    Coverage Criteria

Initial Therapy – Acute HAE Attack

Covered for initial treatment of an acute hereditary angioedema attack when ALL applicable criteria are met:

    ALL of the following

    • Member is at least 13 years of age.
    • Requested agent is prescribed by, or in consultation with, a specialist in allergy, immunology, hematology, pulmonology, or medical genetics.
    • Provider confirms that the member has been counseled to avoid possible medication triggers for HAE attacks, including estrogen-containing medications and ACE inhibitors.
    • Member has a history of ONE or more of the following:

      • Recurrent swelling of mucous membranes, skin surface, or subcutaneous tissue.
      • Recurrent abdominal attacks with severe abdominal pain, nausea, vomiting, or diarrhea.
      • Episodes of upper-airway edema, including mouth, tongue, or laryngeal edema.
      • Prodromal signs or symptoms preceding angioedema attacks, such as anxiety, nausea, paresthesia, or fatigue.
      • Positive family history of HAE.
    • Diagnosis of HAE is confirmed by ONE of the following pathways:

      • HAE due to C1-inhibitor deficiency (HAE-C1-INH) Type 1 or 2:

        • At least ONE confirming finding:

          • Current or historical C1-INH antigenic level is below the lower limit of normal as defined by the laboratory performing the test.
          • Current or historical C1-INH functional level is below the lower limit of normal as defined by the laboratory performing the test.
          • Genetic testing confirms pathogenic mutation(s) in SERPING1.
      • HAE with normal C1-INH (HAE-nC1-INH):

        • Current or historical C1-INH level is normal as defined by the laboratory performing the test.
        • At least ONE confirming finding:

    Continuation Therapy

Continuation therapy is covered when ALL of the following criteria are met:

    ALL of the following

    • Member continues to meet the universal and other indication-specific applicable criteria identified for initial therapy.
    • Member has experienced clinical benefit from therapy, such as reduced attack severity or shortened attack duration.
    • Member has no unacceptable toxicity from the drug, including severe hypersensitivity reactions (including anaphylaxis) or serious arterial or venous thromboembolic events.
    • The cumulative amount of medication(s) the member has on hand for acute HAE treatment is considered when authorization is determined.
    • The authorization provides a sufficient quantity for the member to have a cumulative amount of HAE medication(s) on hand to treat up to 4 acute attacks per 4 weeks for the duration of the authorization.

Coverage is not provided when the member has an FDA-labeled contraindication to Ruconest, including a known or suspected allergy to rabbits or rabbit-derived products, or a history of immediate hypersensitivity reactions, including anaphylaxis, to C1 esterase inhibitor preparations.

Ruconest must not be used in combination with another agent indicated for the treatment of acute hereditary angioedema attacks. Requests for combination duplicate therapy may be appropriate in some situations and will be reviewed on a case-by-case basis.

Coverage is excluded when other causes of angioedema have not been ruled out, including drug-induced angioedema, mast cell-mediated angioedema, or acquired angioedema due to C1-INH deficiency. Coverage is also excluded when the applicable clinical history, diagnostic confirmation, specialist involvement, counseling, age, benefit, toxicity, or quantity requirements are not met.
