OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Initial medical-necessity criteria

    • Age: The member is 3 through 18 months of age.3 through 18 months
    • Diagnosis and treatment plan: The orthosis is part of a treatment plan for a documented diagnosis of synostotic plagiocephaly.Documented synostotic plagiocephaly

    Additional device or replacement

Additional devices beyond the once-per-lifetime benefit may be considered when ALL of the following are documented:

    Additional-device criteria

    • Initial indication: The initial device was obtained to treat synostotic plagiocephaly.Synostotic plagiocephaly
    • Treatment response: Treatment with the device has been effective.Effective treatment
    • Growth-related need: The new device is needed due to growth.Need due to growth

    Not medically necessary indications

Not medically necessary as a treatment for either of the following:

    Non-covered indications

    • Positional condition: Non-synostotic or positional plagiocephaly.

      Considered cosmetic.

    • Brachycephaly.

      Efficacy is not clearly documented.

Non-synostotic or positional plagiocephaly is excluded as a treatment indication because it is considered cosmetic.

Brachycephaly is excluded as a treatment indication because the efficacy of cranial molding orthosis for this condition is not clearly documented.

Cranial molding orthosis is considered not medically necessary for non-synostotic or positional plagiocephaly.

For a device beyond the once-per-lifetime benefit, submit documentation showing that the initial device was obtained to treat synostotic plagiocephaly, treatment has been effective, and the new device is needed due to growth.

    • All listed documentation elements are required for consideration of the additional device.
    Denial Risk

    Failure to Meet Coverage Criteria

Requests that do not meet the established coverage criteria are referred to a TCHP Medical Director or Physician Reviewer. The Denial Policy will be followed for requests that remain unsupported or do not meet criteria.

    Prior Authorization

    Additional Device Beyond Lifetime Limit

An additional cranial molding orthosis beyond the once-per-lifetime limit requires prior authorization and documentation supporting all required conditions for consideration.

    • The initial device treated synostotic plagiocephaly.
    • Treatment with the initial device has been effective.
    • The additional device is needed because of member growth.
