normal gingival architecture with normal bony configuration.
    • Relate the proposed procedure to the documented pocket depths, periodontal support, and treatment objectives.
    Step Therapy

Document Prior Nonsurgical Therapy and Healing

Document completion of initial periodontal therapy, such as scaling and root planing, before surgical treatment and allow adequate healing time for reassessment.

    • Allow a minimum of four weeks after initial periodontal therapy before surgery so the tissues can heal and the success or failure of nonsurgical therapy can be assessed.
    • Document the therapy date, treated areas, and post-therapy reassessment. Exceptions may be considered case by case.
    Denial Risk

    Check Contraindications and Unsupported Indications

Assess for contraindications and unsupported indications before submitting the procedure. Claims may be denied or considered inappropriate when the anatomy or treatment purpose does not support gingivectomy or gingivoplasty.

    • Do not use for infrabony pockets, pockets extending below the mucogingival junction, minimal attached keratinized tissue, or procedures requiring access to alveolar bone.
    • Gingivectomy for crown or restoration access may be unnecessary when periodontal support, bone level, gingival level, or recession already allows adequate access, and may be considered incidental to the restorative procedure.
    • For nonrestorative anatomical crown exposure requiring removal of gingival tissue and supporting bone, evaluate the applicable osseous procedure codes, such as D4230 or D4231, rather than gingivectomy.
    • Gingivectomy to remove inflamed or hypertrophied pericoronal tissue around partially erupted or impacted teeth is reported as excision of pericoronal tissue, D7971.
    • Gingivectomy for altered passive eruption is not benefited; use within six months of orthodontic treatment may be considered cosmetic.
    Billing Rule

No Separate Laser or Electrosurgery Benefit

Laser and electrosurgical techniques are treatment methods or adjunctive armamentarium, not separately eligible benefits.

    • Do not bill a separate additional benefit solely for use of a laser or electrosurgery.
    Billing Rule

    Restorative Access Is Generally Inclusive

Gingivectomy performed to obtain access for a crown or restoration is generally considered inclusive of the primary restorative procedure, subject to the member’s group contract.

    • Do not routinely submit a separate gingivectomy benefit with the associated crown or restoration.
    • When submitted without an associated restorative procedure, evaluate the claim under the applicable D4211 guidelines.
    • Benefits remain group-contract dependent.
