Added notation that many HCPCS codes (S2230, V5070, V5080, V5095, V5150, V5190, V5230, V5242–V5253, V5262, V5263, and V5267) are not on the State of Idaho Medicaid Fee Schedule and therefore may not be covered by Idaho Medicaid.
