    Covered Services, Limitations, and Exclusions

    Covered clinical indications

Gingivectomy or gingivoplasty is considered appropriate when the clinical indication and documentation requirements are met.

    Covered clinical indications and documentation

    • Qualifying clinical indication

      • The procedure is used to eliminate suprabony pockets or restore normal gingival architecture when gingival enlargement, asymmetrical or unaesthetic gingival topography, and normal bony configuration are present.
      • The procedure is used for pocket elimination or gingival recontouring in the presence of suprabony pockets with normal bony configuration.
      • Nonsurgical therapy, such as scaling and root planing, has been unsuccessful in removing subgingival plaque or calculus, and surgical treatment is clinically indicated.
      • Loose or diseased gingival tissue is removed to reduce pocket size.
      • Excess gingival tissue caused by drug-induced gingival hyperplasia, such as Dilantin therapy, requires resculpting.
    • Required periodontal documentation

      • Current, dated, six-point periodontal charting is submitted and documents periodontal pocket depths of at least 5 mm.Obtained within 12 months; pocket depth >= 5 mm
      • The periodontal charting was obtained after completion of nonsurgical periodontal therapy and/or periodontal maintenance.
      • Current, diagnostic-quality pretreatment radiographs are submitted and show the periapical area and alveolar crest.Obtained within 12 months
      • The periodontal diagnosis includes staging and grading consistent with current American Academy of Periodontology and American Dental Association guidance.
    • Prior nonsurgical therapy

      • Initial periodontal therapy, such as scaling and root planing, has been completed before surgical treatment.
      • The record documents reassessment of the outcome or failure of nonsurgical therapy after tissue healing.Minimum 4 weeks before surgical treatment

    Benefit and frequency limitations

Coverage is subject to applicable group-contract, quadrant, and frequency limitations.

    • The service is within the member's applicable group-contract benefit limitations.

Group contracts control benefit availability and limitations.

    • No more than two quadrants are treated per date of service. Two quadrants per date of service

Exceptions may be allowed on a case-by-case basis.

    • For the treated single tooth or multiple teeth in the same quadrant, no more than one periodontal surgical procedure is billed during the applicable frequency period.Typically one procedure in a 36/60-month period

The applicable group contract controls the frequency period, and the treated tooth or teeth must have pocket depths of at least 5 mm.

    Contraindications and noncovered uses

The following findings or uses are contraindicated, nonbenefited, cosmetic, inclusive to another service, or require a different procedure code.

    • The procedure treats an infrabony pocket or a pocket extending below the mucogingival junction.
    • The procedure is performed where there is minimal attached keratinized tissue.
    • The procedure requires access to alveolar bone.
    • Gingivectomy is performed to correct altered passive eruption.

      This use is not benefited.

    • Gingivectomy is performed within six months of orthodontic treatment.

      This use is considered cosmetic.

Gingivectomy or gingivoplasty may be appropriate for mild to moderate periodontal disease or qualifying gingival enlargement when the procedure eliminates suprabony pockets, reduces pocket size, removes loose or diseased tissue, restores normal gingival architecture, or treats persistent subgingival plaque and calculus after unsuccessful nonsurgical therapy. Required support includes current six-point periodontal charting obtained within 12 months that documents pocket depths of at least 5 mm, diagnostic-quality pretreatment radiographs obtained within 12 months, a periodontal diagnosis with staging and grading, and completion of initial nonsurgical periodontal therapy with at least 4 weeks for healing and reassessment.

Contraindications include infrabony pockets, pockets extending below the mucogingival junction, minimal attached keratinized tissue, and procedures requiring access to alveolar bone. Coverage is also subject to applicable group-contract limitations, including generally no more than two quadrants per date of service and typically one periodontal surgical procedure per tooth or same-quadrant teeth within a 36/60-month period when the pocket depth is at least 5 mm.

Gingivectomy performed for the purpose of correcting altered passive eruption is not benefited.

Gingivectomy performed within six months of orthodontic treatment is considered cosmetic.

Laser and electrosurgical techniques are considered adjuncts or treatment armamentarium. They are not eligible for an additional or separate benefit.

Gingivectomy performed to allow access for crowns or restorations is generally inclusive to the primary restorative procedure and is subject to group-contract terms.

When diagnostic findings show that periodontal support, bone level, gingival level, or recession already provides adequate access, gingivectomy may be considered not medically necessary.

A gingivectomy submitted without an associated restorative procedure may be considered incidental or unsupported when the submitted information does not establish clinical need. Such submissions are evaluated under D4211 guidelines and should include the clinical indication and periodontal support demonstrating necessity.

    Documentation and Submission Requirements

    Step Therapy

    Complete nonsurgical therapy first

Complete nonsurgical periodontal therapy, such as scaling and root planing, before surgical treatment and allow a minimum of four weeks for healing and reassessment.

    Documentation Required

    Submit current six-point charting

Submit current, dated, six-point periodontal charting obtained within 12 months. The charting must document pocket depths of at least 5 mm and must be obtained after completion of nonsurgical periodontal therapy and/or periodontal maintenance.

    Documentation Required

    Provide pretreatment radiographs

Submit current, diagnostic-quality pretreatment radiographs obtained within 12 months that show the periapical area and alveolar crest.

    Clinical Context

Gingivectomy is the surgical removal of diseased or loose gingival tissue, generally in the presence of periodontal disease, and may be used to eliminate suprabony pockets or reduce pocket size. Gingivoplasty reshapes otherwise healthy gingiva, generally in the absence of periodontal pockets, to restore normal gingival architecture or recontour excess or asymmetrical tissue. Both procedures may be considered for gingival recontouring when the underlying bony configuration is normal; access for restorative treatment may be addressed separately under the policy's restorative-inclusion rules.

Laser or electrosurgical technique is submitted for an additional or separate benefit.

These techniques are adjuncts to treatment and are not eligible for an additional benefit.

  1. Gingivectomy is performed to allow access for a crown or restoration.

The service is generally inclusive to the primary restorative procedure and is subject to group-contract terms.

  2. Diagnostic findings show that periodontal support level, bone level, gingival level, or recession already allows adequate access.

The procedure may not be medically necessary.

  3. The procedure is submitted without an associated restorative procedure and the submitted information does not establish clinical necessity.

Evaluate under D4211 guidelines and document the clinical indication and periodontal support.

  4. Inflamed or hypertrophied tissue around a partially erupted or impacted tooth is treated.

Report excision of pericoronal tissue, D7971, rather than gingivectomy or gingivoplasty.

  5. Non-restorative anatomical crown exposure involves removal of gingival tissue and supporting bone.

Use D4230 or D4231 rather than gingivectomy or gingivoplasty.

Additional conditionsBenefits are group-contract dependent and generally require a pocket depth of 5 mm or greater
    Documentation Required

    Document failed nonsurgical therapy

Document completion of initial periodontal therapy, such as scaling and root planing, with at least four weeks allowed for tissue healing before surgical treatment. The record should support the outcome or failure of nonsurgical therapy and the need for surgical treatment.

    Documentation Required

    Include periodontal staging and grading

Provide a periodontal diagnosis that includes staging and grading consistent with current American Academy of Periodontology and American Dental Association guidance.

    Documentation Required

    Document clinical rationale

When the procedure is submitted without an associated restorative procedure, evaluate it under D4211 guidelines and document the clinical indication and periodontal support demonstrating that the procedure is necessary.

    Denial Risk

    Check for contraindicated anatomy

Do not submit gingivectomy or gingivoplasty for infrabony pockets, pockets extending below the mucogingival junction, minimal attached keratinized tissue, or procedures requiring access to alveolar bone; these conditions are contraindicated under the policy.

    Denial Risk

    Support medical necessity

Coverage may be denied or limited when charting does not document pocket depths of at least 5 mm, required post-nonsurgical-treatment documentation is absent, or diagnostic findings do not establish medical necessity. The procedure may also be unnecessary when periodontal support, bone level, gingival level, or recession already allows adequate access.

    Denial Risk

    Apply restorative and cosmetic limitations

Gingivectomy performed to allow access for crowns or restorations is generally inclusive of the primary restorative procedure and is subject to group-contract terms. Altered passive eruption is not benefited, and gingivectomy within six months of orthodontic treatment is considered cosmetic.

    Billing Rule

Do not bill a separate laser benefit

Laser and electrosurgical techniques are adjuncts to treatment and are not eligible for an additional or separate benefit. Do not bill these techniques as a separate covered service.

    Billing Rule

    Report the correct procedure code

Report inflamed or hypertrophied tissue around partially erupted or impacted teeth as excision of pericoronal tissue, code D7971. For non-restorative anatomical crown exposure involving removal of gingival tissue and supporting bone, use D4230 or D4231 rather than gingivectomy or gingivoplasty.
