    Clinical Coverage Criteria

    Clinical coverage criteria

Considered appropriate when all applicable clinical, documentation, and procedure limitations below are met.

Gingival flap or apically positioned flap criteria

    • Clinical indication and prior therapy: Treatment is for mild to severe periodontal disease after nonsurgical methods, such as scaling and root planing, have been unsuccessful in removing subgingival plaque or calculus.
    • Required periodontal charting: Current, dated, post-initial-therapy six-point periodontal charting is available after nonsurgical periodontal therapy or periodontal maintenance and documents pocket depths of at least 5 mm.Charting within 12 months; pocket depth minimum 5 mm
    • Required radiographs: Current, dated pretreatment radiographs demonstrate the periapical area and an undistorted image of the alveolar crest.Radiographs within 12 months
    • Anatomic and procedural indication: The procedure is indicated for supra-bony pocket depths requiring increased access to root surfaces. A soft tissue flap is reflected or resected to permit root-surface debridement and removal of granulation tissue; osseous recontouring is not performed.Supra-bony pocket depths
    • Eligible treatment site and tissue condition: The periodontal defect involves natural teeth, and the procedure is performed in the presence of minimal attached keratinized tissue.Natural teeth; minimal attached keratinized tissue
    • Periodontal diagnosis: The periodontal diagnosis includes staging and grading consistent with current American Academy of Periodontology and American Dental Association guidance.
    • Treatment location limitation: The procedure is not performed for pockets extending below the mucogingival junction.
    • Quantity limitation: Gingival flap procedures are limited to two quadrants per date of service. Two quadrants per date of service

Exceptions may be allowed on a case-by-case basis.

    • Same-day procedure limitation: When multiple similar procedures are performed on the same date of service, the less complex procedure is considered inclusive to the primary procedure.
    • Adjunctive laser limitation: Laser use is considered an adjunct to treatment and is not eligible for an additional or separate benefit.

    Exploratory procedure exception

A gingival flap procedure may be benefited for the following exploratory indication:

Exploratory indication: The procedure is performed for exploratory purposes to determine the presence of a cracked tooth or fractured root.

Coverage is limited to periodontal defects involving natural teeth. Gingival flap procedures performed around dental implants are not considered for coverage.

A gingival flap procedure or apically positioned flap is not performed for periodontal pockets extending below the mucogingival junction.

Laser use may support the covered periodontal treatment as an adjunct, but it is not eligible for an additional or separate benefit.

When multiple similar procedures are performed on the same date of service, the less complex procedure is considered inclusive to the primary procedure. For example, scaling and root planing performed with a gingival flap procedure is not separately eligible when it is the less complex procedure.

    Documentation and Denial Risks

    Documentation Required

    Post-Treatment Periodontal Charting

Submit current, dated periodontal charting obtained after completion of initial nonsurgical periodontal therapy or periodontal maintenance. The charting must document six sites per tooth and pocket depths of at least 5 mm.

    • Within 12 months
    • Post-initial-therapy six-point charting
    • Minimum pocket depth: 5 mm
    Documentation Required

    Pretreatment Radiographs Required

Submit current, dated pretreatment radiographs obtained within 12 months. The images must show the periapical area and an undistorted view of the alveolar crest.

    • Radiographs must be dated and current
    • Show the periapical area
    • Show the alveolar crest without distortion

    Procedure Background

A gingival flap procedure reflects the gingival soft tissue to provide access for root-surface debridement and removal of granulation tissue. An apically positioned flap is used to maintain an adequate zone of keratinized tissue, with the goal of achieving pocket elimination. These procedures provide soft-tissue access and do not involve manipulation of the periodontal bone.

ExceptionExceptions are allowed on a case-by-case basis
Exceeding the limitMore than two quadrants require a case-by-case exception
    Documentation Required

    Operative Clinical Documentation

Maintain operative chart notes that demonstrate a soft-tissue flap was reflected or resected, or was planned, to permit debridement of the root surface and removal of granulation tissue. Osseous recontouring is not performed with this procedure.

    • Document flap reflection, resection, or planned access
    • Document root-surface debridement and granulation-tissue removal
    • Do not represent osseous recontouring as part of this procedure
    Documentation Required

    Periodontal Diagnosis Required

Document a periodontal diagnosis that includes staging and grading consistent with current American Academy of Periodontology (AAP) and American Dental Association (ADA) guidance.

    • Include periodontal staging
    • Include periodontal grading
    • Use current AAP and ADA guidance
    Denial Risk

    Document Failure of Nonsurgical Therapy

The procedure is intended for diseased gingiva when nonsurgical therapy, such as scaling and root planing, was unsuccessful in removing subgingival plaque and calculus. Denial risk increases when failure of nonsurgical therapy is not documented or required post-treatment charting is absent.

    • Document unsuccessful nonsurgical periodontal therapy
    • Document completion of nonsurgical therapy or periodontal maintenance
    • Include required post-treatment periodontal charting
    Denial Risk

    Insufficient Periodontal Charting

Claims may be denied when current, dated six-point periodontal charting does not show pocket depths of at least 5 mm.

    • Charting must be within 12 months
    • Chart six sites per tooth
    • Record pocket depths of at least 5 mm
    Denial Risk

    Missing or Inadequate Radiographs

Claims may be denied when required current, dated pretreatment radiographs are missing, older than 12 months, or do not adequately show the periapical area and an undistorted alveolar crest.

    • Radiographs must be current and dated
    • Images must show the periapical area
    • Images must show an undistorted alveolar crest
    Denial Risk

    Qualifying Periodontal Defect

Use the procedure for supra-bony periodontal pockets where increased access to root surfaces is needed. It is not a substitute for treatment involving osseous recontouring, which is not accomplished with this procedure.

    • Indication: supra-bony pocket depths
    • Document the need for increased root-surface access
    • Osseous recontouring is not included
    Denial Risk

Excluded Treatment Site or Pocket Location

Gingival flap procedures are considered only for periodontal defects involving natural teeth. They are not considered when performed around implants or for pockets extending below the mucogingival junction.

    • Eligible site: natural teeth only
    • Excluded site: around implants
    • Excluded location: pockets extending below the mucogingival junction
    Billing Rule

    Quadrant Limit Exceeded

Gingival flap procedures are limited to two quadrants per date of service. Treatment involving more than two quadrants on the same date requires a case-by-case exception and may be denied without supporting review.

    • Maximum: two quadrants per date of service
    • More than two quadrants require a case-by-case exception
    Billing Rule

    Multiple Same-Day Procedures

When multiple similar procedures are performed on the same date of service, the less-complex procedure is inclusive to the primary procedure and is not separately payable. This includes scaling and root planing performed with a gingival flap procedure.

    • Apply the rule to multiple similar same-day procedures
    • Scaling and root planing may be inclusive when performed with the flap procedure
    • Do not separately bill the less-complex procedure
    Supra-bony Pocket
LocationA periodontal pocket located coronal to the alveolar bone crest
Access requirementThe pocket depth must require increased access to root surfaces
Related treatment limitationOsseous recontouring and bone grafting are not required for this indication
