    Policy Background

Benlysta (belimumab) is an intravenous biologic used with standard therapy for active systemic lupus erythematosus (SLE) and active lupus nephritis. The policy distinguishes active SLE without active lupus nephritis from kidney-biopsy-confirmed Class III, IV, or V lupus nephritis and requires appropriate background therapy.

    Initial Therapy Criteria

    Initial criteria

Initial authorization requires an FDA-labeled or otherwise supported indication, appropriate age support, universal criteria, and a qualifying SLE or lupus nephritis pathway.

    Initial Benlysta criteria

    • Age and indication support

      • The member's age is within FDA labeling for the requested indication.
      • There is support for using the requested agent for the member's age and requested indication.
    • Universal criteria

      • The prescriber is a specialist in the area of the member's diagnosis, such as a rheumatologist or nephrologist, or has consulted with a specialist in the area of the member's diagnosis.

    Continuation Therapy Criteria

    Renewal criteria

Renewal requires continued universal and applicable concomitant-therapy criteria plus clinical benefit.

    Benlysta renewal criteria

    • The member continues to meet the universal criteria identified in the initial criteria.
    • The member continues to meet applicable indication-specific concomitant-therapy requirements identified in the initial criteria, excluding prerequisite therapy.
    • The member has had clinical benefit with Benlysta.

    Step Therapy Requirements

    StepRequired therapyCriteria
1Hydroxychloroquine plus one corticosteroid or immunosuppressive agent (azathioprine, methotrexate, mycophenolate, or cyclophosphamide)For active SLE without active lupus nephritis, the member must have tried and had an inadequate response, intolerance, or hypersensitivity to hydroxychloroquine and to one corticosteroid or immunosuppressive agent. An applicable FDA-labeled contraindication may substitute for the relevant trial requirement. The member must continue standard SLE therapy with Benlysta.
    StepRequired therapyCriteria
2Two standard therapy courses before Benlysta is used with LupkynisThe member must have tried and had an inadequate response to two standard therapy courses, then use Benlysta with Lupkynis plus mycophenolate.

    SLE Prerequisite Therapy

For active SLE without active lupus nephritis, document prior hydroxychloroquine therapy and prior treatment with at least one corticosteroid or immunosuppressive agent, unless the applicable FDA-labeled contraindication exception applies.

    • Inadequate response, intolerance, or hypersensitivity to hydroxychloroquine, or an FDA-labeled contraindication to hydroxychloroquine
    • Inadequate response, intolerance, or hypersensitivity to one corticosteroid or immunosuppressive agent—azathioprine, methotrexate, mycophenolate, or cyclophosphamide—or FDA-labeled contraindications to all listed agents
    • Member will continue standard SLE therapy in combination with Benlysta
    Step Therapy

    Lupkynis Sequencing Requirement

For lupus nephritis, if Benlysta will be used with Lupkynis, document inadequate response to two standard therapy courses before the combination is authorized.

    • Examples of standard courses include corticosteroids plus Benlysta with mycophenolate, azathioprine, or cyclophosphamide; and corticosteroids plus Lupkynis with mycophenolate
    • The planned regimen must use Benlysta with Lupkynis plus mycophenolate
    Documentation Required

    Indication and Specialist Documentation

The request must identify the diagnosis and requested indication, the member’s age, and the FDA-labeled age basis or supporting evidence for use at that age. Submit prescriber credentials or documentation of consultation with an appropriate specialist, such as a rheumatologist or nephrologist.

    • Requested indication: active SLE without active LN or active LN
    • Member age and FDA-labeled age support
    • Specialist prescriber or specialist consultation documentation
    Documentation Required

    • Hydroxychloroquine history or contraindication
    • One corticosteroid or immunosuppressive-agent history or, where applicable, contraindications to all listed agents
    • Document the therapy names, outcomes, intolerance, hypersensitivity, or contraindication rationale
    Documentation Required

    Lupus Nephritis Documentation

For active lupus nephritis, submit kidney-biopsy documentation confirming Class III, IV, or V lupus nephritis and identify the planned background immunosuppressive regimen.

    • Kidney biopsy must confirm Class III, IV, or V lupus nephritis
    • Background therapy must include an appropriate regimen, such as a corticosteroid plus mycophenolate, azathioprine, or cyclophosphamide
    Documentation Required

    Lupkynis Combination Documentation

When Benlysta is requested with Lupkynis, document two prior standard therapy courses with inadequate response and confirm the planned combination of Benlysta, Lupkynis, and mycophenolate.

    • Submit the two prior standard-course regimens and treatment outcomes
    • Confirm planned Benlysta plus Lupkynis plus mycophenolate therapy
    Documentation Required

    Combination Therapy Evidence

Benlysta may be used with another immunomodulatory agent only when the prescribing information does not limit the combination and supporting evidence is submitted.

    • Submit prescribing-information support showing combination compatibility
    • Submit clinical-trial, phase III study, or guideline support for the combination therapy
    Documentation Required

    Renewal Documentation

For renewal, submit evidence that the member continues to meet the applicable universal and concomitant-therapy criteria and has experienced clinical benefit with Benlysta.

    • Document ongoing eligibility and required concomitant therapy
    • Document clinical response or other clinical benefit attributable to Benlysta
