OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Universal Initial Criteria

Initial approval requires ALL of the following universal criteria:

    Universal initial criteria

    • Member is at least 18 years of age.Age ≥18 years
    • Member is receiving vitamin A supplementation at the recommended daily allowance.Recommended daily allowance
    • Amvuttra is not used in combination with another transthyretin-reducing or transthyretin-stabilizing agent.

Examples include tafamidis, patisiran, acoramidis, inotersen, and eplontersen.

    Initial Therapy: hATTR Polyneuropathy

For hATTR polyneuropathy, ALL of the following criteria must be met:

    hATTR polyneuropathy

    • Member has a definitive diagnosis of hATTR amyloidosis documented in a proband with suggestive findings, including imaging or histopathology findings of amyloidosis, and a heterozygous pathogenic or likely pathogenic TTR variant identified by molecular genetic testing.
    • At least TWO qualifying neuropathy findings

      • Subjective member symptoms are suggestive of neuropathy.
      • Abnormal nerve conduction studies are consistent with polyneuropathy.
      • Abnormal neurological examination is suggestive of neuropathy.
    • Peripheral neuropathy is attributed to hATTR, and other causes of neuropathy have been excluded.

    Initial Therapy: ATTR Cardiomyopathy

For wild-type or hereditary ATTR cardiomyopathy, ALL of the following criteria must be met:

    ATTR cardiomyopathy

    • Member has evidence of cardiac involvement by echocardiography with an end-diastolic interventricular septal wall thickness greater than 12 mm.> 12 mm

For hATTR polyneuropathy, response is stabilization or improvement in signs and symptoms of neuropathy or MRC muscle strength. For ATTR-CM, response is stabilization or improvement in the frequency of cardiovascular-related hospitalizations or urgent heart failure visits, or total distance walked during the 6-minute walk test.

Concurrent transthyretin therapy is excluded. Amvuttra must not be used in combination with another transthyretin (TTR)-reducing or TTR-stabilizing agent, including tafamidis, patisiran, acoramidis, inotersen, or eplontersen.

    Background

Amvuttra (vutrisiran) is a subcutaneous transthyretin-directed therapy for adults with hereditary transthyretin-mediated amyloidosis presenting as polyneuropathy or with wild-type or hereditary transthyretin amyloidosis presenting as cardiomyopathy. Coverage requires objective confirmation of the applicable amyloidosis manifestation, baseline functional assessment, and ongoing evidence of treatment response and tolerability.

    Amvuttra Renewal

Renewal requires ALL of the following:

    Amvuttra renewal

    • Member continues to meet the universal and relevant indication-specific initial criteria.
    • Member has no unacceptable toxicity from Amvuttra.

Examples of unacceptable toxicity include ocular symptoms related to vitamin A deficiency, such as night blindness.
